目的观察壮腰活血汤治疗腰椎间盘突出症的疗效。方法将198例腰椎间盘突出症患者随机分为治疗组和对照组,治疗组内服壮腰活血汤配合腰椎牵引、超短波等治疗;对照组采取与治疗组相同的物理治疗方法。治疗2个疗程和疗程结束2个月后进行疗效评定。结果治疗组的有效率明显高于对照组(P<0.05—0.01)。结论壮腰活血汤治疗腰椎间盘突出症疗效确切,并且远期疗效更优。

Objective To summary the experience in surgical treatment of differentiated thyroid carcinoma (DTC).Methods The clinical data and follow-up results of 110 cases of DTC were retrospectively analyzed.Results 110 patients received operation and TSH suppress therapy.Histologic examination after operation revealed that papillary thyroid carcinoma(PTC) 88 cases(80%) ,follicular thyroid carcinoma 22 cases(20%) ,cervical lymph node metastasis were found in 22 cases(20%).The operation procedure consists of excision of the suffered lobe,isthmus and greater partial of the opposite lobe 59 cases(along with functional cervical dissection 12 cases) ;excision cf the sufferedlobe,isthmus 29 cases(along with functional cervical dissection 7 cases) ;excision of the suffered lobe,isthmus greater partial of the opposite lobe 18 cases(along with functional cervical dissection 5 cases) ;total thyroidectomy and bilateral cervical dissection 2 cases;total thyroidectomy and bilateral cervical dissection with postoperative 131I therapy,1 cases with metastasis to lung.98 cases 10-year survival rate was 91.8%.Conclusion Differentiated thyroid carcinoma has a good prognosis,a proper surgical procedure is the key point to get a good outcome.

Objective To analyze the therapeutic effects of different treatment modalities in patients with tongue squamous cell carcinoma.Methods A retrospective analysis was performed on the complete clinical and follow-up data of 132 patients with pathologically confirmed tongue squamous cell carcinoma,who were initially treated at the First Affiliated Hospital of Xinjiang Medical University from 2003 to 2011.The Kaplan-Meier method was used to calculate the overall survival (OS) rates for patients who received surgery alone (S),radiotherapy alone (R),surgery plus radiotherapy (S + R),chemotherapy plus surgery (C + S),chemotherapy plus radiotherapy (C + R),and surgery,radiotherapy,and chemotherapy (S + R + C).The OS was compared between these groups by log-rank test.Multivariate analysis was performed using the Cox proportional hazard model to establish independent treatment modalities as prognostic factors.Results The follow-up rate was 100％.The 3-year sample size was 94.The 3-year OS rate for all patients was 72.7％.The univariate analysis showed that among 70 stage Ⅰ and Ⅱ patients,the S,R,S + R,C + S,and S + R + C groups had 3-year OS rates of 86％,67％,97％,100％,and 82％,respectively (P =0.018) ;among 62 stage Ⅲ and Ⅳ patients,the S,R,S + R,C + S,C + R,and S + R + C groups had 3-year OS rates of 38％,14％,92％,40％,14％,and 67％,respectively (P =0.000).The multivariate analysis showed that S + R and S + R + C were independent prognostic factors (P =0.000 and 0.005).onclusions Surgery alone or combination therapy including surgery has a good therapeutic effect for stage Ⅰ-Ⅱ tongue squamous cell carcinoma,while S + R and S + R + C are better treatment modalities for stage Ⅲ-Ⅳ disease;however,advanced patients have a poor prognosis after being treated with R and C + R modalities.

BACKGROUNDTo investigate the characteristics and patterns of lymph node metastasis in lung cancer, and to provide evidence for determining range of lymph node dissection.

METHODSOne hundred and five patients with lung cancer received complete resection combined with systematic lymph node dissection according to the mapping system developed by Naruke.

RESULTSA total of 801 lymph nodes were dissected from 105 lung cancer patients. The positive ratios of N1 and N2 were 15.9% and 14.9%, respectively. Twelve patients were found with skipping N2. These skipping N2 were located in 2, 4, 5, 6, 7 groups respectively. There was no significant relationship between size of primary tumor and lymph node metastasis. Small cell lung cancer showed the highest risk of lymph node metastasis. The metastatic rate of lymph node in adenocarcinoma was markedly higher than that in squamous cell carcinoma. Skipping mediastinal lymphatic metastasis was found more frequently in lower lobar tumors than that in upper lobar ones.

CONCLUSIONSLymph node metastasis of lung cancer may occur in multiple groups and multiple regions, even in a skipping pattern. Systematic lymph node dissection should be routinely performed in pulmonary resection for lung cancer.

OBJECTIVE:To provide idea and reference for TCM therapy for hemoptysis. METHODS:The prescriptions for hemoptysis included in TCM Prescription Dictionary were screened,summarized and logged in TCM inheritance support platform software(V2.5). The medication frequency was analyzed statistically. Apriori algorithm was used to analyze combination of medicinal materials(the number of support degree was 12)and its association rules(confidence degree>0.7)statistically. Correlation coefficient of 2 different medicinal materials(related coefficient of 8,penalty coefficient of 2)and core combination of 3 and 4 ingredients were analyzed statistically by complex system entropy clustering method. Core combination and new prescription were extracted by unsupervised entropy hierarchical clustering method. RESULTS:Totally 105 prescriptions for hemoptysis were screened and included 198 ingredients,among which 39 ingredients were used more than 6 times;there were 30 common couplet medicines(including 15 ingredients)and 11 correlation regularities;48 core combination of 3 ingredients and 5 core combination of 4 ingredients were obtained. Each 6 core combination 1 and 2 were extracted for new prescription clustering to obtain 6 new prescriptions. CONCLUSIONS:The efficacy of each ingredient of the prescriptions for hemoptysis mainly include clearing heat and cooling blood,nourishing Qi and blood,eliminating phlegm and relieving cough. The efficacy of couplet medicines for hemoptysis mainly include clearing heat and cooling blood combined with nourishing Yin and blood,tonifying Qi and Yin. The composition of new prescription shows diverse characteristics,and pay attention to cooling blood to stop bleeding, nourishing Qi and nourishing Yin,or clearing heat and eliminating phlegm.

