Interlocking medullary nails have been widely used in the treatment of long bone fractures especially in tibia and femur. But with increasing use of nails, many preoperative, intra-operative and post-operative complications have been reported. We have experienced 3 cases of very rare complication, fracture of tibia shaft which occurred during removal of interlocking medullary nail after complete healing of original tibial fracture. We report these rare complications with literature review.

But many orthopedic surgeons are not accustomed to issue such medical certificates. So we want to review the way of evaluation and grading of disabilities for such different kinds of medical certificates(for examples: disability grading certificate for use in traffic accident, industrial accident, liability of national compensation, private insurance, the handicapped, military service and others) and compare the differences between them.

6.A Case of Self-inserted Intravesical Newspaper during Masturbation.

Various intravesical foreign bodies have been reported in the literature. We report a rare case of intravesical newspaper that was self-introduced through the urethra as a means of masturbation. A 36 year old male patient was suffered from intermittent dysuria and gross hematuria due to self-inserted newspaper in the bladder. The clinical symptoms, diagnostic maneuvers and treatment of intravesical foreign bodies were described.

7.The general characteristics and states of family of chemical-abusers in an adolescent jail.

BACKGROUND: The drug abuse has become one of social problems especially in the adolescents. It has been well known that the drug abuse is concerned with interaction of biopsychosocial factors and farnily relation factors. So I have studied the general characteristics and rearing attitude and family state of the adolesents who were in a jail because of drug abuse and have intended to give some help for establishing effective strategies for family education and therapy. METHODS: The study was done for adolescents who were in a jail because of violation the illegal drug control law. The group was total 55 men and the age distribution was frorn 15 to 20 years. The Questionnaire was used in Mar 1996 for study and I analyzed 53 cases excluding 2 cases because they were nearly illiterate. The FACES III(Family Adaptability and Cohesion Evaluation Scales), family APGAR score and PBI(Parental Bonding Instrument) were used for evaluating the characteristics of family function and the parental rearing attitude. Statistical management with SAS was executed for data analysis. RESULTS: The average age of the group was 6.9+/-1.4. 48.1% of them lived in the metropolitan area, 40.4% lived in the middle-sized cities and 11.5% lived in the small country towns. The education level of 91.8% was under middle-school graduate. They have use drugs since the age of 14.6+/-1.8, they began to use glue sniffing at the first time in 78.8% and butane gas inhalation in 17.3%. The state of family showed the death of father in 28.9% and the death of mother in 13.5 % and 76.9% of deaths occured before they have become 13 years. Their parents divorced in 25 %. The mean APGAR score was 4.56+/-2.6, which means moderately dysfunctional state of family. In FACES III adaptability score was 20.3+/-5.7 and cohesion score was 26.2+/-6.9. According to the family type by FACES III, there were 5 cases(9.4%) in balanced, 19 cases(35.9%) in midrange, and 29 cases(54.7%) in extreme. In PBI mother overprotection score was 16.24+/-4.59, mother care score was 22.5+6.8, father overprotection score was 16.71+/-5.1 father care was 21.94+/-6.24. CONCLUSIONS: The drug abused adolescents in a jail were in hadicapped family because of deaths and divorce of parents. Their family function score was low and more than half of them had the extreme type of family. Therefore it is wise for us to have a concern for the family problem when understanding and managing drug abused adolescents.

8.BioStore: A Repository System for Registering and Distributing Public Biology Databases.

Although abundant biology data have been accumulated in public biology databases, such as GenBank and PIR, few easy-interface services are provided for users to access or update them. We have developed a system, named BioStore, that is composed of several programs to aid users to not only access public data but also share their own data easily. The service can be used for maintaining a local database as a repository of raw data files of several public databases and distributing the data files to other users. Currently, BioStore manipulates major bio-databases and will expand to include more databases and more useful interfaces.

9.Management of Penile Curvature Combined with Impotence and Complete Urethral Stricture after Pelvic Trauma.

After pelvic trauma, complications of urethral stricture and organic impotence may develop. Certain cases of these are combined with penile curvature, which treatment method of artificial erection alone is not suitable, because severe penile pain may occur during erection, Therefore proper surgical methods should be added for straightening the penile curvature. After pelvic trauma, complete urethral stricture and organic impotence with penile curvature developed in 3 patients who complained severe pain due to curvature during the intracavernosal injection test. To all, treatment of choice for complete urethral sticture was visual internal urethrotomy for prevention of further development of curvature, and for curvature, Nesbit's penile straightening operations were performed, one by one. All patients could well urinate and enjoy their sexual intercourse after intracavernosal injection of PGE1 or papaverine. We recommend visual internal urethrotomy and the Nesbit's operation as effective methods to treat the case of penile curvature combined with complete urethral stricture after pelvic trauma.

10.Comparison of BTA TRAK Assay with Voided Urine Cytology in the Detection of Primary and Recurrent Bladder Cancer.

PURPOSE: We prospectively evaluated the diagnostic efficacy of the BTA TRAK assay according to the stage, grade and hematuria in detecting primary and recurrent bladder cancer, and compared results with voided urine cytology. MATERIALS AND MTHODS: Urinalysis, cytology and BTA TRAK assay were performed simultaneously with the single voided fresh urine samples from 130 subjects. The sensitivity and specificity of the BTA TRAK assay were compared to those of urine cytology and analyzed according to the stage or grade. The subjects were also divided into 4 groups according to the degree of hematuria and the influence of hematuria on the result of the BTA TRAK assay was evaluated. RESULTS: The overall sensitivity and specificity of the BTA TRAK assay for detecting bladder cancer were 82.8% and 65.3%, respectively and those of urine cytology were 44.8% and 100%. The sensitivity of the BTA TRAK assay was significantly higher than that of urine cytology in bladder cancer with lower stage and grade. On univariate and multivariate analysis, gross hematuria and the presence of bladder cancer affected the results of the BTA TRAK assay significantly. In cases following after transurethral resection of bladder tumor (TURB), the sensitivity and specificity of the BTA TRAK assay for detecting recurrent bladder cancer were 100% and 79.5%, respectively. CONCLUSIONS: The BTA TRAK assay was more sensitive but less specific than voided urine cytology. Because gross hematuria affected the result of the BTA TRAK assay independently, it appears reasonable to delay the BTA TRAK assay until gross hematuria subsides in cases with gross hematuria. In cases following after TURB, the BTA TRAK assay appears to be useful for detecting recurrent bladder cancer.
