1.Correlation Between the Expression of Epidermal Growth Factor Receptor and MR Features in Glioma.

PURPOSE: The aim of this study was to find correlation between the expression of epidermal growth factor receptor (ECFR) and MR findings in the brain glioma. MATERIALS AND METHODS: MR features including edema, margin, necrosis, heterogeneity, hemorrhage and contrast enhancement were retrospectively analyzed with preoperative MR images in 41 patients with proven brain gliomas (8 low grade astrocytomas, 12 anaplastic astrocytomas, 21 glioblastoma multiformes). Immunohistochemical study of EGFR was done and their expressions were graded by both stained distribution and intensity. Correlation analysis between the MR features and EGFR expressions was done. RESULTS: Peritumoral edema was correlated with both distribution (r=0.71, p=0.00) and stain intensity (r=0.69, p=0.00) of EGFR expression. Other MR features showed no statistical correlation with EGFR expression. CONCLUSION: MRI is useful in evaluation of brain glioma, and peritumoral edema is useful findings that suggests EGFR expression as well as malignant histopathologic grade of the tumor.

2.Neurilemmoma of extremities: MR findings.

Six patients with twenty histologically proven neurilemmomas of the extremities were studied using magnetic resonance(MR) imaging. The size, number, signal intensity on spin-echo T1WI(TR 500-650ms/TE 14-25ms)and gradient -echo (TR 200-600ms/TE 14-20ms; flip angle 25-30)image, enhancement pattern, detectability of nerve of origin, nerve-lesion relationship, and presence of a capsule were analyzed. The masses ranged from 1 to 12cm in longitudinal diameter and originated from the median nerve, ulnar nerve, sciatic nerve, radial nerve, and tibial nerve. All the nerve tracts except for those of 5 lesions, which could not be detected due to their small diameter, were visualized as low intensity tubular structures. All visible nerve tracts were situated along the periphery of the lesion and this finding was considered to be specific for neurilemmona. All neurilemmomas were isointense with the surrounding muscle on spin-echo T1WI and hyperintense on gradient-echo image. After a GD-DTPA injection, all masses showed moderate or marked enhancement and more prominent inhomogeneity than that on nonenhanced scan. In 19 out of 20 lesions(95%), a low signal intensity capsule surrounding the masses could be seen. Four of the six patients showed multiple masses, which was unusual as neurilemmoma usually arises as a solitary mass. In conclusion, the MR findings, especially the eccentric location of the mass lesion from the nerve of origin and the presence of a capsule, were useful in making a diagnosis of neurilemmoma of the extremity and that multiple neurilemmomas were not uncommon.

3.The Effect of the Ranitidine (Zantac)(R) in the Treatment of Peptic Ulcer.

The therapeutie efficacy of ranitidine was evaluated itn 48 in- and out- patients with endoscopically diagnosed 18 cases of gaetric ulcer and 30 casea of ducnienal ulcer. In the open study, every pabenta was treated with ranitidine 150 mg b.i.d. for 4 weeks, and waa followed up by gastroscopy after 4 weeks of the treatment. The reeults obtained were summarized as follows; 1) 15 out of 18 cases (83. 3%) of gastric ulcers and 26 out of 30 cases (86. 7%) of duodenal ulcers had been completely healed up in 4 weeks. 2) There was a significant relationship between healing of ulcer and the relief of symptoms (x =6.12, P<0.005).3) There were no significant untoward reactions, efther subjective or objective, to the administration of the drug, except one case of severe epigastic discomfort. In conclusion ranitidine appears to be fairy effective and safe for the treatment of patient with peptic ulcr diseases.

6.Renal Dysplasia: A Clinicopathologic Review of Six Cases.

