In this paper, two unusual cases of fetal giant lymphangiomas diagnosed before delivery is reported in 18 and 26-week pregnant woman. They were diagnosed by ultrasound. Although the cause of lymphangioma is not clearly established, they probably arise from a failure of the developing lymphatic tissue to establish normal connection with the draining lymphatics. These anomalies are most often cervical (about 70~80%) but occasionally present in the axilla (about 10%), thorax and abdomen. Lymphangioma arising at posterior nuchal region is called cystic hygroma. Lymphangioma may be divided histologically into three types ; simple, cavernous or cystic. Ultrasound examination is essential method in prenatal diagnosis of fetal lymphangioma. The differential diagnosis of these fetal lymphangioma should include meningomyelocele, benign cystic teratoma, nuchal edema, encephalocele, and subchorial placenta cyst. About 60 to 70% of lymphangioma is accompanied with chromosomal abnormalities, and most common type is Turner's symdrome (40~80%), but occasionally trisomy 21, 18, 13 and 47 XXY. But chromosomal studies of these cases showed normal findings. The 26-week fetus was IUFD at 29 gestational weeks and terminated by hysterotomy. Other 18-week fetus was terminated by vaginal delivery after intrauterine decompression.

To evaluate the relationship between eating behavior and neurotic trait in middle school students, the authors applied eating scale, Middlesex Hospital Puestionaire and Zung's self-reting depression scale to 419 middle school students(male : 187, female : 232) in Taegu city in 1993. There was no significant relationship between weight devided groups. Female middle school students showed significant high scores(p<0.05) in all scales compared to male middle school students. The correlation between eating scale and the other two scales showed significant correlationship(p<0.05) Among psychosocial factors, the items related to dissatisfaction to present and future self image(each p<0.05, p<0.05), dissatisfaction to present environmental status(p<0.05) and dissatisfaction to future attitude showed significant high score in both sex. The item related to dissatisfaction to present interpersonal relationship (p<0.05) showed significant high score in male middle school students. The items related to eat consumption rate of 61-90% (p<0.05), unsatisfied to relationship with family(p<0.05), unsatisfied to childhood family atmosphere(p<0.05), unsatisfied to past self image(p<0.05) and unsatisfied to present relationship between parent(p<0.05) showed significant high score.

The authors studied ED50 of bethanechol on the contractilities of the smooth muscles isolated from various organs of rat under the presence of atropine (a classical competitive blocker of cholinergic muscarinic receptor) or minaprine (a newly developed antidepressant drug) to investigate the peripheral anticholinergic effect of minaprine. The results were as follow: 1) There was no significant difference between ED50 of bethanechol in the control group and that under the presence of minaprine 10⁻⁸M and 10⁻⁷M in the smooth muscles isolated from the duodenum. 2) There was no significant difference between ED50 of bethanechol in the control group and that under the presence of minaprine 10⁻⁸M and 10⁻⁷M in the smooth muscles isolated from the ascending colon. 3) There was significant difference between ED50 of bethanechol in the control group and that under the presence of minaprine 10⁻⁸M and 10⁻⁷M in the smooth muscles isolated from the urinary bladder (P<0.01) 4) There was significant difference between ED50 of the atropine 10⁻⁸ and minaprine (10⁻⁸M) in the smooth muscles isolated from the urinary bladder (P<0.05).

BACKGROUND: The aging process may be induced, at least in part, by reactive oxygen species(ROS). It has been though that the lung could be a good source of ROS because it has a high oxygen tension. In the present study, we invetigated the inducibility of the first and last lines against oxidative stress, superoxide dismutases (Cu/Zn-SOD and Mn-SOD) as a scavenger of O2- and metallothionein(MT) as a scavenger of OH·, respectively, in mouse lungs with age. METHODS: Oxidative stress was induced by paraquat, an intracellular superoxide generator, at 1, 4, 8, and 12 months of age and then SODs and MT mRNAs were determined by RT-PCR method. RESULTS: The steady-state level of Mn-SOD mRNA increased from 1 to 8 months but decreased thereafter. However, Mn-SOD mRNA was not induced by paraquat after 1 month. On the other hand, there was no change in the steady-state level of Cu/Zn/-SOD mRNA, which decrease abruptly at 12 months of age. Additionally, Cu/Zn/-SOD mRNA was not induced by paraquat at any age. There was no change in the steady-state level of MT mRNA with age whereas its inducibility by paraquat was intact at all ages. CONCLUSION: These results indicate that lack of induction of SODs with age may be one of the causative factors in the aging process while induction of MT may play an important role in the defense against oxidative stress. It is therefore implicated that the tissue antioxidant/proxidant balance could be one determinants of meal life span.

