1.An overview on the establishment of the system of burn prevention and treatment in China by comparing with that of foreign countries.

The number of burn patients in China is still huge because of the demographic and socio-economical reasons. Distinguished achievements have been made in Chinese burn surgery during decades of development, with a generally acknowledged high survival rate of burn victims. However, there are still many problems, including an emphasis on the treatment rather than on prevention, poor pre-hospital care system, unbalanced development of burn centers, shortage of burn rehabilitation and psycho-social support, shortage of financial support, and underdevelopment of social charity for burn patients. Only if the efforts of the burn centers, government, and the whole society join together can the problems be solved and a good system of burn prevention, clinical care, and rehabilitation be established and perfected. The deficiencies in the development of the system in China and the possible solutions are reviewed and discussed in this paper.

2.On the key point in improving the level of burn treatment from the five aspects of PRIDE.

The "PRIDE"-five combined measures, including "protection of cell function and anti-shock treatment to reduce ischemic-hypoxic injury", "rehabilitation from early postburn stage", "inhalation injury treatment", "deep burn wound repair", and "enterogenous infection control", are key measures to improve the levels of burn treatment. The progressive advances in combined measures denoted as "PRIDE" have raised the burn treatment to a higher level and greatly elevated the survival rate. The degree of attention has also been raised from improvement in survival rate only to higher survival rate and better life quality with improvement in rehabilitation. Clinically however, many problems remain to be solved in the treatment of severe burn patients. Further basic and translational studies on "PRIDE" will be helpful to further improve the level of burn treatment.

3.Experiences in rescue and treatment of mass burn casualties in fifty years.

The article reviewed the history and the main experiences of rescue of mass burn casualties and their treatment during the past fifty years in China. Some issues including medical support for mass burn casually and treatment regime in future, such as the prevention of burn calamities, further elevation of the cure rate and lowering in the rate of disability, further development in network of burn care and preliminary scheme of rescue of mass burn casualties and their treatment, accelerating the development and study on the substitutes of allo-skin graft were discussed.

So far there is no clear and accurate definition for burns due to specific causes, though they are encountered sometimes. Burns due to specific causes sometimes are neglected because of their low incidence, and many surgeons, even burn specialists are not familiar with them because of low incidence, thus leading to poor results in the treatment. Burns due to specific causes are often involved in complicated and compound injuries. The treatment is often difficult, leading to delayed wound healing. In this paper, the definition, classification, and clinical characteristics of burns due to specific causes are described. Problems involved in diagnosis and treatment of common ones such as electrical, chemical, and compound burns are discussed. Facing an increase of those burns with specific causes, the author suggests that the research into the prevention and treatment should be highly strengthened.

9.Current situation and reflection on rehabilitation after burn injury in China.

After over 50 years' development, the modern burn care system of China has made remarkable progress, however, we still lack ample attention to functional rehabilitation and quality of life of burn patients recovering from burn injury. The burn rehabilitation in China is still in juvenile stage, but it is really encouraging that more and more burn care facilities are paying attention to it. To further promote the development of burn rehabilitation in China, more emphasis must be stressed on the formation of multidisciplinary team in our burn care system, the concept of early and integral rehabilitation should be promoted, and therapeutic regimens and research work on function and aesthetics related body parts like hand and face should be emphasized. Moreover, we should also carry out rehabilitation on pediatric burn patients and promulgate the idea of burn rehabilitation in the society at large with the purpose of seeking supports for burn rehabilitation from all sectors of the society.

10.Facing wound healing-let our mind soar.

When I started my career as a burn doctor, I looked over all those chapters dealing with wound healing wishing desperately to help me understand the healing process and mechanism of wound. Whereas is that the whole picture of wound healing? Decades have passed; we are still following these protocols. Do we really have nothing better to do with wound healing? When I was asked to write this comment, I was thinking that we should think high and wide in facing wound healing.
