1.Superior Oblique Palsy.

I reviewed charts of 26 patients with superior oblique palsy who had been treated surgically. Diagnosis of superior oblique palsy was based on the Bielschowsky head tilt test(BHT) and overacting inferior oblique muscle. Four patients(15%) were diagnosed as having bilateral superior oblique palsy, and 22 patients(85%) were diagnosed as monocular superior oblique palsy. After myectomy of the overacting inferior oblique muscle, 6(27%) out of 22 patients initially diagnosed as monocular superior oblique palsy deveioped findings of superior oblique palsy on the opposite eye; these patients were considered as having masked bilateral superior oblique palsy. Among the 16 patients with unilateral superior oblique palsy, 11 patients(69%) showed negative BHT, 4 patients(25%) showed equivocal BHT and 1 patient(6%) still showed positive BET after surgery. Among the 14 patients who could be followed up for more than 3 months after the operation, 11 patients(79%) showed negative BHT, 2 patients(14.%) showed equivocal BHT, and 1 patient(7%) showed positive BHT. According to these results, the overacting inferior oblique muscle in superior oblique palsy plays a very important role in the elevation of the eye on ipsilateral head tilt, and the mechanism of the BHT classically described should be reconsidered. The primary operation was inferior oblique myectomy and secondary or tertiary operations were inferior oblique myectomy, marginal myotomy or superior rectus recession on the same or the other eye.

3.Lipoxygenase-mediated N-demethylation of pesticides in vitro.

OBJECTIVEIn order to explore the pathway of dealkylation of pesticides other than cytochrome P450 monocoxygenases, lipoxygenase (LOX)-mediated demethylation of aminocarb and some other pesticides were measured.
METHODFormaldehyde generated in the reaction was estimated by Nash reaction to express the rate of demethylation of pesticides mediated by soy lipoxygenase (SLO).
RESULTSN-demethylation of aminocarb mediated by SLO was found to depend on the incubation time, concentration of the enzyme, concentration of aminocarb and hydrogen peroxide. Under optimal conditions, Vmax value of 18 nmol of formaldehyde.min-1.nmol-1 of lipoxygenase was observed. The reaction exhibited Km values of 3.4 mmol/L for aminocarb and 235 mumol/L for hydrogen peroxide. A strong inhibition of the reaction by nordihydroguaiaretic acid, gossypol, and phenidone clearly implicated the lipoxygenase involvement as the protein catalyst. A significant decline in the formaldehyde accumulation in the presence of either reduced glutathione or dithiothreitol suggested generation of a free radical species as an initial oxidation intermediate during the demethylation of aminocarb by SLO. The inhibition of formaldehyde generation by butylated hydroxyanisole(BHT) and butylated hydroxy toluene(BHA) further supported this contention. In addition to aminocarb, seven other pesticides were also found to undergo N-demethylation, albeit at relatively low rates.
CONCLUSIONCertain pesticides may oxidatively undergo dealkylation via the lipoxygenase pathway in animals and plants.

6.Effectiveness of anterior middle superior alveolar injection using a computer-controlled local anesthetic delivery system for maxillary periodontal flap surgery

BACKGROUND: Profound anesthesia with adequate duration is required in periodontal flap surgery, which involves the manipulation of both hard and soft tissues. The anterior middle superior alveolar (AMSA) injection may be an alternative to multiple injections required for this purpose in the maxilla. The present study aimed to assess the effectiveness of AMSA injection using computer-controlled local anesthetic delivery (CCLAD) system to anesthetize buccal hard tissue (BHT), buccal soft tissue (BST), palatal hard tissue (PHT), and palatal soft tissue (PST) around the maxillary teeth. METHODS: Thirty-five patients who were indicated for open flap debridement in a whole maxillary quadrant were given AMSA injection using the CCLAD. The effectiveness of anesthesia was evaluated using subjective and objective parameters around each tooth. Supraperiosteal infiltrations were administered to complete the surgery wherever the AMSA injection was ineffective. RESULTS: The AMSA injection was more effective on the palatal tissues than on the buccal tissues, as 94.14% of PST and 87.89% of PHT sites were anesthetized compared to 49.22% and 43.75% of BHT and BST sites, respectively. There was no significant difference in the frequency of anesthesia around the anterior and posterior teeth. The PHT was significantly more anesthetized (P = 0.003) in males than in females. CONCLUSIONS: The AMSA injection using CCLAD is highly effective on palatal tissues and could be used as a first-line anesthesia for periodontal flap surgery. However, its effect on buccal tissues is less predictable, with supraperiosteal infiltration often required to supplement the AMSA injection.