To evaluate the diagnostic accuracy of two-dimensional echocardiogrphy(2-D echo) in ventricular septal defect, location and size of the defects, estimated right ventricular systolic pressure and associated cardiac anomaly were compared to the operative findings in 139 children operated for correction of ventricular septal defect at Severance Hospital from Jan. 1983 to June 1987. In addition, postoperative complications and mortality cases were anlysed. The following results were obtained; 1) Perimembranous defects were 82 cases(66.1%), subarterial infundibular defects 33 cases(26.6%), and muscular defect was found in only 1 case(0.8%). The accuracy of 2-D echo in localizing the defects was 84.2%. 2) In 79.2% of the patients, the defects were moderate to large in size, and actual size measured at operation was larger than that obtained by echocardography in general. 3) The estimated right ventricular systolic pressure was correlated(r=0.650) with that measured at the operating field. 4) Combined cardiac anomalies were patent ductus arteriosus(22 cases), atrial septal defect(3 cases), valvular pulmonic stenosis(2 cases) and interventricular septal aneurysm(2 cases). The sensitivity of 2-D echo in detecting these anomalies was 65.5% and the specificity was 96.4%. 5) Among 32 patients who had postoperative complications, in two thirds, there were pulmonary complication including lung atelectasis(16 cases), pleural effusion(5 cases)and pneumonia(5 cases). 6) Operative mortality was 2.9%(4 cases). The causes of death were low cardiac output state due to left ventricular myocardial failure in 3 patients and respiratory failure from asphyxia in one case. In conclusion, with close cooperation with cardiac surgeons, there will be few problems in diagnosing and operating patients with ventricular septal defect on the basis of two-dimensional echocardiographic findings without invasive procedures, such as cardiac catheterization, even with pulmonary hypertension, unless Eisenmenger syndrome is complicated.

Double outlet right ventricle, a complex of congenital cardiac anomalies in which both great arteries arise wholly or in large part from the morphologic right ventricle presents diverse clinical manifestations according to the morphological characteristics. Eighty cases with DORV were diagnosed and operated at Yonsei Cardiovascular Center from 1988 to 1992. The analysis of the morphological characteristics of this anomaly and comparative study of operative methods and mortality according to the morphological classification led to the following results: 1) The location of VSD varied to be subaortic in 40 cases, subpulmonic in 15, doubly committed in 6 and noncommitted in 19 cases. The great arteries were interrelated in D-malposition in 43 cases and L-malposition in 18 and 26 cases among above mentioned 61 cases revealed the side-by-side relationship. Ten of the cases showed normal position and the rest 9, A-malposition. 2) Pulmonary stenosis was found in 60 cases, and when associated with subpulmonic VSD, occurring less frequently. PDA and ASD secundum were associated in successional order and only 3 cases were verified to carry aortic arch anomalies such as coarctation and interruption, all with subpulmonic VSD. 3) Intraventricular tunnel repair was possible in every case associated with subaortic VSD and the postoperative prognosis was excellent. In comparison, most of the cases associated with other types of VSD in which only palliative surgery or various types of intraventricular repair were performed, the general outcome was poor. But total cavopulmonary connection, one of the physiological corrective methods, showing a better postoperative prognosis is being carefully considered for its broad adoptation in future. In conclusion, double outlet right ventricle, a pathophysiological complex of various anomalies, should be thoroughly evaluated for the morphological characteristics to decide the most appropriate types of operation and for consequent improvement of prognosis.

Safety for both mother and infant is an imperative element in anesthesia for cesarean section, and ketamine has been widely used in obstetrical anesthesia especially for analgesis for normal vagianl delivery. Small doses of ketamins(0.2~0.5mg/kg) provides satisfactory analgesia during delivery without significant marternal and fetal complications. We added nitrous oxide to the small dose of ketamine and applied it to the induction of anesthesia for cesarean section and the results were compared with those of thiopental. A small dose of ketamine with nitrous oxide may be superior to an ordinary dose of thiopental, in the effects on the maternal cardiovascular system and the Apgar score of the fetus although the Apgar score only is not sufficient to evaluate neonatal condition.
