1.The application of percutaneous mechanical thrombectomy with AngioJet system in management of acute lower limb arterial ischemia

Objective To discuss the clinical application of mechanical thrombectomy with AngioJet system for acute lower limb arterial ischemia (ALI).Methods A total of 12 ALI patients,who underwent percutaneous mechanical thrombectomy with AngioJet systemn during the period from January 2015 to November 2016,were enrolled in this study.The clinical data were retrospectively analyzed.The blood flow classification score after thrombolysis in myocardial infarction (TIMI) was used to evaluate the blood perfusion condition,and Cooley standard of efficacy score was used to assess the clinical curative effect.Results The technical success rate of mechanical thrombectomy with AngioJet system was 91.7% (11/12).The average restored perfusion time was (1.5±0.6) hours.The clinical success rate and limb salvage rate were 83.3% (10/ 12) and 91.7% (11/12),respectively.The TIMI flow scores were improved from preoperative grade 0 (n=8) and Ⅰ (n=4) to postoperative grade 0 (n=1),Ⅰ (n=3) and Ⅱ (n=8).In 11 patients (91.7%) the symptoms of lower limb arterial ischemia were strikingly improved after mechanical thrombectomy.In one patient,the postoperative TIMI flow score remained 0 as preoperative state and the symptoms of lower limb arterial ischemia were not improved although catheter directed thrombolysis therapy was employed for 24 hours,and above-knee surgical amputation had to be carried out.Cooley efficacy score showed that complete cure was seen in 4 patients (33.3%),good response in 6 patients (50.0%),general improvement in one patient (8.3%) and pool response in one patient (8.3%).No severe bleeding complications occurred.Conclusion Percutaneous mechanical thrombectomy with AngioJet can rapidly recover the blood perfusion in patients with ALI,thus,further deterioration of the disease can be prevented and the limb salvage rate can be improved.Therefore,this technique has good clinical application value.
2.Research progress of RNA carried by exosomes in malignant bone neoplasm

Malignant bone tumors, one of the most common bone tumors includes osteosarcoma, Ewing's sarcoma, multiple myeloma, and metastatic bone tumors etc. These tumors are often accompanied by distant metastatic lesions at time of diagnosis, leading to low 5-year survival rates. At present, the use of biomarkers for early detection in order to facilitate early treatment are very limited. Therefore, most medical researchers are exploring the roles of exosomes in detecting malignant bone tumors. Exosomes are extracellular microvesicles secreted by different types of cells, which exist in a variety of body fluids. They are new intercellular information carriers that play important physiological roles. Current literature have reported that the RNA contained in exosomes (such as mRNA, miRNAs, lncRNAs and circRNAs) play important roles in the incidence as well as development of malignant bone tumors. However, the previous studies mostly focused on the roles of exosomal RNA in malignant bone tumor diseases, in tumor cell proliferation or apoptosis, transfers, evasion of immune surveillance and chemotherapy drug resistance etc. However, exosomal RNAs may function in the whole process of the disease progression via regulation networks. Our review of existing literature revealed that exosomal RNAs affacted the proliferation, metastasis, immune evasion and drug resistance of five common malignant bone tumors; Osteosarcoma, Ewing sarcoma, Chondrosarcoma, multiple myeloma, and metastatic bone tumors. Therefore, by elucidating on the mechanism of exosomal RNAs in the occurrence and development of malignant bone tumors, this study, could provide new ideas for early diagnosis, concomitant diagnosis, efficacy estimation (chemotherapy, radiotherapy and immunotherapy, etc.) as well as assessing the prognosis of malignant bone tumors.
3.Air pressure cuff-assisted thrombolytic therapy in treating deep venous thrombosis of lower extremity:a comparison study of two kinds of air pressure cuff

