3.Isolation of perchloric acid soluble, heat stable, ethanol extractable protein from Mycobacterium tuberculosis.

The perchloric acid soluble, heat stable, and ethanol insoluble antigen of M. tuberculosis (TB-PBE) was prepared, and antigenicity of this antigen was studied in vivo and in vitro. TB-PBE showed a single band of 60 kDa by SDS-PAGE. Sera from the patients with active pulmonary tuberculosis did not react with this antigen by ELISA. A delayed hypersensitivity skin reaction was induced with this antigen and was correlated with the reaction with PPD. Skin biopsy was performed in this skin lesion induced by TB-PBE and stained by H-E and immunohistochemical methods. TB-PBE induced an inflammatory lesion similar to a lesion induced by PPD. Blastogenic activity of the peripheral blood mononuclear cells stimulated by TB-PBE increased, and showed a peak reaction at 7 days after stimulation. The blastogenic activity changed in a dose-dependent manner. After stimulation with TB-PBE, mononuclear cells were analyzed by FACS. DR+ T cells and CD4/CD8 ratio increased after stimulation by TB-PBE. These cells secreted IL-2, not IL-4 after stimulation with TB-PBE. In the immunofluorescence test, mouse antiserum against TB-PBE showed a positive reaction with M. tuberculosis and showed cross-reactivity with M. bovive and other atypical mycobacteria, but not with S. aureus. With these results, it is evident that TB-PBE is an antigen which can induce cell mediated immunity in vivo and in vitro.

5.A Case of Bilateral Verrucous Hemangioma Combined with Tuberous Sclerosis.

We report a 37-year-old man who had a cutaneous marker of tuberous sclerosis with angiofibromas, periungal fibromas and who also showed bilateral verrucous hemangiomas on buttock. It is not known whether there is association between verrucous hemangioma and tuberous sclerosis, but there are some evidence suggest a more than chance association. We describe a case of bilateral verrucous hemangiomas combined with tuberous sclerosis.

6.Folk remedies in Korean rheumatoid arthritis patients.

BACKGROUD: A lot of "Folk Remedies" on rheumatoid arthritis treatment prevail in Korea, but they have never been evaluated or investigated systemically. Therefore we studied folk remedies used in rheumatoid arthritis patients. METHODS: We delivered 12 questionnaires to 106 rheumatoid arthritis out -patients of family medicine, internal medicine, and orthopedics, located in Seoul and Kyungki area. These questionnaires addressed to demographic characteristics, duration of rheumatoid arthritis, current treatments, kind of folk remedies experienced, decision to choose folk remedies and its subjective effects, and volition to try new folk remedies. RESULTS: There were up to 27 kinds of folk remedies used by 77 people (72.8% of total). Lower education level was proved to be correlated with folk remedies experience. Most patients began their remedies introduced by relatives or close friends (83.1%) . Many Patients on folk remedies were skeptical about its effect, but they were still willing to try new folk medicine (80.6%). Many rheumatoid arthritis patients experienced folk remedies and were skeptical about its effects, but they were still willing to try new folk medicine. Therefore, education on folk remedies is needed.

7.Treatment of mandibular arteriovenous malformation by embolization : a case report.

Mandibular arteriovenous malformations(AVM) are relatively rare and potentially life threatening lesions. Surgical treatment consists of wide resection of the mandible, which is difficult and potentially hazardous due to significant blood loss during surgery. Therefore, some authors advocate that transvenous embolization may be a safer and more effective method in the treatment of mandibular AVM. We report a treatment case of mandibular AVM in a 9-year-old-girl for episodes of spontaneous bleeding for 6 days using by selective transarterial embolization ,direct puncture embolization and microcoil embolization.

8.Microsporum Canis Infections Transmitted from a Tiger in a Group .

Microsporum canis is one of the zoophiles and pets(especially dogs and cats) play a role as a source of this organism. M. canis is most likely transmitted by contact of these animal hosts with humans. We report on 16 foreigners who are infected with M. canis transmitted from a show tiger. Fourteen of the patients had directly contact with a show tiger on occasion. Patients showed typical tinea corporis lesions on the extremities, and the tiger showed hairless patches on the abdomen. The culture and microscopic findings from patients' lesions revealed M. canis as the causative fungus.

9.Penile Expression of Nitric Oxide Synthase Isoforms in the Type II Diabetic Rat.

PURPOSE: Erectile dysfunction commonly occurs in men with diabetes mellitus. Nitric oxide synthase (NOS) is essential for regulation of penile erection, and the isoforms of NOS have significance in a variety of complications of diabetes. However, little is known about the localization, abundance, and changes in NOS in diabetes. Therefore, we characterized the localization and abundance of NOS isoforms and explored their relationships and how they are altered in diabetes. MATERIALS AND METHODS: The Otsuka Long-Evans Tokushima Fatty (OLETF) rat, which models type II diabetes mellitus, and the non-diabetic control Long-Evans Tokushima Otsuka (LETO) rat were used. We analyzed the distribution of NOS isoforms in the isolated corupus cavernosum of both LETO and OLETF rats by immunohistochemistry (IHC). The mRNA expression of NOS isoforms was also analyzed by reverse transcriptase-polymerase chain reaction (RT-PCR). RESULTS: The concentration of nitrite/nitrate in cavernosal homogenate was higher in diabetic rats than non-diabetic rats, but the difference was not statistically significant. There was a reduction of immunoreactivity for nNOS and eNOS as well as nNOS and eNOS mRNA expression (p<0.05) in diabetic rats, compared to non-diabetic rats. In contrast, the IHC stain showed that iNOS increased in both the endothelium and vascular smooth muscle in the diabetic rats, and the mRNA expression of iNOS gene was also up-regulated (p<0.05). CONCLUSIONS: Change in the activity levels of NOS isoforms within the corpus cavernosum could be a major source of pathogenesis of erectile dysfunction in diabetes mellitus.

10.Right Single Lung Transplantation in Pulmonary Emphysema Patient: A report of case.

Lung transplantation has been successfully employed for variety of obstructive lung disease. Single lung transplantation has become a therapeutic option for end-stage obstructive lung disease. The patient, a 57 year old man with emphysema, suffered from severe dyspnea, which progressively aggravated him for the last three years. A single lung transplantation was performed from a young brain-dead donor on April 7th, 1999 in the department of thoracic surgery, Respiratory Center, Yongdong Severance hospital, yonsei University. The immunosuppressive regimen was based on cyclosporine A and azathioprine from beginning, adding steroid. Single lung transplantation was feasible and beneficial in patients with end-stage emphysema.
