4.Total Serum IgE Level in Each Age Group of Patients with Atopic Dermatitis.

We evaluated median value and lower confidence limit of total serum IgE level in each age group of 1,493 pure atopic dermatitis patient without respiratory atopic diseases. The results were as follows . 1. The median value of total serum IgE level of each age group in patients with atopic dermatitis distributed from 56.5 IU/@Ll to 660It.J/C. The distributions of the median value of total serum IgE levels of total patients with atopic dermatitis, total male patients with atopic dermatitis, and total female patients with atopic dermatitis were 185 IU/C, 220 IUj'@C, and 165 IUj'C. There was no statistical significance of median value of total serum IgE level between mole patients and female patients. 2. The 95% and 99% lower confidence limit(one-sided) of total serum IgE level of ea.ch age group in patients with atopic dermatitis distributed from 32.6 IU /n4 to 287IU/v4 and from 31 IU/m0 to 202 IU4. The 95% lower confidence limit (one-sided) of total serum IgE level of total patients with atopic dermatitis, tatal male patients with atopic dermatitis, and. total female patients with atopic dermatitis were 170 II.J/m0, 185 IU,m/l, and 133 IUm/l. The 99% lower confidence limit.(one-sided) of total serum IgE level of total patient with atopic dermatitis, total male patient with atopic dermatitis, and total female patients with atopic dermatitis were 165 IU/ml, 180 IU/ml, and 125 IU/ml.

8.Pure Epithelioid Malignant Schwanoma: A case report.

The pure epithelioid malignant schwanoma is a rare form of malignant schwanoma and differs from the ordinary epithelioid malignant schwanoma because of the absence of a spindle cell component. We present a case of purely epithelioid malignant schwanoma arising in the parapharyngeal area from vagus nerve without difinite evidence of von Recklinghausen's disease. The patient was a 28-year-old man with palpable right neck mass and swallowing difficulty during 2 years. The mass was an ovoid encapsulated tumor and measured 5x3x3 cm in dimension. The cut surface showed brownish tan homogeneous nodular appearance with partly myxoid area. Microscopically the tumor showed nodular pattern composed of epitheliod cells which were arranged in both tight clusters and stringy cords. The cells were ovoid in shape and had vesicular nuclei with single prominent eosinophilic nucleolei and sometimes abundant intracytoplasmic mucin. Mitoses were infrequent. Ultrastructural study showed rudimentary cell junction and degenerated cytoplasmic organelles including scattered mitochondriae, short segments of rough endoplasmic reticulum and lipid droplets. There were no definite basement membrane and melanosome. Immunohistochemical study showed the cells being positive for S100 protein, neuron specific enolase and myelin basic protein and negative for cytokeratin.

9.A Case of Purpura fulminans Caused by Streptococcus pneumoniae.

Purpura fulminans is a potentially disabling and life-threatening disorder characterized by acute onset of progressive cutaneous hemorrhage and necrosis on distal extremities, and disseminated intravascular coagulopathy. We experienced a case of purpura fulminans due to Streptococcus pneumoniae. A 42-year-old women presented with skin petechiae, ecchymosis and gangrene on distal extremities with laboratory evidence of DIC. The latex agglutination test with CSF was positive at Streptococcus pneumoniae. To our knowledge, this is the first report of purpura fulminans caused by Streptococcus pneumoniae in Korea.

10.Visualization of the Pulmonary Arteries in the Patients with Pulmonary Atresia or Hypoplasia by Pulmonary Vein Wedge Angiography.

The surgical management of patients whose pulmonary arteries are discontinuous from the ventricular mass depends in part on adequate visualization and quantification of the pulmonary arteries. Pulmonary vein wedge angiography has proven effective technique in demonstrating the true mediastinal(intrapericardial) pulmonary arteries(when present) when standard anterograde injections do not suffice. Pulmonary vein wedge angiography was performed in 11 patients during cardiac catheterization at Serverance Hospital Yonsei University College of Medicine between June, 1984 and December, 1985. In all the patients, venous angiography gave a more complete, clear picture of the pulmonary arteries. We feel that pulmonary venous wedge angiography often is superior to and at least complements standard angiographic methods of opacifying the pulmonary artries in patients with pulmonary atresia or occluded pulmonary arteries. We recommend it as a routine part of the evaluation of these patients.