AIM:To investigate the expression of CUE domain-containing 2 (CUEDC2) in hepatocellular car-cinoma ( HCC) and to analyze its clinical prognostic significance .METHODS:Total 186 formalin-fixed paraffin-embed-ded tissues obtained from surgical HCC with detailed clinicopathological and follow -up data were used .The expression of CUEDC2 was detected by immunohistochemistry .The relationships between the expression of CUEDC 2 and clinicopatholog-ical characteristics and prognosis were analyzed .RESULTS: The positive rate of CUEDC 2 in HCC was 85.5% ( 159/186), among which, the low expression was 52.2%(97/186) and the high expression was 47.8%(89/186).CUEDC2 expression was correlated with serum alpha-fetal protein (AFP) level, tumor size, tumor number, tumor differentiation and TNM stage (P<0.05).Kaplan-Meier survival curves showed that the patients with high expression of CUEDC 2 were asso-ciated with significantly shorter overall survival and recurrence-free survival than those with low CUEDC 2 expression ( P<0.05).Multivariate Cox regression analysis revealed 3 independent prognostic factors including CUEDC 2 expression, ser-um AFP and tumor number .CONCLUSION:CUEDC2 was expressed in most HCC tissues , which was relevant to tumor growth, tumor differentiation and prognosis .CUEDC2 could be a novel valuable molecular marker to predict the HCC prog-nosis.

Objective To investigate the role of microRNA-106a and microRNA-106b (miR-106a/b) in diagnosis and prognosis of hepatocellular carcinoma (HCC).Methods In this study,108 HCC patients and 54 age-and sex-matched healthy controls were enrolled.Blood samples were collected from each participant,and total RNA was extracted from the plasma.We determined miR-106a/b expression levels using quantitative reverse transcription polymerase chain reaction (qRT-PCR).Results The miR-106a/b expression levels in HCC patients were elevated compared with the healthy controls (P＜ 0.001).The ROC curve analysis showed that miR-106a/b expression levels could be used to predict the risk of HCC,with AUC values of 0.670 (95％ CI:0.573-0.768) and 0.684 (95％ CI:0.593-0.776),respectively.The miR-106a expression level in HCC patients correlated positively with hepatitis B surface antigen (HBsAg) presence (P =0.028),differentiation (P =0.025),tumor size (P =0.002),lymph node metastasis (P =0.028),and TNM stage (P =0.037).The miR-106b expression level correlated positively with HBsAg presence (P =0.003),alpha-fetoprotein (AFP) level (P =0.031),and tumor size (P =0.005).To further investigate the correlation of miR-106a/b expression levels with overall survival (OS),Kaplan-Meier curves were plotted.The results showed that HCC patients with high miR-106a expression displayed shorter OS (P =0.013).In addition,univariate and multivariate Cox proportional hazards regression showed that miR-106a was an independent risk factor for HCC prognosis.Conclusion Circulating miR-106a/b expression levels could be used as diagnostic biomarkers for HCC,and a high circulating miR-106a expression level is an independent risk factor for poor prognosis of HCC patients.

Objective To investigate the characteristics of adverse drug event (ADE) related to tacrolimus (Tac) in pediatric solid organ transplant recipients. Methods The data were retrieved from the US Food and Drug Administration Adverse Event Reporting System database from the first quarter of 2004 to the second quarter of 2023. The ADE data of pediatric organ transplant recipients with Tac as the primary suspected drug were extracted. The relationship between Tac and ADE was quantitatively analyzed by proportional imbalance method. Basic characteristics and signal strength of ADE related to Tac were analyzed. ADE related to Tac in children of different ages and different types of organ transplantation were analyzed. Results A total of 1 443 children's ADE reports involving Tac were screened, including 188 cases (13.0%) of heart transplantation, 668 cases (46.3%) of liver transplantation, 531 cases (36.8%) of kidney transplantation and 56 cases (3.9%) of lung transplantation. The median age of children was 10 years old. The top three countries with ADE reporting were the United States, France and the United Kingdom. China reported 26 cases, accounting for 1.8%. Infection and infectious diseases accounted for the highest proportion (20.96%) in ADE related to Tac, including EB virus and cytomegalovirus infection, etc. Infection and infectious diseases occupied the largest proportion of ADE related to Tac in children of different ages, whereas the pathogen types were different. Rejection, unstable immunosuppression level and renal function damage were also common ADE related to Tac in children of all ages. Nervous system disease was the main ADE in heart transplant recipients, while infection and infectious diseases were more common in liver and kidney transplant recipients. Rejection was the most common ADE in lung transplant recipients. Conclusions ADE related to Tac possess different distribution characteristics in different types of organ transplantation. Extensive attention should be paid to individualized drug monitoring and risk assessment in pediatric organ transplant recipients, thereby optimizing Tac treatment and reducing the risk of ADE.
