2.Present status of Enterobius vermicularis infestation among children in kindergartens.

BACKGROUND: An Epidemiological study was undertaken to evaluate the prevalence of E. vermicularis infestation among children in kindergartens. METHODS: A total of 247 cellotape anal swab specimens from kindergarten children in Inchon were examined for E. vermicularis eggs. Each specimen was collected in the morning. RESULTS: Out of 247 specimens from two kindergartens in Inchon, E. vermicularis eggs were detected in 168 specimens, the egg detection rate being 68.0%. The egg detection rates in boys and girls were 63.9% out of 133 children and 72.8% out of 114 children, respectively. The egg detection rate in girls was higher than that of boys. The egg detection rates by age group was 65.5%(age 3), S7.4%(age 4), 72.0%(age 5) and 75.4%(age 6), respectively. CONCLUSIONS: Enterobiasis is a cosmopolitan infection. Surveys for children of kindergartens indicate that E.vermicularis is probably the most widely distributed human helminth, its incidence varing from 59.2% to 73.8% in groups studied since pinworm infection is frequently a familial problem that infestation of the household frequently results in reinfection. Careful washing of hands and cleansing of fingernails before meals and after use of the toilet are the most practical measures, all member of family should be treated simultaneously when practicable.

4.The Effect of Serum Obtained before and after Treatment for Endometriosis on in vitro Fertilization Rate of Mouse Oocyte.

OBJECTIVE: To evaluate the effect of serum obtained before and after treatment for endometriosis on in vitro fertilization and development of two cell mouse embryo. Design: Pretreatment and posttreatment comparoson of fertilization of mouse oocyte and embryo development in serum supplement form patients with endometriosis; result were compared using Stuent T-test analysis. METHOD: Infertility Clinic, Department of Obstetrics and Gynecology, Collage of Medicine, Won Kwang university, Korea. Patients was chosed eleven consecutive women with endometriosis. Interventions was all patient underwent laparoscopic or conservative surgery. This was followed by a 6-month course of burserelin acetate 900 microgram/d. Main outcome was measured total number of fertilization and embryo that was fertilization after 24 hours and reached blastocyst stage after 72 hours of incubation were compared before and after treatment. RESULT: Before treatment, 47% of the oocyte were fertilized and 31% of the embryo reached blastocyst stage. After treatment, Significantly more fertilized and Significantly more embryo developed to blastocyst on the stage I and II of endometriosis. CONCLUSION: The fertilization and embryo toxicity of serum samples from patients with endometriosis is lost after treatment.

5.Analysis of Contaminated Ground Water Inducing Methemoglobinemia and Epidemiologic Investigation of Contaminated Ground Water.

PURPOSE: Drining of Nitrate-contaminated water has been the most common cause of acquired methemoglobinemia. We had cyanotic infant diagnosed as methemoglobinemia caused by feeding with powdered formula mixed with nitrate-contaminated ground water. METHODS: We had done epidemiologic investigation to identify the cause of an infant's methemoglobinemia. Analysis of ground water and blood test of involved family members and neighbors were performed RESULTS: Analysis of the 8 household ground water revealed high level of nitrate(29837 ppm), indicating contamination of water as the cause of methemoglobinemia. We had done blood tests to evaluate the methemoglobin level of 65 people in the neighborhood ; among them, there was no infant and we could not find another cyanotic case of methemoglobinemia. They had normal blood level of methemoglobinemic concentration. CONCLUSIONS: If ground water will be used for drinking water, especially for formula water, epidemiologic investigation and analysis of ground waters should be recommended.

7.Reproductive Factors Related to Serum Estrogen, Progesterone, and Sex Hormone Binding Globulin in Postmenopausal Women in Korea.

The etiology of breast cancer is not yet clear. Several epidemiologic studies have supported the concepts that endogenous female sex hormones, i.e., estrogen and progesterone, may play a central role in the development of breast cancer. Female reproductive factors such as menstruation, pregnancy, and breast feeding are well-known risk factors of breast cancer. There have been many suggestions that all these factors are midiated by female sex hormeones. However, only a few studies have evaluated the relationship between the reproductive factors and endogenous female sex hormones. This study was designed to evaluate the relationship between the reproductive factors and endogenous female sex hormones in Korean women. We analyzed the relationship between reproductive factors and female sex hormones in 153 premenopausal women 153 postmenopausal women who participated in a community health promotion program in Haman County, Korea. The questionnaires about reproductive factors were completed by personal interview. Serum level of total estradiol(E2), progesterone(:g), and sex hormone binding globulin(SHBG) were measured by radioimmunoassay. There was no significant correlation between age at menarche and E2 in both premenopausal and postmenopausal groups. In postmenopausal subjects, E2 level increased significantly with the age at menopause increasing(r=0.25, p=0.009), adjusting for the potential confounding effect of both age and body mass index. There was a positive correlation between E2 and the intervals between age at menarche and age at menopause(r=0.25, p=0.009). Moreover, the serum level of SHBG was significantly correlated with total months of breast feeding(r=0.19, p=0.048), as well as with age at first fullterm pregnancy(r=-0.24, p=0.01). However, Pg was correlated with none of reproductive factors in both groups. This study observed that female reproductive factors, e.g., age at menopause, breast feeding, age at first fullterm pregnancy, were correlated with serum female hormones, particularly E2 and SHBG. The results provide an evidence that the relationship between the reproductive factors and breast cancer risk may be mediated, at least in part, through serum female hormones, especially estradiol in Korean women.