Renal dysplasia results from aberrant histogenesis in metanephric differentiation. It is characterized morphologically by abnormal organization and a persistence of primitive structures, such as cartilage, undifferentiated mesenchyme, and immature tubules. Six cases of renal dysplasia from five children and one adult are reviewed. Five patients were female and one patient was male. The chief complaint was urinary incontinence in four patients, dysuria in one patient, and the sixth patient suffered from vesicoureteral reflux. No evidence of family history of renal dysplasia in any patient was seen. According to Risdon's classification, three cases were hypoplastic dysplasia, one case was dysplasia in a duplex system, one case was dysplasia in a triplex system, and one case was dysplasia with vesicoureteral reflux. The ipsilateral ectopic ureteral orifice was identified in four patients, two of which drained into a Gartner's duct cyst, and the orifice was suggested in one patient. On histologic examination, all cases showed primitive ducts surrounded by concentrically arranged primitive mesenchyme. Nests of metaplastic cartilage were observed within the stroma in three of the six cases.

7.The Epidemiology of Childhood Enuresis in Seoul and Kyunggi Province.

PURPOSE: To establish the prevalence of childhood enuresis in Korea, an epidemiological study was performed. We also evaluated factors closely related to enuresis and investigated methods of managing enuresis in family. MATERIALS AND METHODS: A self-administered 23 questionnaires was completed voluntarily by the parents of 2,575 children aged 5 to 13 years living in Seoul and Kyunggi Province. RESULTS: From a response rate of 66%, the overall prevalence of enuresis was 8.74%(20.8% in pre-school children, 6.2% in elementary school children). The prevalence of enuresis was higher in boys(60.9%) than in girls(39.1%). The factors closely related to enuresis were deep sleeping, inability to wake-up for voiding, early steeping, family history of enuresis and daytime incontinence. The prevalence of primary and secondary enuresis was 78% and 22%, respectively. When analyzed by a 10-point visual analogue scale to measure parental concern about enuresis of their kids, the mean concern score was 5.37. There was a significant relationship between the frequency of enuretic episode and parental concern score(p < 0.05). Of the enuretic children, 27.6% had consulted with a health worker and the most favored method of treatment was herb medicine. CONCLUSIONS: These results suggest that prevalence rates of childhood enuresis in Seoul and Kyunggi province are similar to those of previous studies of other countries. The sleep pattern, daytime incontinence and family history are closely related to development of enuresis.

9.Erectile Response to Intracavernous Injection of Prostaglandin E1 or Papaverine in Impotence Patients with Diabetes Mellitus.

PURPOSE: To assess the erectile response to intracavernous injection of prostaglandin E1 (PGE 1) or papaverine in the impotence patients with diabetes mellitus (DM), and to determine the factors which would predict good response to either drug. MATERIALS AND METHODS: We injected papaverine (10~60 mg) or PGE1 (10~20 mg) into cavernosal tissues of 305 impotence patients with DM (mean age; 56.1 years). Erectile response was classified into poor, moderate, or good, and was comparatively analyzed according to kinds of vasoactive agents, patients` age, duration of DM and impotence, and treatment modality of DM. RESULTS: The frequency of good response to PGE1 was significantly higher than to papaverine (61.5% vs. 42.8%; p<0.05). However, no association between erectile response and patients` age, duration of impotence and DM, and treatment modality of DM was noted. CONCLUSIONS: Prostaglandin E1 seems to be more effective substance than papaverine for intracavernous injection therapy in impotence patients with DM.

10.Comparison between L and E gene amplification analytical methods for human papillomavirus typing.

OBJECTIVE: L and E6/E7 gene amplification analyses were compared to identify human papillomavirus (HPV) infection and verify the HPV type, with the intent to minimize HPV typing errors. METHODS: L1 gene verified HPV typing was accomplished via polymerase chain reaction (PCR) and membrane assays. Verification of HPV typing via E6/E7 genes was accomplished through nested multiplexed PCR. The results from 104 samples were compared. RESULTS: The rates of accordance and difference were 35% and 65%, respectively. For 29% of the analyses, nested multiplexed PCR was more diversified than the membrane assay. CONCLUSION: HPV can be classified into low-risk HPV and high-risk HPV groups. In parallel amplifications of the L and E genes is more efficient for accurate diagnosis in light of the different symptoms and attendant precautions of the risk groups.
