1.Multiple Pyogenic Granulomas in Port-wine Stain after Laser Therapy.

Pyogenic granuloma (PG) and port-wine stain (PWS) are vascular lesions that rarely occur in association. While PWSs are congenital capillary vascular malformations, PGs are acquired benign vascular proliferations. Although the pathogenesis of PG is unknown, many authors feel that they represent reactive hyperplasias, especially as they are often seen in response to trauma. We report the multiple pyogenic granulomas developed in a port-wine stain following Nd-YAG laser therapy.

2.Etretinate Therapy of Epidermal Nevi.

Two cases of epidermal nevi treated with systemic etretinate are presented. Case 1, a 21-year-old female, had been suffered from brownish verrucous papules and plaques on the left inguinal area, thigh, lower leg and dorsum of the foot since her first month of life; and case 2, a 4-year-old boy, had been suffered from brownish verrucous papular plaques on the left chest, axilla and back since birth. In both cases, there was no family history of similar disease. On histopathological examination, case 1 showed the findings of epidermolytic. hyperkeratosis but case 2 did not, with the exception of clumping of keratohyaline granules. In the treatment with systemic etretinate, we observed marked clinical improvement in case 1, but not in case 2. Any particular side effects of etretinate were not observed during treatment. We consider that etretinate therapy is useful in treating epidermal nevus with epidermolytic hyperkeratosis histopathologically.

3.A Case of Paraffinoma.

We experienced a 57-year-old male patient who have had multiple coin sized, erythematous, psinful, indurated ulcerations with purulent discharge on the right calf area since 10 years before. Previously he received injections of paraffin in the lesion site for correcting the thinning of right calf due to complication of pyoderma. Histopathologic examination showed "Swiss cheese" appearance of parsffinoma. Good result was obtained after treatment with surgical excision and split thickness skin graft.

4.A Case of Nevoid Basal Cell Carcinoma Syndrome with Multiple, Symmetrically Distributed Basal Cell Carcinomas.

Nevoid basal cell carcinoma syndrome is a rare inherited disorder characterized by multiple cutaneous basal cell carcinoma, pits of the palms and soles, cysts of the jaws, and ectopic calcification of cranial membranes. The patient was a 66 year old female. She complained of multiple, symmetrically distributed black-colored papules on the both sides of the neck. On physical examination, multiple pits were detected in the palms and soles. The histopathologic findings of the papules showed variable shaped and sized tumor nests composed of basaloma cells. On PNS CT for search of metastasis of the cancer, odontogenic keratocyst was detected at the Lt. alveolar process of maxilla by accident. We diagnosed her nevoid basal cell carcinoma syndrome and the lesions on both sides of the neck were removed completely.

5.Two Cases of Tattoo Reaction.

Tattooing has been performed the for cosmetic purpose by many beautician in the beauty parlors without having been any medicalknowledge. This has resulted in various serious complications. One patient is a 56-year-old female, who had been developed pruritic erythernstous indurated plaques on the tattooing sites about 3 months after getting tattoo. Skin biopsy findings revealed granulomatous inflamrnation and patch test showed positive reaction to potsssiurn dichromate. The other patient is a 55-year-old female, who had been developed erythematous papules on the both lips of tattooing sites about 1 year after getting tattoo. Skin biopsy findings revealed lichenoid resction. Patch test showed positive reactions to reornycin eulfate and nickel sulfate. Patch test of as is revealed positive reactions to red color.

6.A Case of Intracranial Tuberculoma and Optic Disc Tuberculoma Suspected by Miliary Tuberculosis.

Intracranial tuberculoma results from hematogenous spread of pulmonary, intestinal or urogenital tuberculosis. However, it might be caused by pulmonary tuberculosis, mainly. Clinically, symptoms of intracranial tuberculoma are headache and seizure, its symptoms are simillar to intracranial tumor. A 25-year-old-unmarried shopgirl was visited to this hospital because of headache, dizziness and visual disturbance for couple weeks in Sep. 1995. She had been treated with anti-tuberculosis agents of miliary tuberculosis during past nine months period. Brain MRI revealed intracranial tuberculoma and brain edema but not involved optic nerve. Ophthalmic examination revealed severe papilledema and splinter hemorrhage with bitemporal hemianopsis and central scotoma. This finding was strongly suggested of optic disc tuberculoma. Her symptoms became much better following repeated retrobulbar steroid injection with continuous anti-tuberculosis agents. We report a interesting case with intracranial tuberculoma and optic disc tuberculoma associated by miliary tuberculosis during anti-tuberculous treatment.

7.Serum Concentration of CA-125 during the First Trimester of Normal and Abnormal Pregnancies.

A prospective study was initiated to compare maternal serum concentration of CA-125 during the first trimester of normal and abnormal pregnancies. Serum specimens were obtained from 87 women with a normal intrauterine pregnancy and 47 women with abnormal pregnancies which were ended in spontaneo abortion or pathologically confirmed to be missed abortion. In normal pregnancies, the mean serum CA-125 concentrations were increased significantly from amenorhea 6 weeks (139.838.7 IU/ml), and were higher statistically than the values tested in the same weeks of abnormal pregnancies. In abnormal pregnancies serum CA-125 concentations were relatively lower than those of normal pregnancies. But these differences were not statistically significant except the values tested in amenorhea 6weeks. So serum levels of CA-125 may not be proved useful in monitoring of early pregnancies outcome.

8.Study for the Outcome of Mid-Trimester, Emergency Cervical Cerclage in Patients with Painless Dilatation of Cervix.

A retrospective study was conducted to evaluate the clinical outcomes of midtrimester emergency cervical cerclage in patients with painless cervical dilatation with/without prolapsed amniotic sac, including cases of no previous history of cervical incompetence. 10 patients including 1 twin pregnancy were teviewed for study, All had received emergency cervical cerclage with Macdonald operation or Shirodka operation at 18 to 31 weeksgestation. The procedures were carried out without serious complication except for one patient who had received re-operation 3 days after Macdonald operation because of re-protrusion of amniotic sae. The mean procedure to delivery internal was 8.9+/-5.3 (range 1 to 17) weeks. The mean gestational age at delivery was 32.4+/-5.6 (range 22 to 39+6) weeks for the entire group, 36.1+/-2.6 (range 32+4 to 39+6) weeks for the 6 who achived viability, and 27.6+/-5.0 (range 22 to 29+4) weeks for the 3 who died during the neonatal period. 1 died during gestation and was delivered at 33+6 weeksgestation. The mean birthweight was 2181.6+/-971.6 (range 498 to 3500)g for the entire group, and 2712.9+/- 571.5 (range 1860 to 3500)g for the 7 infants who lived until after neonatal period. The total survival rate for 11 babies was 64%. This study demonstrates that midtrimester emergency cervical cerclage for the patients who have dilated cervix with/without prolapsed amniotic sac is valuable method to try in the cases predicted they would lost the baby with conservative treatment only.
