1.Six cases of uremic pruritus treated with emollient and UVB.

Phototherapy with UVB has become a cornerstone in treating uremic pruritus. To investigate the additional benefit of emollient, we performed emollien-phototherapy in six uremic pruritus patients on hemodialysis, one of which was not responding with UVB only. After applying mineral oil over the whole body, UVB phototh.rapy was performed two to three times weekly. Improvement was noted within one to three treatments and after three to ten treatments, pruritus markedly or totally disappeared in al six patients. This result suggests that emollient-phototherapy is as effective as, or in some patients, more effective than phototherapy with UVB only in managing uemic pruritus.

2.A Case of Surgically Corrected-Combined form of Total Anomalous Pulmonary Venous Return.

Total anomalous pulmonary venous return(connection) is a congenital heart disease. Especially, the reports of the patients with double connections of all pulmonary veins were rare. An infant who underwent operation for total anomalous pulmonary venous return had double drainge of all pulmonary veins to both left vertical vein and anomalous descending vein. This second lesion was not diagnosed preoperatively, but in operative field. Operative treatments were performed by ligation of left vertical vein and central pulmonary vein-to-left atrium anastomosis without ligation of the descending anomalous vein. For these less common variants fo total anomalous pulmonary venous return,a more detailed and precise understanding of morphology can facilitate accurate surgical repair. And also, early detection of the disease is an another issue for a good prognosis.

4.Parenting Stress and Efficacy of the Mothers of Children with Internet Addiction and Attention-Deficit Hyperactivity Disorder.

OBJECTIVES: The purpose of this study was to compare levels of parenting stress and efficacy in mothers of children suffering from attention-deficit hyperactivity disorder (ADHD) and internet addiction disorder (IAD), mothers of children with ADHD alone only, and those with normal children. METHODS: Participants were composed of 25 mothers of children with ADHD and IAD, 35 mothers of children with ADHD alone, and 50 mothers of normal children. They completed the Parenting Stress Index (PSI) and Parenting Sense of Competence (PSOC). RESULTS: In comparison with the mothers of normal children, the mothers of children with ADHD and IAD and the mothers of children with ADHD alone showed significantly higher levels of parenting stress and lower parenting efficacy. However, a comparison of mothers of children with ADHD and IAD with those of children with ADHD alone did not show any significant differences in their levels of parenting stress or efficacy. CONCLUSION: Mothers of children with ADHD, regardless of the presence of IAD exhibited a higher level of parenting stress and a lower level of parenting efficacy than mothers of normal children.

5.A Case of Endophthalmitis after Laser in situ Keratomi leusisLASIK.

Laser in situ keratomileusis[LASIK] is a refractive surgical procedure that is effective in treating myopia and astigmatism. The incidence of corneal infection after LASIK is approximately 0.02% according to international experience. We experience the case of a 20-year old man who developed endophthalmitis; corneal edema, epithelial defect, hypopyon was found 2 days after LASIK. He was immediately given intravenous and subconjuntival injection[tobramycin and ceftazidime] and topical fortified eyedrop[tobramycin and ceftazidime]. The microbiologic study revealed Serratia marcescens as the causative organism. Three days after therapy was instituted, the hypopyon resolved. After 4 months of LASIK, visual acuity was 1.0 and maintained recently. We report a case of endophthalmitis after LASIK by Serratia marcescens which detected early and treated by antibiotics. In addition, close postoperative surveillance and education for hygiene was required on early postoperative period.

6.Radiological Findings of Vascular Involvement in Behcet's Disease.

Angiographic findings were analysed in 11 cases of Behcet'sdisease which was diagnosed clinically in Seoul national University Hospital from July 1982 to June 1983. Arterial involvement was noted in 5 cases and venous involvement in 6 cases respectively. Arterial aneurysm and occlusion were arteriographic findings. Arterial aneurysm was noted at large arteries in all 5 cases and multifocal involvement was noted in 4 cases. Characteristic finding of arterial involvement was protruding saccular aneurysm. Venous involvement was thrombotic occlusion of deep veins in iliac, femoral and tibial areas in all 6 cases. Behcet's disease should be differentiated whenever these arterial or venous angiographic findings are revealed in a young adult.

8.Immunohistochemical study on sweat gland tumors.

The histogenesis and differentiation of sweat gland tumors are controversial. Twenty-two cases of sweat gland tumors were stained by immunoperoxidase technique (ABC method) for the presence of S-100 protein, CEA, and two kinds of keratin. Four syringomas, 4 eccrine poromas, 2 eccrine porocarcinomas, 2 eccrine spiradenomas, 1 papillary eccrine adenoma, 3 clear cell hidradenomas, 3 mixed tumors of skin, 2 papillary syringocystadenomas, and 1 cylindroma were included. All samples were formalin-fixed and paraffin-erribedded. Two monoclonal cytokeratin ant.ibodies, MA-902 (specific for cytokeratin No. 8) and MA-903 (specific for cytokeratins No.1,5,10,11) were used. In normal eccrine and apocrine glands, MA-902 stains cells of the intradermal duct and secretory portion. While MA-903 stains cells of the intraepidermal and intradermal duct and myoepithelial cells of eccine and apocrine glands, S-100 protein is found in the secretory cells of the intradermalduct and secretory portion, while CEA stains the secretory and ductal cells of eccrine and apocrine glands. All sweat gland tumors we studied stained by 4 antibodies in variable positive rates, Based on these findings, we discuss the histogenesis of various sweat gland tumors.

10.The Relationship of VO2Max/Min in cardiopulmonary exercise test and fat distribution.

BACKGROUND: Cardiopulmonary exercise test is a useful test for the evaluation of the cardiovascular and respiratory systems. Obese subjects have an increased resting metabolic rate (VO2) compared to non-obese subjects and the increase is more marked during dynamic exercise, which results in the limitation of maximal exercise in obese subjects. In this study, the influence of the obesity and fat distribution on the maximal exercise capacity were evaluated. METHODS: Maximal exercise capacity was represented by maximam maximum oxygen uptake and VO2 max in the cardiopulmonary test. Obesity, total fat content and abdominal obesity(waist to hip ratio, WHR) were measured by bioelectrical impedence method. Total of 42 volunteers(male 22, female 20) were evaluated. RESULTS: 1) Weight to height ratio(mean±SD) was 110%±14.9% in men and 100±11.1% in women. 2) Fat ratio(mean±SD) was 23.3±5.2% in men and 27.55±3.9% in woman. 3) Waist to hip ratio(mean±SD) was 0.85±0.04 in men and 0.8±0.03 in woman. 4) In men, VO2max/min/Kg was negatively correlate with obesity, fat ratio, and abdominal fat distribution. 5) In woman, VO2max/Kg was negatively correlated with obesity and fat ratio, but did not show significant relationship with abdominal fat distribution. CONCLUSION: Obesity was a limiting factor for maximal exercise in both men and women. Abdominal obesity was a limiting factor for maximal exercise in men but its implication to women needs further evaluation.
