1.The Clinical Study of Chemonucleolys in Herniated Nucleus Pulposus of Lumbar Spine.

Chemonucleolysis with chymopapain has been shown to be an effective alternative to surgery for the treatment of Sciatica secondary to lumbar disc herniation after failure of conservative treatment. 92 lumbar herniated disc patients had been treated by chemonucleolysis in Eul Ji General Hospital during 3 years from March 1988 to 1991 and were evaluated clinically. 1) Of 92 patients, 60 patients(65%) were between 20 and 30 years old age group and male was predominent. 2) The lesion was the most common in L4-5space(52 patients, 56.6%). 3) The results were better in the younger group and the more limited straight leg raising group. 4) The excellent and good results were obtained in 82.6%. 5) In noncompensation cases, 69 patients(88.0%) had marked improvement, but in compensation cases, 7 patients(50%) had marked improvement. 6) In conclusion, above finding suggest that chemonucleolysis is a safe and effective nonsurgical method for treatment of herniated lumbar disc before surgical intervention.

2.A Case of Dumbbell-Shaped Neurobibroma in Cauda Equina.

A case of neurofibroma in cauda equina which had been misdiagnosed as lumbar herniated nucleus puposus 6 years ago in other hospital was presented. In January 1984, the patient had taken the operation of total laminectomy and disckectomy at L2~3 level and symtoms were relieved thereafter. However severe low back pain and right sciatic radiation recurred in April 1990. In this hospital, lumbar myelography and spin C-T examination revealed the intradural extramedullary mass shadow at L2~3 level. On surgery, the dumbbell-shaped neurofibroma at the site was removed.

3.A Clinical Study and Analysis of Chemonuecleolysis Versus Automated Percutaneous Lumbar Discectomy in Herniated Nucleus Pulposus of the Lumbar Spine.

The authors reviewed the radiographs and medical records of 134 consecutive patients who underwent chemonucleolysis and automated percutaneous lumbar discectomy(APLD) between March 1990 and December 1994 at our institute. Among the 34 consecutive patients, 90 patients underwent chemonucleolysis and the other remaining patients underwent APLD using a nucleotome. The results are summarized as follows: 1) In both groups, predominent patients were of young age and males. 2) The most common lesion was in L4-5 interspace in both groups 3) The posteior lateral type showed high incidence in APLD group. 4) The results were better in the younger age group and when spinal CT demonstrated posterior lateral type in both groups. 5) The results showed relatively good success rate of 84.4% in chemonucleolysis group and 88.6% in APLD group. 6) Better results can be anticipated when patients are carefully selected and operated on with a higher skillful technique.

4.Antioxidant Effects of Haematococcus Extracts for Ischemia-Reperfusion Injury on Liver of Mice.

PURPOSE: An IRI (Ischemia-reperfusion injury) of the liver is one of the main problems in major liver surgery, liver transplantation and liver trauma, which can lead to liver failure. The mechanism of a liver IRI is supposed to be highly related to the oxygen free radicals (Should the read, "free oxygen radicals"?) made in the course of the damage process. Therefore, antioxidants must be one of the most effective therapeutic modalities for liver IRI. Haematococcus pluvialis contains a lot of astaxanthin, which is known of powerful antioxidant. Since the use of synthetic astaxanthin is not permitted to humans, only naturally produced astaxanthin, such as Haematococcus, is a candidate for use. In this report, the antioxidant effects of a Haematococcus extract has been studied and compared with other antioxidants and synthetic astaxanthin. METHODS: To evaluate the effect of the Haematococcus extract in a liver IRI, the mouse liver IRI model was employed. Following ischemia for 60 minutes, three reperfusion periods were used; 3, 24 and 48 hours. The MDA (malondialdehyde) values in the liver, lungs and serum were all evaluated, and two types of Haematococcus extract compared with other antioxidants and synthetic astaxanthin. RESULTS: In all tissues and serum, the antioxidants all produced lower levels of MDA than in the control. The oil- extracted Haematococcus group had much lower MDA levels compared to the biomass Haematococcus and other antioxidants groups. The effects of synthetic astaxanthin are similar to those of oil-extracted Haematococcus. CONCLUSION: The Haematococcus extract has very potent antioxidant effects compared with vitamin E and Superoxide dismutase (SOD) in protecting the liver from IRI. Oil-extracted Haematococcus is better than biomass Haematococcus, and was even comparable to the effects of synthetic astaxanthin.

