Objective: To analysis the relationship between glucocorticosteroids (GCS) usage and side effects in the treatment of severe acute respiratory syndrome (SARS). Methods: All clinical records of probable SARS patients in Beijing were collected and input into an Epi6 database, in which 1 291 patients had entire information and met the clinical criteria of SARS. The usage of GCS and GCS associated side effects were analyzed retrospectively. Results: Patients accepted GCS therapy were 83.96% (n= 1 084), whereas 16.04%(n=207) did not take GCS. The average dosage of GCS was 160 mg/d in the first week, and then reduced to 80 mg/d and 40 mg/d in the second and the third weeks, respectively. Initial blood glucose, systolic pressure (SBP), and diastolic pressure (DBP) were no significant difference between GCS group and non-GCS group. The highest blood glucose during the treatment in GCS group was markedly higher than that in non-GCS group [(8.68? 4.80 ) mmol/L vs (6.39?3.71) mmol/L, P05). After GCS administration, SBP and DBP were increased gradually, and reached their peaks in the fourth week [SBP (117.2?14.0) mm Hg and DBP (72.5?9.1) mm Hg vs SBP (120.0?12.5) mm Hg and DBP (74.5?8.7) mm Hg, P

Objective To establish the reference ranges of the spatial angles among cardiac chambers and great vessels in second and third trimester fetuses measured by spatiotemporal image correlation (STIC).Methods Volume images of 352 normal fetuses from 20 to 38 weeks of gestation were recruited in the study.An off-line analysis of acquired volume datasets was carried out with multiplanar mode.Parameters measured included angles between:(1) the 4-chamber view and the left ventricular long axis view; (2) the left ventricular long axis view and main pulmonary artery; and (3) the ductal arch and aortic arch.The relationships between above-mentioned angles and gestational age were assessed by correlation and regression analysis.Results The angle between the 4-chamber view and the left ventricular long axis view (range:55.7° - 35.7°,mean:45.7° ± 5.12°) was uncorrelated with gestational age (r = 0.03,P = 0.51).In contrast,the angle between the left ventricular long axis view and main pulmonary artery,and the angle between the ductal arch and aortic arch were correlated with gestational age (P ＜ 0.001),and the correlation coefficient was - 0.53 and 0.57 respectively.The best-fit exponential curve regression equations of the angle between the left ventricular long axis view and main pulmonary artery was:Y = 154- 4.24X +0.05X2 ,and the angle between the ductal arch and aortic arch was:Y = - 20.8 + 2.65X - 0.37X2.Conclusions The angles among cardiac chambers and great arteries of fetuses from 20 to 38 weeks of gestation can be quantitatively measured by STIC.The reference ranges provide a reliable quantitative standard to estimate the spatial relationships of the cardiac large arteries of fetuses,which may be clinically useful in prenatal screening congenital heart disease.

Objective To observe the effects of different interventions on the body mass index (BMI) and body fat content in overweight and obese adolescents, with an attempt to design reasonable weight relief program.Methods Totally 67 overweight or obese adolescents from a middle school of Beijing were randomly divided into three groups: control group ( n = 16 ), diet intervention group ( n= 22), and combined interventions group ( n =29). The changes of BMI and body fat rate (BFR) were observed. Results BMI significantly decreased after interventions ( diet intervention group: P = 0. 000; combined interventions group: P = 0. 018 ); the change of BMI in diet intervention group was significantly larger than that in control group ( P = 0. 035 ). The trunk and body BFRs also significantly decreased after interventions (diet intervention group: P=0. 000, P = 0. 013; combined interventions group: P = 0. 000, P = 0. 000 ); the changes of trunk and body BFRs were significantly larger in combined interventions group than those in control group ( P = 0.005, P = 0. 003 ). Conclusion Diet intervention and combined interventions are both effective in achieving weight loss in adolescents, and combined interventions have superior effectiveness.

