1.A case of multiple trichoepithelioma.

Multiple trichoepithelioma is an uncommon skin disease with autosomal dominant transmission. A 21-year-old female ahd asymptomatic, flesh-colored, firm papules and nodules on the face, chest and back. Histopathologic examination showed multiple horn cysts and tumor islands composed of basaloid cells.

2.A case of nodular amyloidosis.

Localized primary cuteneous nodular amyloidosis is the rarest form of the cutaneous amyloidosis, which appears as single or multiple nodules on the extremities, trunk, genitalia or face. A 44-Year-old woman had asymptomatic, translucent brown to pink papules and nodules on the toes for 5 years. Histopathologic exarnination showed deposition to pale, eosinophilic, amorphous material throughout the dermis, Under the electron microscope, the material consisted of straight, nonbranching, nonanastoimosing filaments. We thus made the diagnosis of nodular amyoidosis.

3.Generalized Pseudoxanthoma Elasticum.

We report a patient with a cutis laxa-like, generalized PXE without systemic involvement. A 28-year-old woman had loose, pendulous skin of the neck, axillae, thighs, trunk and body folds which resulted in a prematurely aged appearance. She had no family history of related diseases. Histological examination showed considerable accumulations of swollen and irregularly clumped fibers in the middle and lower dermis and von Kossa s stain revealed calcium deposits along the altered elastic fibers.

5.Consideration of Rescue Breathing methods during Infant Basic Life Support.

BACKGROUND: The current recommendation of rescue breathing techniques in the infant is to blow both the nose and mouth of infants by caregiver's mouth but recent studies suggest some caregivers may have trouble sealing a infant's mouth and nose. The purpose of this study is to estimate the ability of mothers to create a seal to their infants for the provision of rescue breathing according to current recommendation. METHODS: Infants up to 1 year of age(n=98) and their mothers were enrolled from emergency department of Chonbuk National University Hospital. We measured the facial lengths of the infants and the width of mouth in their mothers and compared mouth widths of mothers with their infant's nose-to-mouth lengths. RESULTS: In infants between 0 to 6 months, Mother transverse lengths were significantly larger than mouth, nose and nose-to-mouth lengths of the infants. In infants between 7 to 12 months, Mother transverse lengths were significantly larger than mouth and nose lengths of the infants but there was no statistically significant difference between mother transverse lengths and nose-to-mouth lengths of infants. CONCLUSION: The finding indicate that the present recommendation to cover mouth and nose is partly not possible. We recommend that the mouth-to-nose ventilation of rescue breathing techniques for the infants who have respiratory arrest be taught to mothers.

6.The 1 Case Report of the Transient Osteoporosis of the Hip

Transient osteoporosis of the hip is a self-limited condition of uncertain etiology and pathogenesis that affects young and middle aged adults. Even its clinical features are similar to the features of osteonecrosis the prognosis and clinical end result are quite different. So differential diagnosis between the two is very important. We present a case of transient osteoporosis of the hip in middle aged man, which was completely reeovered after conservative treatment.

7.Comparisons of 12-Hour and 24-Hour Sustained-Release Theophyllines in the Management of Asthma.

BACKGROUND: Sustained-release theophylline, which is generally prescribed as a twice-daily equal-dose regimen, is one of the more common asthma treatments. the development of a sustained-release drug delivery technology that enables improved control of the theophylline blood levels represents a significant advancement in both the efficacy and safety of dosing. METHOD: A crossover study was conducted with 25 adult chronic asthmatic patients requiring daily bronchodilator therapy. The study group included thirteen males and twelve females with ages ranging from 19 to 71 years. The overall approach was to place the patients first on the twice-daily preparation(Etheophyl®) for 28 days at 8 AM and 8 PM, and measure the pulmonary function and theophylline level on the 28th day. the patients were subsequently switched to the once-daily preparation(Uniphyl®) in the same daily dose at 8 PM on the 29th day and the same parameters were measured on the 56th day. RESULTS: the mean serum levels of theophylline were 8.18±1.66µg/ml in the Etheophyl®-treated period and 8.00±1.75µg/ml in the Uniphyl®-treated period. In addition, the FEV1 showed 71.40±7.48 percent in the Etheophyl®-treated and 69.18±9.00 percent in the Uniphyl®-treated period. Thus there were no significant differences between the once-daily and twice-daily preparation. CONCLUSION: The results indicated little clinical differences between the two medication. The two drugs are equally effective in controlling asthma over the four weeks of treatment.

9.The Comparison of Sulcus Angle and Congruence Angle in the Patient with and without the Patella Pain Syndrome

Chondromalacia patella is a symptom complex characterized by aching pain behind the knee cap, retropatella tenderness that is aggravated by the stair climbing. Malalignment is an important factor causing chondromalacia patella. And to recognize it, is important in the diagnosis and treatment. We studied 3 parameters in the chondromalacic knee and nonchondromalacic knee and the following results was obtained. l. In nonchondromalacic knee, the sulcus angle is 141° 7′ and the congruence angle is −5° 1′ 2. In chonduomalacic knee the sulcus sagle is 139° 2′ and the congruence angle is +5° 3′ 3. In the group with abnormal congruence angle, there was much degenerative changes in the patellofemaral joint. So, it can be inferred that chondromalacia is a precursor of the osteoarthritis and the patient with abnormal congruence angle should be treated actively at the early stage if the conservative treatment had failed.
