Rickets is a syndrome of diverse etiology, characterized pathophysiologically by a failure of normal mineralization of bone and epiphyseal cartilage and clinically by skeletal deformities. Since the introduction of Vitamin D into the food supply of developed countries, nutritional Vitamin D defiency rickets has become an uncommon disease. In the developing countries Vitamin D deficient rickets occurs not uncommonly, however, and resultant skeletal disorders are still found infrequently. Recently, theories of Vitamin D metabolism have explained the rachitic syndrome, and many of the mystries of the calcium regulatory mechanism have also been uncovered. From January 1971 to May 1975 thirty-two patients with rickets were observed at Yonsei University Severance Hospital. These cases are analysed with a review of the literature. Following observations were made: Of the 32 patients, 14(43.8%) occured between 2 and 3 years of age. 2. The incidence in females was greater than in males (males 28%, females 72%). 3. Bow leg deformity was a major complaint and presenting physical finding. The enlargement of a joint was next. 4. The amelioration of laboratory values and radiological changes were revealed after 2 weeks from the initial treatment. 5. Brace was effective for tibial torsion and was combined with the process of medical treatment.

2.Neuroendocrine Tumor of the Colon.

Neuroendocrine tumor (NET) of the colon and rectum has been reported to have a low incidence and aggressive progression; it is frequently misdiagnosed and its treatment is not well documented. Four NET cases were collected at our hospital during the previous year of a colon cancer survey. Endoscopic mucosal biopsy of the colon was done for each case and all the cases proved to be adenocarcinomas. Curative surgery was conducted after the preoperative diagnostic and staging evaluation was completed. The locations of the primary lesions of the patients were all different; cecum, ascending colon, splenic flexure colon and sigmoid colon. The disease was advanced in all cases and the first postoperative diagnosis was poorly differentiated adenocarcinoma. Immunohistochemical tests followed and all the cases were positive for chromogranin A staining. A retrospective study was then conducted.

3.Finite element analysis on the connection types of abutment and fixture.

PURPOSE: This study was performed to compare the stress distribution pattern of abutment-fixture connection area using 3-dimensional finite element model analysis when 5 different implant systems which have internal connection. MATERIALS AND METHODS: For the analysis, a finite element model of implant was designed to locate at first molar area. Stress distribution was observed when vertical load of 200 N was applied at several points on the occlusal surfaces of the implants, including center, points 1.5 mm, 3.0 mm away from center and oblique load of 200 N was applied 30degrees inclined to the implant axis. The finite element model was analyzed by using of 3G. Author (PlassoTech, California, USA). RESULTS: The DAS tech implant (internal step with no taper) showed more favorable stress distribution than other internally connected implants. AS compare to the situations when the loading was applied within the boundary of implants and an oblique loading was applied, it showed higher equivalent stress and equivalent elastic strain when the loading was applied beyond the boundary of implants. Regardless of loading condition, the abutments showed higher equivalent stress and equivalent elastic strain than the fixtures. CONCLUSION: When the occlusal contact is afforded, the distribution of stress varies depending on the design of connection area and the location of loading. More favorable stress distribution is expected when the contact load was applied within the diameter of fixtures and the DAS tech implant (internal step with no tapering) has more benefits than the other design of internally connected implants.

4.Time Series Analysis between the Concentration of Air Pollutants and the Number of Respiratory Disease Outpatients in Seoul.

OBJECTIVES: This study was conducted to investigate whether air pollution levels in Seoul have short term effects on respiratory disease outpatients. METHODS: We analyzed five air pollutants (CO, NOy, Oy, SOp, PMyp) and daily counts of outpatient for respiratory disease divided by age group ((15yrs, 15-64yrs, 65+yrs) during the period of July 1997-June 1998 using Generalized Additive Model(GAM). RESULTS: After controlling for seasonal trends, day of the week, week of the month, temperature and humidity, CO (RR=1. 041 ~ l. 051, 0-day lag) and NOy (RR=1. 024~1. 050, 3-day lag) were significantly associated with daily counts for respiratory disease in all age groups. 0, and SO, were associated, with a 3-day lag in the age group under 15 years (O RR=1. 047, 95% CI=1. 045- 1. 049 i SOp RR=1. 018, 95% CI=1. 017- l. 020) and with a 0-day lag in the age group 15 to 64 years (0, RR=1. 035, 95% CI=1. 032-1. 037; SO, RR=1. 037, 95% CI=1. 035-1. 039), in the age group over 65 years (O RR=1. 042, 95% CI=1. 033-1. 051; SO RR=1.029, 95% CI=1.022-1.036). PM was weakly associated, with a 1-day lag in the age group under 15 years (RR=1.011, 95% CI=1.010-1.013) and with a 0-day lag in the age group 15 to 64 years (RR=1. 021, 95% CI=1. 019-1. 023), in the age group over 65 years (RR=1. 025, 95% CI=1. 018-1. 032). In the two pollutant model, the magnitudes of the relative risk for CO, NOy and Oq were maintained, but SOy was not associated with daily counts for respiratory disease in the age group under 15 years after controlling for NO,. PMyp was not associated in all age groups after controlling for CO, so the association with PMyp in the single pollutant model might be confounded by CO. In this study, the impact Of PMyp on the respiratory disease was not large as compared with other pollutants. CONCLUSIONS: Air pollution in Seoul may increase the incidence of the respiratory disease, so more positive attention for the control of air pollution should be paid.