Objective To explore the effects of diet and/or exercise intervention on oxidative stress of overweight or obese adolescents.Methods Thirty-one adolescents with normal body weight(normal control group)and 93 overweight or obese adolescents(average age(13.6 ±0.7)years; body mass index (BMI)22.4 to 34.1 kg/m2)were enrolled in this study.The overweight or obese participants were then randomly assigned to the diet group(group A),exercise group(group B),diet plus exercise group(group C),and non-intervention group(group D).Individualized caloric intake was carried out.One-hour afterschool exercise was performed once per day,4 days per week for 10 weeks.Changes of anthropometry,body composition,and metabolic biomarkers were determined.Results Compared to the normal control group,serum levels of fasting glucose,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and malondialdehyde(M DA)of the in overweight group were significant increased(all P ＜ 0.05),although the level of SOD was significant decreased(P ＜ 0.05).Serum MDA level was positively correlated with bodyweight,BMI,TC,and LDL-C(R values were 0.209,0.228,0.274,and 0.263,respectively ; all P ＜0.05),and serum SOD level was negatively correlated with bodyweight and BMI(R values were-0.334 and-0.362,respectively ; both P ＜ 0.05).Group A,B and C showed a significant decrease in BMI,waist circumference and body fat after the intervention(all P ＜ 0.05).The level of MDA was significantly decreased after the intervention in group A and B(both P ＜ 0.05); the levels of TC and LDL-C were significantly decreased in group B and C(both P ＜ 0.05).Conclusion:Oxidative stress could have existed in overweight adolescents even when their serum glucose and lipid profiles are at a normal level.Ten-week diet and/or exercise intervention may contribute to improved body weight control,lipid metabolism,and oxidative stress response in this population.

Objective:To investigate the effects and the possible mechanisms of Neiguan(PC6)and Gongsun(SP4)on the hypothalamic-pituitary-adrenal(HPA)axis in rats with functional dyspepsia(FD),thus to provide a theoretical basis for the clinical application of the Eight Confluent Points.Methods:Forty specific-pathogen-free Sprague-Dawley rats were divided into a blank group,a model group,an electroacupuncture(EA)group,and a Western medicine group by the random number table method,with 10 rats in each group.Rats in the blank group did not receive modeling or intervention.Rats in the other three groups were subjected to the FD with mood disorder model using the compound etiology modeling method.After the successful modeling,rats in the model group did not receive any interventions,rats in the Western medicine group received deanxit and mosaprid intervention,and those in the EA group received EA intervention on the ipsilateral Neiguan(PC6)and Gongsun(SP4)for 21 d.The sugar-water consumption rate was measured before the experiment and before and after interventions to assess the emotional status.The gastric emptying rate was measured after interventions to assess the gastrointestinal dynamics.The expression levels of hypothalamic corticotropin-releasing hormone(CRH),pituitary adrenocorticotropic hormone(ACTH),and adrenal corticosterone(CORT)were measured by enzyme-linked immunosorbent assay.Results:Compared with the blank group,the sugar-water consumption rate and the gastric emptying rate were decreased(P<0.01),and the hypothalamic CRH,pituitary ACTH,and adrenal CORT expression levels were increased(P<0.01)in the model group.Compared with the model group,the sugar-water consumption rate and the gastric emptying rate were significantly increased(P<0.01),while the expression levels of hypothalamic CRH,pituitary ACTH,and adrenal CORT were significantly decreased(P<0.01)in the EA group and the Western medicine group.The differences between the EA group and the Western medicine group were not statistically significant(P>0.05).Conclusion:The Eight Confluent Points Neiguan(PC6)and Gongsun(SP4)can improve the mood and gastrointestinal dynamics in FD rats,which may be achieved by down-regulating the hypothalamic CRH,pituitary ACTH,and adrenal CORT,as well as by correcting the HPA axis hyperfunction.

Objective To study the relationships between microscopic marginal extensions and tumor types ,and ultrasonic characteristics of malignant liver tumors . Methods Two‐dimensional ultrasonography , contrast‐enhanced ultrasonography and ultrasound‐guided biopsy of hepatic tumors and surrounding hepatic tissues were performed in 78 patients with malignant liver tumor . Pathological microscopic extensions were observed after hematoxylin‐eosin staining of biopsy specimens . Results T he microscopic marginal extension rates of hepatocellular carcinoma ( HCC) ,intrahepatic cholangiocarcinoma ( ICC) and metastatic liver cancer ( M LC) were 62 .5% ( 30/48 ) ,87 .5% ( 7/8 ) ,and 91 .0% ( 20/22 ) ,respectively . For tumors with well‐defined and poorly‐defined two‐dimensional ultrasound boundary ,microscopic marginal extension rates were 50% ( 14/28) ,and 86% ( 43/50) . T umor type and two‐dimensional ultrasound boundary were independent predictors for microscopic extension rate ( P ＜ 0 .05 ) . T he median microscopic extension distances of HCC ,ICC and M LC were 1 .0 ( 0 ,3 .0 ) mm ,4 .0 ( 2 .3 ,4 .0 ) mm ,and 2 .0 ( 1 .8 ,4 .0 ) mm ,respectively . T he distance of microscopic extension increased with tumor size . T umor type and tumor size were independent predictors for the distance of microscopic extension ( P ＜0 .05) . Conclusions ICC ,MLC ,and tumors with larger diameter or poorly‐defined two‐dimensional ultrasound boundary have a larger distance of microscopic extension .

