2.Correlation of vertebral artery hypoplasia with posterior circulation cerebral infarction

Objective To explore the relationship of vertebral artery hypoplasia (VAH) with acute posterior circulation cerebral infarction (APCCI).Methods One hundred and eighty-nine consecutive patients diagnosed as having acute cerebral infarction admitted to our hospital from March 2015 to March 2016 were enrolled in our study;retrospective analysis was performed on their clinical and radiographic data to compare the general clinical characteristics of patients with posterior circulation infarction (PCI) and anterior circulation infarction (ACI).VAH was conformed by cranial magnetic resonance angiography (MRA),vertebral artery ultrasound or cranial MRA+vertebral artery Ultrasound.The infarction lesions in VAH patients with APCCI were typed;the correlation between vertebral artery diameter's differences and National Institute of Health stroke scale (NIHSS) scores in VAH patients with APCCI was illuminated.Results In 189 consecutive patients with acute cerebral infarction,32 (16.93%) were with VAH;after exclusion of 8 with infarction in both anterior and posterior circulation,90 (49.72%) were in PCI group while 91 (50.28%) in the ACI group.Univariate analysis showed that statistical differences were noted between the two groups in triglyceride (TG) and VAH levels (P<0.05).In 23 patients with APCCI with VAH,brain stem infarction consisted of 9 patients (5 VAHs were ipsilateral to the infarction,3 VAHs contralateral and one bi-VAH),cerebellum infarction consisted of one patient (ipsilateral side),thalamus infarction 10 patients (4 VAHs were ipsilateral to the infarction,and 6 VAHs contralateral),occipital lobe infarction 2 patients (one ipsilateral and one contralateral) and cerebellum with thalamus infarction one patient (the side of VAH was ipsilateral to the cerebellum and contralateral to the thalamus).After excluding one patient with bi-VAH,in 22 patients with APCCI combined with VAH,significantly positive correlation between vertebral artery diameter's differences and NIHSS scores was noted (r=0.560,P=0.000).Conclusions It might be the best assessment standard of VAH measured by combination of transcranial MRA and vertebral ultrasound.TG and VAH may be risk factors of APCCI,and VAH could be one independent risk factor of APCCI.The side of VAH is usually ipsilateral to the brain stem and cerebellum infarction,while contralateral to the thalamus infarction in APCCI patients with VAH.Also,vertebral artery diameter's differences are positively correlated with NIHSS scores.
3.Influencing factors and prevention and treatment progress of vascular restenosis after interventional treatment of lower extremity arteriosclerosis obliterans

Lower extremity arteriosclerosis obliterans is a clinical manifestation of atherosclerosis in the lower extremities.At present, the main treatment methods include stent implantation, balloon angioplasty.However the incidence of restenosis after interventional surgery is high, which seriously affects the effect of surgical treatment and the prognosis of patients.This article reviews the mechanism, influencing factors and the latest progress in the prevention and treatment of vascular restenosis after arteriosclerosis obliterans of the lower extremity orteriosclerosis obliterans intervention, which is of important clinical significance to the early prevention and treatment of instent restenosis.
4.Effectiveness of artificial intelligence-endoscopic ultrasound biliary and pancreatic recognition system: a crossover study

Objective:To explore the effectiveness of the artificial intelligence-endoscopic ultrasound (AI-EUS) biliary and pancreatic recognition system in assisting the recognition of EUS images.Methods:Subjects who received EUS due to suspicious biliary and pancreatic diseases from December 2019 to August 2020 were prospectively collected from the database of Department of Gastroenterology, Renmin Hospital of Wuhan University. Pancreatic EUS images of 28 subjects were included for recognition of pancreas standard station. EUS images of bile duct of 29 subjects were included for recognition of bile duct standard station. Eight new endoscopists from the Gastroenterology Department of Renmin Hospital of Wuhan University read the 57 EUS videos with and without the assistance of AI-EUS biliary and pancreatic recognition system. Accuracy of endoscopists' identification of biliary and pancreatic standard sites with and without the assistance of AI-EUS was compared.Results:The accuracy of pancreas standard station identification of the new endoscopists increased from 67.2% (903/1 344) to 78.4% (1 054/1 344) with the assistance of AI-EUS. The accuracy of bile duct standard station identification increased from 56.4% (523/928) to 73.8% (685/928).Conclusion:AI-EUS biliary and pancreatic recognition system can improve the accuracy of EUS images recognition of biliary and pancreatic system, which can assist diagnosis in clinical work.
5. Study on the inclusion technic of hydroxypropyl-β-cyclodextrin in tea tree oil and evaluation of its included compounds

Objective:
This study takes hydroxypropyl-β-cyclodextrin (HP-β-CD) as the inclusion materials to optimize the preparation technic of tea tree oil (TTO) and evaluate its pharmaceutical performance.
Methods:
Take the production rate of HP-β-CD tea tree oil inclusion and entrapment rate as the evaluation index, taking the orthogonal test method to optimize the production technic of tea tree oil (HP-β-CD inclusion and using infrared (IR), differential thermal scanning (DSC) method to characterize the inclusion compound to analyze the stability of TTO-HP-β-CD.
Results:
The best technic to produce HP-β-CD tea tree oil is as follow: the ratio of TTO and HP-β-CD should be equal to 1/10, at 40 ℃, within 1 h. The average drug loading shoud be 9.25% ± 3.25%. The IR, DSC characterization results showed that the characteristic peak of tea tree oil disappeared after the microspheres, which indicated the HP-β-CD encapsulated the tea tree oil with good compatibility. In 80 ℃ water bath, the TTO-HP-β-CD was stable with the retention rate 40% after 8 h, the retention rate was 4.32 times than that of the unwrapped tea tree oil.
Conclusions
The HP-β-CD tea tree oil obviously has higher rate of inclusion and stability. Therefore, it’s worth to promoting and being used in the pharmacy preparations and cosmetics field.
