1.Correlation Between the Expression of Epidermal Growth Factor Receptor and MR Features in Glioma.

PURPOSE: The aim of this study was to find correlation between the expression of epidermal growth factor receptor (ECFR) and MR findings in the brain glioma. MATERIALS AND METHODS: MR features including edema, margin, necrosis, heterogeneity, hemorrhage and contrast enhancement were retrospectively analyzed with preoperative MR images in 41 patients with proven brain gliomas (8 low grade astrocytomas, 12 anaplastic astrocytomas, 21 glioblastoma multiformes). Immunohistochemical study of EGFR was done and their expressions were graded by both stained distribution and intensity. Correlation analysis between the MR features and EGFR expressions was done. RESULTS: Peritumoral edema was correlated with both distribution (r=0.71, p=0.00) and stain intensity (r=0.69, p=0.00) of EGFR expression. Other MR features showed no statistical correlation with EGFR expression. CONCLUSION: MRI is useful in evaluation of brain glioma, and peritumoral edema is useful findings that suggests EGFR expression as well as malignant histopathologic grade of the tumor.

2.Change of the Effect of TGF-beta1 on Physeal Chondrocytes According to Culture Methods in Vitro.

PURPOSE: We intended to check the growth rates and phenotypic markers of chondrocytes in the dedifferentiated cells cultivated in various conditions in order to establish the ideal culture system for implantation. MATERIALS AND METHODS: Culturing rabbit chondrocytes from proximal tibia, we checked the phenotypes at first, second, and third week. Then we cultured the chondrocytes in different circumstances such as monolayer or three dimensional gel in the presence or abscence of TGF-B1, and checked the growth rates and phenotypic markers. RESULTS: There was no difference in growth rates and mRNA level of type I, type II collagen and aggrecan between the cells cultured in monolayer and three dimensional gel of collagen. However, the responses of the cells to TGF-B1, were quite different between these two groups. In monolayer culture, the expression of type I collagen was depressed by TGF-B1 while the growth rate was markedly increased. Oppositely in three dimensional culture, the mRNA level of type I collagen was markedly increased and the growth rate was completely suppressed by TGF-B1. The expression of type II collagen could be detected only in TGF-B1-treated cells cultured in three dimensional gel for 4 or more days. The mRNA level of aggrecan was also increased by TGF-B1, in the cells cultured in three dimensional gel. CONCLUSIONS: These results suggest that the number of chondrocytes can be efficiently expanded by culturing the cells in monolayer and the phenotypes of chondrocyte can be restored by culturing the cells in three dimensional gel containing TGF-B1. The application of semi-solid gel containing differentiated chondrocytes in physeal implantation should be further evaluated

Arthroscopy became a popular techique for the diagnosis and treatment of wrist disorder. But the specific indications for the wrist arthroscopy are not well defined. So we reviewed our experience of 16 wrist arthroscopies of radiocarpal and midcarpal joints. With this review, arthroscopy was of benifit in the diagnosis and treatment of the lesion of the triangular fibrocartilage complex and also in the synovectomy.

5.Coniparision of Clinical Results Between Keratomileusis In Situ and Laser Assisted In Situ Keratomileusis for High Myopia from -15 Diopter to -23 Diopter.

Photorefractive keratectomy (PRK), Keratomileusis In Situ (KM), Laser Assisted In Situ Keratomileusis (LASIK), and clear lens extraction have been performed for high myopic patients. Among the above surgical procedures KM and LASIK are applied to young patients in general. However, comparison of clinical results of these two procedures was not reported yet in Korea. To evaluate the clinical results of KM (n=16) and LASIK (n=11) for high myopic patients between -15 diopter (D) and -23 diopter (D), spherical equivalent (S.E) and uncorrected visual acuity were studied for more than ten months, retrospectively. The mean S.E. was changed from -17.36 D before operation to -3.70 D ten months after operation in KM, whereas it was from -17.41 D to -0.31 D in LASIK. At ten months after operation, uncorrected visual acuity equal or better than best corrected visual acuity before operation was 43. 8% in KM and 63. 6% in LASIK. This results show that the clinical result of LASIK were superior to that of KM for high myopia.

6.Prevalence of Hypertension Among the Aged in Urban Area.