Objective To evaluate the effectiveness of spatio-temporal image correlation with M-mode display ( STIC-M ) in monitoring fetal left ventricular systolic function in fetuses with congenital heart disease(CHD) . Methods Five hundred and thirty-six normal fetuses and 34 fetuses with CHD( 29 without hydrops and 5 with hydrops) were involved in the study . Left ventricular fractional shortening ( LVFS) was measured using STIC-M . The data of normal fetuses was used to construct reference ranges of LVFS for assessment of fetuses with CHD . Results The LVFS of the normal fetuses [ range :26 .8％ - 42 .9％ , mean :( 34 .9 ± 4 .1) ％ ] was negatively correlated with gestational age ( r = - 0 .16 , P < 0 .001) . Compared with the normal controls ,LVFS was significantly decreased in CHD fetuses with hydrops ( P < 0 .001) . However ,there was no significant difference in LVFS between normal controls and CHD fetuses without hydrops ( P > 0 .05) . Conclusions STIC-M is a new method that can be used to measure LVFS to evaluate fetal ventricular systolic function . The fetal ventricular contractile function of CHD fetuses without hydrops may not be damaged or is in compensation stage . The fetuses with cardiac hydrops generally become lower .

Objective: To investigate the clinical value of laparoscopic peritoneal dialysis catheter implantation in peritoneal chemotherapy for gastric cancer with peritoneal metastasis. Methods: From January 2019 to June 2019, the clinical data of 6 patients diagnosed as gastric cancer with peritoneal metastasis were retrospectively analyzed in the Gastrointestinal Surgery Department of Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine. Five were male and 1 was female. The median age was 69.5 (28-77) years. The median body mass index (BMI) was 22.8 (19.6-23.5). All procedures were performed under general anesthesia with endotracheal intubation. The patient′s body position and facility layout in the operating room were consistent with those of laparoscopic gastrectomy. The operator′s position: the main surgeon was located on the right side of the patient, the first assistant stood on the left side of the patient, and the scopist stood between the patient′s legs. Surgical procedure: (1) trocar location: three abdominal trocars was adopted, with one 12 mm umbilical port for the 30° laparoscope (point A). Location of the other two trocars was dependent on the procedure of exploration or biopsy as well as the two polyester cuff position of the peritoneal dialysis catheter: Usually one 5 mm port in the anterior midline 5 cm inferior to the umbilicus point was selected as point B to ensure that the distal end of the catheter could reach the Douglas pouch. The other 5 mm port was located in the right lower quadrant lateral to the umbilicus to establish the subcutaneous tunnel tract, and the proximal cuff was situated 2 cm away from the desired exit site (point C).(2) exploration of the abdominal cavity: a 30° laparoscope was inserted from 12 mm trocar below the umbilicus to explore the entire peritoneal cavity. The uterus and adnexa should be explored additionally for women. Once peritoneal metastasis was investigated and identified, primary laparoscopic peritoneal dialysis catheter implantation was performed so as to facilitate subsequent peritoneal chemotherapy. Ascites were collected for cytology in patients with ascites. (3) peritoneal dialysis catheter placement: the peritoneal dialysis catheter was introduced into the abdominal cavity from point A. Under the direct vision of laparoscopy, 2-0 absorbable ligature was reserved at the expected fixation point of the proximal cuff (point B) for the final knot closure. Non-traumatic graspers were used to pull the distal cuff of peritoneal dialysis catheter out of the abdominal cavity through point B. The 5-mm trocar was removed simultaneously, and the distal cuff was fixed between bilateral rectus sheaths at the anterior midline port site preperitoneally. To prevent subsequent ascites and chemotherapy fluid extravasation, the reserved crocheted wire was knotted. From point C the subcutaneous tunnel tract was created before the peritoneal steath towards the port site lateral to the umbilicus. Satisfactory catheter irrigation and outflow were then confirmed. Chemotherapy regimen after peritoneal dialysis catheterization: all patients began intraperitoneal chemotherapy on the second day after surgery. On the 1st and 8th day of each 3-weeks cycle, paclitaxel (20 mg/m²) was administered through peritoneal dialysis catheter, and paclitaxel (50 mg/m²) was injected intravenously. Meanwhile, S-1 was orally administered twice daily at a dose of 80 mg·m^-2·d^-1 for 14 consecutive days followed by 7-days rest. To observe the patients′ intraoperative and postoperative conditions. Results: All the procedures were performed successfully without intraoperative complications or conversion to laparotomy. No 30 day postoperative complications were observed. The median operative time was 33.5 (23-38) min. The median time to first flatus was 1(1-2) days, and the median postoperative hospital stay was 3 (3-4) days, without short-term complications within 30 days postoperatively. The last follow-up was up to July 10, 2019, and the patients were followed for 4(1-6) months. No ascites extravasation was observed and no death occurred in the 6 patients. There was no catheter obstruction or peritoneal fluid extravasation during and after chemotherapy. Conclusion Laparoscopic peritoneal dialysis catheter implantation was safe and feasible for patients with peritoneal metastasis of gastric cancer. The abdominal exploration, tumor staging and the abdominal chemotherapy device implantation can be completed simultaneously, which could simplify the surgical approach, improve the quality of life for patients and further propose a new direction for the development of abdominal chemotherapy.