Objective To investigate the optimal pressure range produced by two types of air pressure cuff in order to block superficial vein to assist thrombolytic therapy for deep venous thrombosis (DVT) of lower extremity,which can effectively dissolve the thrombus as well as improve the patient's comfort,and to provide useful basis for the implementation of individualized and standard nursing process.Methods A total of 80 patients with lower limb DVT were randomly and equally divided into the modified air pressure cuff group (study group,n=40) and the traditional standard sphygmomanometer cuff group (control group,n=40).Thrombolytic therapy by using continuous intravenous infusion of urokinase via the dorsalis pedis vein with infusion pump was performed in all patients of both groups.The daily dose of urokinase was the same.The pressure value to obstruct lower limb superficial venous blood flow,that was the airbag pressure value when the deep veins of lower limb became visualized on venography,was determined.Leg swelling fade-away rate,mean thrombus clearance rate and patient's comfort degree,etc.were calculated,and the results were compared between the two groups.Results In the study group,the pressure value to block superficial venous blood flow of lower limb was 65-75 mmHg with a mean of (70±5) mmHg;which in the control group was 60-85 mmHg with a mean of (70±10) mmHg.The Leg swelling fade-away rate,the mean thrombus clearance rate determined at different time points,and the patient's comfort degree of the study group were remarkably better than those of the control group (P<0.05).Conclusion Compared to the use of traditional standard sphygmomanometer cuff,the use of modified air pressure cuff to block the lower limb superficial venous blood flow can obtain better thrombolytic effect,meanwhile,the patients can get high comfort degree in the therapeutic process.
4.Recent advance in role of long non-coding RNA mediated competing endogenous RNA regulatory network in ischemic stroke

Ischemic stroke (IS) is a kind of cerebrovascular disease caused by temporary or permanent decrease in local cerebral blood flow. The research of this disease by revealing its pathophysiological process and exploring new therapeutic targets from the cellular and molecular levels has always been the hot spot. In recent years, the competing endogenous RNA (ceRNA) regulatory network mediated by long non-coding RNA (lncRNA) has been proved to play a key role in the pathology and physiology of IS, which can effectively improve the diagnosis, treatment and prognosis of IS. This article aims to describe the mechanism of micro RNA (miRNA) and lncRNA in IS and the role of lncRNA as miRNA's ceRNA in the cell apoptosis, cell viability, angiogenesis, inflammatory response, oxidative stress, and neuroprotection of IS patients, in order to broaden the vision of the occurrence and development of IS and provide new ideas for its diagnosis and treatment.
5.Progress of non-coding RNA in angiogenesis of osteosarcoma

Osteosarcoma needs to continuously induce angiogenesis to satisfy its own nutritional supply in the process of development. Therefore, the inhibition of osteosarcoma cell-induced angiogenesis as a target has become a research hot in recent years. Currently, vascular inhibitors targeting mRNA-encoded protein have been applied in clinic, but the efficacy is poor. Non-coding RNA (ncRNA) is a type of RNA molecules that do not participate in protein coding. ncRNA can regulate the angiogenesis of osteosarcoma by regulating the secretion of vascular factors such as vascular endothelial growth factor, angiogenin-2 and hypoxia-inducible factor 1 or the interaction between ncRNAs. This article reviews the role of ncRNA in the angiogenesis of osteosarcoma to provide references for clinical targeted therapy strategies for osteosarcoma.
6.Advancement on the pathogenesis of hereditary multiple exostoses

Hereditary multiple exostoses (HME) are benign bone tumors characterized by autosomal dominant inheritance, which can cause skeletal malformation in adolescents, seriously affecting the body's aesthetic and motor functions. Currently, there are no guidelines for diagnosing and treating HME, and the main treatment is surgical treatment to remove the tumor and correct the deformity. However, osteochondroma is multiple and difficult to be completely resected. Therefore, more and more scholars are exploring the method of conservative treatment. However, the current understanding of the pathogenesis of HME is limited, and there are no safe and effective drugs in the clinic. Most hypotheses regarding the pathogenesis of HME are based on genetic mutations. Patients with HME may have EXT tumor suppressor gene mutations and function loss caused by secondary mutations such as loss of gene heterozygosity, which ultimately induce abnormal proliferation and differentiation of cartilage in growth plates. Abnormal EXT gene expression causes a decrease in the level of heparan sulphate (HS), leading to abnormalities in multiple molecular pathways that regulate the development and differentiation of growth plate chondrocytes, which together participate in the entire process of HME development and progression. This paper reviews the relevant studies on the pathogenesis of HME in recent years, in order to better understand the pathological process of HME, provide a theoretical basis for the diagnosis and treatment of HME, and also provide ideas for the development of drugs targeting HME.
7.Effectiveness and safety of the home-made umbrella-shaped Octoparms inferior vena cava filter in the prevention of pulmonary embolism