5.Marginal fit related to margin types of glass infiltrated alumina core fabricated from aqueous-based alumina tape.

STATEMETN OF PROBLEM: In-Ceram system is one of the all-ceramic crowns that can be used in anterior 3 unit fixed partial dentures and posterior single crowns. The alumina core used in In-Ceram system is manufactured using slip-casting technique. The slip-casting technique is difficult and technique sensitive. To improve this problem, tape-casting method was introduced into dentistry. There were no studies to examine the effect of margin design on the margin fit-ness of all-ceramic crowns fabricated from alumina tape. PURPOSE: The purpose of this study was to compare the marginal fitness of glass infiltrated alu-mina core fabricated from aqueous-based alumina tape according to different margin types (90 .,110 ., 135 .shoulder margin). MATERIAL AND METHOD: Three upper central resin incisors were prepared with 90 ., 110 .and 135 .shoulder margins for all-ceramic crowns, respectively. The resin teeth were duplicated and master die and special plaster die were made as usual. After alumina cores were fabri-cated from aqueous-based alumina tape, cores were cemented to each 15 epoxy dies replicated from three resin teeth with resin cement. These cemented cores were embedded in epoxy resin. Specimens were cut mesiodistally and buccolingually. Marginal gap and discrepancy were measured under microscope. RESULTS: The marginal gap and discrepancy of 90.marginal angle was 75.1 mu m, 86.6 mu m, 110 .marginal angle was 41.5 mu m, 50.7 mu m and 135 .marginal angle was 51.7 mu m, 54.2 mu m, respectively. The smallest value was seen in 110 (angle, which was statistically significant com-pared to that of 90 .angle (p<0.05). CONCLUSION: Marginal fitness of alumina cores made of alumina tape with 110 .shoulder mar-gin was best and others were clinically acceptable.

BACKGROUND: The classical treatment of the cervical cancer is surgery, radiotherapy, chemotherapy. Even though the improvement of treatment successful rate, conventional therapy has some limitations. Recent cutting edge of cancer therapy has been developed in gene level including understand the biological characteristics of the cancer cells, enhance the human immune response, suppress the cancer cell proliferation. Therefore, the gene therapy is proposed to new treatment strategy. PURPOSE: The transfection efficiency of cervical cancer cell lines and cervical cancer cell line xerografted nude mouse was investigated by transfection of liposome and infection of adenovirus mediated suppressor(p53) and reportor(LacZ) gene. METHOD: The cervical cancer cell lines was used in this study were CaSki, SiHa (HPV16 positive, wild type p53 gene), HeLa, HelaS3(HPV18 positive, wild type p53 gene) and C33A, HT3(HPV negative, mutant p53). Direct plasmide and AdCMVp53 gene transfection was performed by using liposome system (pRcCMVLacZ / lipofectin, FuGene 6, Ca-phosphate). LacZ gene was used as the reportor gene for the transfection efficiency evaluation. Expression of p53 in cell lines and tumor tissue was confirmed by western blot and immunohistochemical staining. Xenografted nude mouse of SiHa cell line was infected by AdCMVp53 and AdCMVLacZ. Transfection efficiency was observed by same as above. RESULTS: In cervical cancer cell lines, gene transfection using liposome system(pRcCMVLacZ/lipofectin, FuGene 6, Ca-phosphate)revealed different transfection efficiency, especially pRcCMVLacZ in Fugene 6 showed 18-40% of high transfection efficiency in 6 cervical cancer cell lines by X-gal staining and AdCMVp53 showed 95-98% of the high transfection efficiency in HeLa, C33A. AdCMVp53 was significantly expressed at 2-5days after injection xenografted nude mouse on the western blot and transfection efficiency was 19.79+/-5.36, 26.26+/-11.69, 14.77+/-3.98,15.99+/-6.43%(day1-5). AdCMVLacZ were found to immuno- histochemistry analysis, in vivo transfection efficiency was 61.26+/-4.66,59.63+/-9.12, 29.46+/-14.33, 31.73+/- 22.64%(day 1-5) atx200 and 88.65+/-8.65, 70.85+/-20.94, 40.75+/-25.44, 48.21+/-10.97% (day 1-5) atx400. CONCLUSION: As a results, adenovirus-mediated transfection efficiency was higher in vivo experiment compared to cell lines. These high efficiency of adenovirus-mediated suppressor gene(p53) could become a significant meaningful data gene therapy strategy both transgenic mice and cervical cancer cell lines.