5.Clinical trial of oral administration of Bifidobacterium longum in dogs with atopic dermatitis

This study assessed the effects of probiotics in canine atopic dermatitis (AD). We enrolled 11 client-owned dogs with AD and randomly allocated them to two groups. The probiotics group (n = 7) was prescribed with Bifidobacterium longum while the control group (n = 4) received a placebo powder once a day for 12 weeks. In both groups, the degree of skin lesions was evaluated based on the Canine Atopic Dermatitis Extent and Severity Index (CADESI)-4. We also measured the transepidermal water loss (TEWL). We assessed pruritus severity using the pruritus visual analog scale (PVAS). Alteration of consumed drug doses was converted into medication scores. All the evaluation indices were surveyed every 4 weeks. In the probiotics group, there was a significant decrease in the CADESI-4 score at 4, 8, and 12 weeks compared to that of the baseline score (p < 0.05). There was no significant difference in TEWL, PVAS, and medication score at each time point and between groups. Although these results showed that Bifidobacterium longum did not reduce pruritus, TEWL, and the dosage of drugs for canine AD, it was effective in improving skin lesions, therefore, probiotics could be considered in canine AD with severe skin symptoms.
6.Comparison of Antidepressants Tolerability in Cancer Patients Referred for Psychiatric Consultation.

OBJECTIVES: Many patients diagnosed with cancer suffer from various psychiatric symptoms such as depression, anxiety and insomnia as well as cancer itself. Patients with cancer are more vulnerable to possible adverse events of psychotropic medications. Although antidepressants are widely used among cancer patients, there is little information about tolerability of antidepressants. This study was conducted to compare tolerability of antidepressants in cancer patients referred for psychiatric consultation. METHODS: The participants were cancer patients who had been referred to psychiatrist for their psychiatric symptoms. We retrospectively analyzed the data of patients diagnosed with cancer from 9 general hospitals in Korea. The discontinuation rate for a 6 months period after treatment initiation for three antidepressants(Escitalopram, Mirtazapine, Paroxetine) were compared. RESULTS: Antidepressants were prescribed for 96.3% of subjects and Escitalopram 150(47.2%), Mirtazapine 92 (28.9%) and Paroxetine 76(23.9%) were prescribed frequently in order There were no significant differences in discontinuation rates among the three antidepressants during the 6 month period after initiation of pharmacotherapy. But there was a difference in discontinuation rates between inpatients versus outpatients(p<0.0001). CONCLUSIONS: In a naturalistic setting for the antidepressant treatment for cancer patients, it seems that there are no differences in discontinuation rates among these three antidepressants. It is therefore essential that such interactions are carefully considered when treating patients of antidepressants who already have cancer.

7.Effect of conjugated linoleic acid on compound 48/80-induced pruritus in mice.

Pruritus is one of the most important symptoms of allergic inflammatory skin disease. Conjugated linoleic acid (CLA) has been reported to have preventive effects against allergic inflammation. The objective of this study was to determine whether or not oral administration of CLA suppresses pruritus induced by compound 48/80 (composed of N-methyl-p-methoxy phenethylamine with formaldehyde) in mice, and if so, whether or not this effect is associated with serum histamine and prostaglandin (PG) E2 levels. Liquid CLA mixture (36.25% 9c-11t CLA, 36.95% 10t-12c CLA, 1.12% 9c-11c, and 1.94% t9-t11 CLA) was emulsified in 0.5% carboxymethyl cellulose (CMC) sodium salt and orally administered to mice at doses of 200 mg/kg once per day for 3 days. Similarly, disodium chromoglycate (DSCG), an antipruritic substance, was administered orally at the same concentrations as the negative control. Compound 48/80, a pruritus-inducing reagent, was subcutaneously injected 30 minutes after final administration of CLA. Scratching behavior of mice was counted just after compound 48/80 injection. Serum histamine and PGE2 concentrations were evaluated individually. Mice administered with CLA showed reduced frequency of scratching behavior compared to those without CLA. Antipruritic activities in CLA-treated and DSCG-treated groups were 48.5% and 26.8%, respectively. CLA and DSCG also diminished serum concentrations of histamine and PGE2 compared to compound 48/80 alone, respectively. This result suggests that dietary CLA has an antipruritic effect by down-regulating serum histamine and PGE2 levels for relief of compound 48/80-induced scratching behavior in mice, which will be useful in allergic pruritus as a preventive medicine.