Objective:To evaluate the effectiveness and safety of the home-made umbrella-shaped Octoparms inferior vena cava filter in the prevention of pulmonary embolism.Methods:A multicenter, randomized, positive parallel controlled, non-inferiority clinical trial was conducted in ten hospitals in China from October 2017 to March 2019. A total of 188 subjects were enrolled according to the same inclusion and exclusion criteria in different institutes. The 188 subjects were randomly divided into the trial group or the control group according to 1∶1 by the central randomization system, with 94 cases in each group. Octoparms inferior vena cava filter was used in the trial group, and the Celect inferior vena cava filter in the control group. The primary effective index was clinical success rate,including the clinical success rate of filter placement and filter retrieval. The secondary index included the rate of manual success of the delivery sheath system,incidence of pulmonary embolism(within 6 months), incidence of filter fracture,migration (>20 mm),tilt(>15°) on insertion/retrieval,and the situation of inferior vena cava flow(within 6 months). Safety evaluation included the incidence of filter related complications and device-related adverse events immediately after surgery and during follow-up.Results:The success rate of implantation was 100% in 188 subjects. Filter retrieval was performed in 87 cases (92.55%) in the trial group and 91 cases (96.81%) in the control group. The clinical success rate of the trial group was 97.87%(92/94) and that of the control group 98.94%(93/94). There was no significant difference between the two groups (χ 2=0.77, P=0.380). The success rate of delivery sheath system was 96.81%(91/94) and 98.94%(93/94) in the trail group and the control group,respectively. There was no significant difference between the two groups( P=0.621). There was 1 case (1.22%) of new asymptomatic pulmonary embolism in the trial group after filter placement and 2 cases (2.44%) in the control group. There was no significant difference between the two groups ( P>0.05). No filter fracture or migration (>20 mm) occurred in either group. The tilting of filter (>15°) was found in 1 case (1.06%) in the test group and 1 case (1.06%) in the control group when the filter was placed. The tilting of filter (>15°) was found in 0 case in the test group and 2 cases (2.44%) in the control group when the filter was retrieved. There was no significant difference between the two groups ( P>0.05). Inferior vena cava thrombosis before filter retrieval was found in 5 cases (5.75%) in trial group and 3 cases (3.30%) in control group. There was no significant difference between the two groups ( P=0.489). There were no immediate serious complications during filter placement/removal in either group. No filter obstruction,migration,deformation,penetration and occlusion of inferior vena cava. The incidence of device-related adverse events was low in both group. There was no significant difference between the two groups ( P>0.05). Conclusion:The home-made umbrella-shaped Octoparms inferior vena cava filter is effective and safe in preventing pulmonary embolism, and is not worse than Celect filter.
8.Extracellular vesicles carrying non-coding RNA regulate the activation of osteoclasts