To estimate the prevalence of hypertension among the aged in urban area, the blood pressure was examined on the subjects of 565 men and 762 women of 65 years old and older among the residents of Nam-Ku and Soosung-Ku in Taegu between January and December, 1986. The mean systolic blood pressure was 136.0±25.01 mmHg in male and 133.0±24.56 mmHg in female. The mean diastolic blood pressure was 83.7±14.41 mmHg in male and 82.4±14.43mmHg in female. There was no significant differences between male and female in both mean systolic and diastolic blood pressure. According to WHO category, the prevalence of pure systolic hypertension (≥169/≤95 mmHg) was 7.8% in male and 6.8% in female. The prevalence of pure diastolic hypertension (≥160/≤95 mmHg) was 6.0% in male and 3.9% in female. The prevalence of both systolic and diastolic hypertension (≥160/≤95 mmHg) was 13.5% in male and 12.9% in female. According to Joint National Committee category, the percentage of normal blood pressure (/<90 mmHg) was 46.7% in male and 50.8% in female. The prevalence of mild hypertension (/90=104 mmHg) was 34.5% in male and 34.6% in female. The prevalence of moderate hypertension (/105=114 mmHg) was 5.1% in male and 2.5% in female. The prevalence of severe hypertension (/≥115 mmHg) was 2.3% in male and 2.5% in female. The prevalence of borderline isolated systolic hypertension (140-159/<90 mmHg) was 9.0% in male and 6.6% in female. The prevalence of isolated systolic hypertension (≥160/<90 mmHg) was 2.3% in male and 2.5% in female.

7.Attitudes of Physician, Nurse and Patient towards Physician's and Nurse's Uniform.

A survey was conducted to study attitudes of physician, nurse and patient towards physician's and nurse's uniform, from March 1 to March 31, 1996. The study population was 130 physicians and 147 nurses engaged in Yeungnam University Medical Center and 211 inpatients of Yeungnam University Medical Center. A questionnaire method was used to collect data. The following are summaries of findings: In the respect of physician's uniform, both physicians and nurses preferred other type of gown to the traditional coat-typed one and especially, nurses preferred more than physicians. Patients showed no difference in the preference of the traditional one and other form of gown as a whole but those who had higher educational level preferred other form of gown(p<0.01). It tended to agree wearing casual wear of physician during the working time of weekend and holiday as a whole. Younger physicians showed significantly higher preference for it(p<0.05). Regarding the nurse's uniform, both physicians and nurses preferred trousers, and 96% of the nurses did. Especially, nurses who were forty years old or more and who served at outpatient department and administrative and aid parts expressed 100 percent partiality to trousers. For the patients, those who had lower educational level preferred skirt and those who had higher educational level preferred trousers. As to the color of nurse's gown, 46.7% of the physicians and nurses liked white color. The physicians preferred white and nurses preferred other color(p<0.01). Of the patients, 79.1% liked white color. Regarding the wearing cap, 95.9% of the nurses replied it didn't have to wear the cap. The nurses who were fifty or more and who served at outpatient department and special parts gave whole answers of not having to wear the cap. On the other hand, 77.7% of the patients answered nurse had to wear the cap. From the above findings, it would be advisable to give a change to the forms and colors of the gowns to match with the trend and sense of the time instead of insisting on the traditional typical ones.

9.Re-repture after primary flexor tendon repair of the hand.

In the treatment of flexor tendon injury of the hand, re-rupture after primary tendon repair is one of complications which occurs occasionally and so impose burden on both the patient and the surgeon. Authors experienced twelve cases of re-rupture after primary flexor tendon repair of 274 patients from Mar. 1989 to Mar.1996. The incidence of re-rupture after primary flexor tendon repair was 4.4% in author's series. One case happened with slip down injury, and in six cases re-rupture occurred during physical therapy with snapping click sound. However in five cases, the patients conldnt recognize any related causes. In majority of cases, re-ruptures were identified between three and five weeks after primary repair by the surgeon and the patient, so it could be suggested that the attention should be paid for the high possiblity of re-rupture during this period. Operative findings were the resorption and friability of repaired end with insecure suture fixation in two cases, rupture of suture material in four cases and loosening of the knot in six cases. From this study, the authors suggest the importance of knot, and recommend to make more than four knots on suture tie with attention to the tie direction, and advise careful physical therapy according to each patients' different situation. In the treatment of re-rupture, end-to-end re-anastomosis was available in seven cases (59%). In five cases (41%), tendon graft was needed. The clinical result of the re-rupture cases was evaluated by the Stickland evaluation method, and it was satisfactory in 67% of the patients who had the complication of re-rupture.

10.Lowe's syndrome in a female child: A case report

Lowe et al. have described a disease characterized by organic aciduria, decreased renal ammonia production, hydrophthalmos, and mental retardation, this disease is now called as Lowe's syndrome or oculo-cerebro-renal syndrome which manifests growth plates, metabolic acidosis, and characteristic eye changes, including bilateral cataract, glaucoma and nystagmus. It is a congenital hereditary affectation. Most of the patients with this syndrome are male sex. We report a new case of Lowe's syndrome in a 5-year old girl.