8.Safety Evaluation of Human Fibroblasts in Mice: Tumorigenicity, 13-week Toxicity and Distribution Studies.

Human fibroblasts were developed for cellular therapy with the aim of correcting of depressed scars, but the safety of that in vivo is unclear. In this study, we assessed the safety of human fibroblasts by investigating the tumorigenicity, 13-week toxicity and through distribution studies. In the tumorigenicity test, nude mice were divided into three dosage level treatment groups with a negative/positive control group. At 6 months after intradermal transplantation, all of the treatment groups showed no development of a nodule on the injection sites and organs. Toxicity studies were performed using ICR and BALB/c mice for 13 weeks. The mice were divided into three dosage level treatment groups with a control and a syngeneic group. There was no treatment-related effect on clinical signs, mortality, body weight, food/water consumption, hematology, serum biochemistry, urine, necropsy findings and histopathological findings in any groups. These results suggest that the no-observed-effect level (NOEL) of the human fibroblasts was greater than 7.5x10(7) cells/kg for mice. In the distribution study, groups were treated with fibroblasts labeled with a fluorescent dye (CM-DiI) at low and high doses with a control and a syngeneic group. At 24 hours, a large percentage of the labeled fibroblasts were observed at the dermal layer. At 3 months, fluorescence of the labeled fibroblasts continued to be observed. Other tissues were not detected the fluorescence at any time. These studies demonstrate that the safety of human fibroblasts is reasonable with no toxic effect, no tumorigenicity and retention in the dermis. Our studies define preclinical safety testing standards relevant to the development of cellular therapeutics.

9.The Screening of Middle-aged and Geriatric Diseases for 2,021 Residents over 39 Years of Age in Hwasun County, Korea.

OBJECTIVE: Middle-aged and geriatric diseases are becoming prevalent in developing countries and some industrialized nations. The present study was undertaken to determine the prevalence of middle-aged and geriatric diseases in Hwasun County, a rural area of Korea, and to examine the correlation between them and various clinical parameters. METHODS: Population-based cross-sectional study by random cluster sampling of registered residents over 39 years of age was performed. Out of the 3,138 subjects scheduled for the survey, 2,021 residents (male 812, female 1,209) underwent the actual examinations (response rate:64.4%). We examined their weight, height, blood pressure, diet pattern, past and family history, alcohol-drinking and smoking history, and various laboratory parameters. RESULTS: The mean body mass index (BMI) was 22.8+/-3.8. In BMI criteria, the crude prevalence of obesity was found to be 24.6% (grade I obesity; 22.9%, grade II obesity; 1.7%). We found the increasing hypertension, diabetes mellitus, hypercholesterolemia as BMI increased. Overall crude prevalence was 21.4% for hypertension defined as BP (blood pressure) > or =140/90 mmHg or subject on medication, and 16.1% for BP > or =160/100 mmHg or subject on medication. The crude prevalence of diabetes mellitus, hypercholesterolemia, alcoholic liver disease, ischemic heart disease diagnosed by electrocardiography were 6.2%, 6.0%, 4.4%, and 2.3%, respectively. These prevalence were found to be relatively lower than those of reported elsewhere in the world. CONCLUSION: These data indicate that middle-aged and geriatric diseases in the rural area of korea are becoming prevalent, although they are less prevalent than in the urban area or advanced countries. We hope that the present study contributes to understand the prevalence of middle-aged and geriatric diseases in the rural area of Korea, and to prevent and control them adequately.

10.The effect of heat treatment of the TI6AL4V specimen fabricated by selective laser melting method on the crystalline structure and chemical compositions

In this study, the Ti6Al4V specimens were fabricated using selective laser melting (SLM) method (Ti6Al4V-AS group). These specimens were performed to heat treatment at 950℃ and 550℃ using vacuum electric furnace (Ti6Al4V-HT group). The mechanical properties, metallographic shape, crystalline structure, and chemical compositions of the alloy specimens before and after heat treatment were analyzed. The Ti6Al4V-AS group showed ultimate tensile strength (UTS) and low elongation values. Acicular crystal grains were observed in metallographic image and the α'-Ti phase was dominant. After heat treatment (Ti6Al4V-HT group), the UTS was lowered and the elongation was increased. In addition, a lamellar structure in which columnar crystal grains are arranged was observed, α+β-Ti phase was formed, and crystallinity increased. As a result of analyzing the chemical composition, the Ti6Al4V-AS specimen contained a higher carbon content than the Ti6Al4V-HT specimen. This high carbon content could be related to the formation of α'-phase, high strength and low elongation of the Ti6Al4V-AS specimen. Ti6Al4V-HT specimen showed that the overall carbon content was low, but the carbon content was locally high in the α-Ti phase region. Therefore, it is necessary to control effectively the incorporation of carbon gas in the production of Ti6Al4V specimens using SLM.