BACKGROUND:It has been demonstrated that osteoclast activation plays an important role in skeletal system-related diseases.The mechanism of regulation of osteoclast activation by extracellular vesicles carrying non-coding RNA has not been fully elucidated. OBJECTIVE:To review and summarize relevant literature in and outside China,and to review the regulation of osteoclast activation by different non-coding RNAs in extracellular vesicles in different diseases,so as to provide a certain direction for subsequent research. METHODS:"Non-coding RNA,miRNA,lncRNA,circRNA,snoRNA,osteoclasts,extracellular vesicles,exosome,microparticle,apoptotic bodies"were used as search terms to search the databases of CNKI,WanFang,and VIP."Extracellular vesicles,exosome,microparticle,apoptotic bodies,non-coding RNA,miRNA,lncRNA,circRNA,snoRNA,osteoclast"were used as search terms to search PubMed.Finally,71 articles were included. RESULTS AND CONCLUSION:(1)The activation of osteoclasts is affected by many factors,among which the specific mechanism of non-coding RNA regulating osteoclast activation is not clear.(2)Extracellular vesicles can be secreted by osteoblasts,bone marrow mesenchymal stem cells,tumor cells and other cells.As a carrier of intercellular communication,extracellular vesicles can carry non-coding RNA to regulate osteoclast activation.(3)In the current studies on the regulation of osteoclast activation by extracellular vesicles carrying non-coding RNA,most of the diseases are osteoporosis,followed by tumor bone metastasis,and most types of non-coding RNA are miRNA.(4)There are relatively few studies on the regulation of extracellular vesicles carrying lncRNA and circRNA and snoRNA on osteoclast activation,and the regulatory mechanism is mainly ceRNA mechanism.(5)In conclusion,an in-depth study of the regulatory mechanism of extracellular vesicles carrying non-coding RNA on osteoclast activation is helpful to find key targets for the treatment of skeletal system-related diseases.
9.Marein alleviates myocardial fibrosis in diabetic mice

Objective To study the effect of marein on myocardial fibrosis in diabetic mice.Methods Ten lep-tin receptor gene defective heterozygous(db/m)mice aged 5-6 weeks were selected as the control group and 30 diabetic mice with leptin receptor gene defective db/db were divided into:db/db group(db/db,n=10),metformin(Met)positive group(280 mg/kg daily,n=10)and marein drug intervention group(50 mg/kg,n=10).After continuous administration for 8 weeks,the cardiac morphological changes were observed by HE staining and Masson staining.The distribution and expression of vimentin were detected by immunohistochemis-try method.The expression of fibronectin,vimentin,and transforming growth factor-β1(TGF-β1)protein in cardiac tissue was detected by Western blot.Results Myocardial fiber hypertrophy was observed in db/db group,and myocardial structural damage was improved in metformin group and marein group.Compared with db/m group,the expression of myocardial collagen fiber in db/db group increased(P<0.01),while the expression of myo-cardial collagen fiber in metformin group and marein group decreased(P<0.01).Compared with the control group,the expression of vimentin in myocardial tissue of db/db group was significantly increased(P<0.01),while the expression of vimentin in metformin group and marein group was significantly decreased(P<0.01).The expression of fibronectin,vimentin and TGF-β1 in db/db group was significantly increased as compared with those in db/m group(P<0.01),while the expression of fibronectin,vimentin and TGF-β1 in metformin group and marein group were significantly decreased(P<0.01).Conclusions Marein improves myocardial fibrosis in diabetic db/db mice.
10.Bringing scientific research education closer to undergraduates through International Genetically Engineered Machine competition.

In recent years, the International Genetically Engineered Machine (iGEM) competition has experienced rapid global development. In 2017 alone, the number of iGEM teams registered around the globe reached an unprecedented 313, with 98 iGEM teams from China having enrolled in the competition and obtained outstanding results. In contrast to the many college students' innovation projects and scientific research training programs in China, iGEM's organization mode is focused on student-centered research learning. Moreover, it achieved a rich educational effect, embodying a new educational idea, which gives it great significance for the extracurricular scientific research training of undergraduates in Chinese universities. In this article, we took Peking University's participation in the iGEM competition as a starting point. The first part introduces the background and general situation of the iGEM competition. The second part reproduces the general procedure of one iGEM season and organization of Peking University's team. The third part compares iGEM's organization mode with those of other undergraduate research training courses and discusses them in detail. The fourth part sums up the experience with iGEM activities as well as explains its effect on developing the research capacity of undergraduate students as well as inspiring them to organize an undergraduate scientific research competition. This article aims to provide a reference for the organization of iGEM activities in domestic universities and for the reform of undergraduate education.