BACKGROUND:Oleic acid can regulate inflammation and immune responses,and has the potential to repair skin wounds.Oleic acid has a short retention time at the lesion.It is prone to self oxidation and deterioration in the air,and suitable drug carriers are needed to fully exert the therapeutic effect of oleic acid. OBJECTIVE:To investigate the efficacy of oleic acid-liposome gel in the treatment of chronic burn wounds. METHODS:Oleic acid liposome solution was prepared by thin film dispersion method,and then dissolved in Poloxamer gel matrix to prepare oleic acid-liposome gel.(1)In vitro experiment:Oleic acid-liposome gel solution was prepared by adding different volumes of oleic acid-liposome gel into cell medium(volume ratio:1:3,1:9,1:27,respectively).Alma-blue reagent was used to detect the effects of different concentrations of oleic acid-liposome gel on the proliferation of human keratinocytes and human fibroblasts.Crystal violet staining was used to observe cell morphology.(2)In vivo experiment:The animal model of chronic burn wounds was established by using full-thickness burn of SD rat back skin combined with local subcutaneous injection of epirubicin.The 30 successfully modeled rats were randomly divided into five groups with six rats in each group.The wounds of oleic acid liposome gel group,oleic acid group,liposome gel group,positive control group and negative control group were applied with gauze of oleic acid liposome gel,oleic acid,liposome gel,recombinant human epidermal growth factor gel and normal saline.The dressing was changed once every other day.A total of 16 doses were administered.The wound healing was observed. RESULTS AND CONCLUSION:(1)In vitro experiments:Alma-blue reagent detection and crystal violet staining showed that oleic acid liposome gel solution with volume ratio of 1:9 could promote the proliferation of human keratinocytes and human fibroblasts.(2)In vivo experiment:The wound healing time of the oleic acid liposome gel group was shorter than that of the other four groups(P＜0.01),and the wound healing rate at 4,8,12,16,and 20 days was higher than that of the other four groups(P＜0.01).After administration,hematoxylin-eosin staining showed epithelialization and healing of wounds in all five groups,and the epidermal thickness of oleic acid liposome gel group was the closest to normal skin and better than the other four groups.Immunohistochemical staining showed that the expressions of cytokeratin 10,tumor protein 63,α-smooth muscle actin,collagen I,tumor necrosis factor α,interleukin 6,malonaldehyde,and superoxide dismutase in oleic acid liposome gel group were closest to those in normal skin,and superior to those in other four groups.On days 12 and 32 of administration,the expressions of tumor necrosis factor α,interleukin 6,malondialdehyde,and superoxide dismutase in wound homogenate supernatant in oleic acid liposome gel group were closest to those in normal skin,and superior to those in other four groups.(3)The results showed that oleic acid liposome gel could promote the proliferation of keratinocytes and fibroblasts,reduce inflammation and oxidative stress injury,and promote the healing of chronic burn wounds.