7.Objective olfactory function test through analysis of human Q-EEG changes.

BACKGROUND AND OBJECTIVES: Increased industrialization and the changing environment have caused complex olfactory problems in the modern population, and the need for an objective evaluation of such problems has thus come to our attention. In this study, we used the human Q-EEG (Bio-Logic, Brain Atlas III) equipment to evaluate olfactory fuction, and tried to lay out a basis for an objective test of olfactory function in humans. MATERIALS AND METHODS: Twenty normosmic adults and ten anosmic patients were studied by the human Q-EEG equipment. EEG changes were analyzed by a nonparametric test (Mann-Whitney test, p<0.05). A mixture of S-D alcohol, benzophenol, BHT (benzyl hydroxy toluene), water, and fragrance were used as an odorant. The odorant was applied to the one third anterior of the inferior turbinate using a small piece of filter paper soaked in the odorant. RESULTS: During odorant stimulation, the Q-EEG analyzer showed that the delta band (0-3.5 Hz) decreased in the whole brain region, while the alpha band (8-11.5 Hz) increased in the bilateral temporal region in the normosmic patients. There was no Q-EEG changes in the anosmic patients. CONCLUSION: It is suggested that human Q-EEG analysis may provide the basis for the development of an objective test of olfactory function in humans.

8.Enhancement of O-dealkylation in Mouse Liver by Dietary Administrations of BHA and BHT: Studies with Isolated Perfused Livers and Hepatic Microsomes.

Effects of feeding 2(3)-tert-butyl 4-hydroxyanisole (BHA) and 3, 5-di-tert-butyl 4-hydroxytoluene (BHT) on the rates of mixed function oxidation and conjugation enzyme reactions have been determined using isolated hepatic microsomal fractions and isolated perfused livers of mice. The treatments with either of the antioxidants have increased the rates of O-demethylation for p-nitroanisole and of O-deethylation for 7-ethoxycoumarin up to 2-fold, both in microsomes and in perfused liver. Analysis of the perfusate showed that the increased amounts of p-nitrophenol and 7-hydroxycoumarin produced by the elevated mixed-function oxidase activities were reflected by the increase in the amounts of glucuronide conjugates and not in the increase for the amounts of the sulfate ester conjugates. Comparison of results also indicated that in the perfused liver, the maximal rate of metabolite conjugation is limited by the maximal rates of the initial mixed function oxidase activities.

9.Inhibition of expression of P-selectin by antioxidant in cholesterol-fed rats.

Butylated hydroxytoluene (BHT) can inhibit experimental atherosclerosis in animals. Although the agent is an antioxidant, the exact mechanism of the reaction in atherosclerosis is still unknown. To investigate the effects of BHT on expression of P-selectin (PADGEM, GMP-140), intercellular adhesion molecule-1 (ICAM-1) and class II MHC (Ia) antigen, we proposed an experiment on rats. Male rats (n=18 per group) were fed either a normal cholesterol control diet, a normal cholesterol diet containing 0.5% BHT (BD), a high cholesterol diet containing 1.5% cholesterol and 0.1% sodium cholate (CD), or the CD diet containing 0.5% BHT (BCD). Rats were sacrificed after 3 days, and after 1, 2, 4, 10, and 17 weeks of dietary treatment. Although there was no gross or light microscopic atherosclerotic lesions, scanning electron microscopy revealed monocytic adhesion to aortic endothelium and mild endothelial injuries in CD and BCD groups. Immunohistochemically, the addition of BHT to a high cholesterol diet inhibited P-selectin expression but not in ICAM-1 and Ia antigen. These findings suggest that in rats, high cholesterol diets induce expression of ICAM-1, P-selectin and Ia antigen. In addition, the antiatherogenic effect of BHT may play a role in the inhibition of P-selectin.

10.Black Hairy Tongue Associated with Erlotinib Treatment in a Patient with Advanced Lung Cancer.

Erlotinib is a tyrosine kinase inhibitor that acts on the epidermal growth factor receptor (EGFR). There have been many reports of the mucocutaneous side effects related to several EGFR inhibitors (EGFRIs). However, no case of black hairy tongue (BHT) associated with EGFRI has been reported. Herein, we report the first case of erlotinib-induced BHT in a 61-year-old man with advanced lung cancer. Considering recent use of EGFRIs worldwide, dermatologists should recognize the possible occurrence of BHT associated with EGFRIs such as erlotinib.
