1.Therapeutical effect of mesenchymal stem cells overexpressing ICAM-1 in murine type 1 diabetes nephropathy

Objective To explore the curative effects of mesenchymal stem cells(MSC)that overexpress in murine type 1 diabetes nephropathy (DN).Methods Mice were randomly divided into normal control(NC) group,DN group,C3-treated group,C3-MIGR1-treated group and C3-MIGR1-ICAM-1-treated group.Mice were given streptozotocin until the DN model was set up.The murine DN model was treated with murine MSC(C3H10T1/2),transfection empty vector of murine MSCs(C3H10T1/2-MIGR1/MSC) and murine MSCs (C3H10T1/2-ICAM-1/MSC)that overexpressed ICAM-1.After transplantation, the pathological features of kidneys were observed by Masson staining and the number of homing MSC cells to the kidney was calculated on days 1,3,7 by frozen section, while qPCR was used to analyze the expression of signaling molecules for collagen1, TGF-β1 and SMAD2 after treatment with various MSCs.Results Compared with DN group, the renal fibrosis treated with MSCs overexpressing ICAM-1 was significantly decreased by Masson staining.Three and seven days after transplant, the homing cells of MSC in different groups displayed no difference using tissue freezing section method.Furthermore, TGF-β1/SMAD signaling was lowly activated after the treatment with MSCs that overexpressed ICAM-1 compared with model mice(P<0.01).Conclusion MSCs that overexpress ICAM-1 can protect kidneys in the DN model.
2.Practice experience of establishment of abdominal heart transplantation model combined with tail vein injection in mice (with video demonstration)

Objective To summarize the practice experience of establishing a stable abdominal heart transplantation model combined with tail vein injection in mice. Methods In the preliminary experiment, 50 pairs of donor and recipient Kunming mice received isotransplantation, 40 pairs of donor and recipient C57BL/6J mice underwent isotransplantation. In the formal experiment, 10 pairs of donor and recipient C57BL/6J mice received isotransplantation, 30 pairs of Balb/c mice as the donor and C57BL/6J mice as the recipient received allotransplantation. The time of each step of the heart transplantation (including harvesting and dressing of the donor heart, vascular anastomosis of the recipient, etc.) was recorded. The duration of transplanted heart beat and the survival time of the recipient was observed daily after operation. The time required for tail vein injection in the transplanted mice was recorded. 3.Diseases distribution and data analysis of children patients with allopatry direct settlement of medical insurance

Objective To research the performance of direct settlement for allopatry medical insurance at a tertiary specialized children′s hospital in Beijing, and to analyze the basics of these inpatients and the structural characteristics of their diseases, for basis and suggestions for improving the policy of direct settlement of allopatry medical insurance. Methods A statistical description of direct settlement data was made for allopatry medical insurance at this hospital from August 2017 to July 2018, to study the age, hometown, attribute, distribution of inpatient departments and disease distribution of these children. Results The patients mainly came upon referrals, preschool children in most cases. The average daily hospitalization cost of the disease was high relatively, and the number of hospitalizations was high as well. Compared with children inpatients in Beijing, these patients tend to come for surgical treatment for malignant tumors and congenital diseases.Conclusions The allopatry direct settlement of children′s medical insurance settlement is found mostly with diseases those are complicated and severe. The management of such settlement can be improved by cooperating with their local institutions in advance, data checking in the course, simplifying the formalities for special diseases, and the differential reimbursement ratio for these patients.
4.Surgical efficacy and prognostic factors for patients with advanced nasal and sinuses malignancies

OBJECTIVE To investigate the surgical efficacy and prognostic factors of patients with advanced nasal and sinuses malignancies,in order to provide more reference for surgical plan selection and prognosis evaluation.METHODS A total of 117 patients with advanced nasal and sinuses malignancies were retrospectively chosen in the period from January 2010 to January 2019 in our hospital.The clinical characteristics and follow-up survival data were analyzed,and the independent prognostic factors of advanced nasal and paranasal malignant tumors were evaluated by univariate and multivariate methods.RESULTS The progression free survival rate and overall survival rate were 48.71%and 62.39%,respectively.The median progression free survival time and overall survival time were 32.48 months and 39.80 months,respectively.Univariate and multivariate analysis showed that age,histopathological type,marginal status and whether adjuvant therapy was accepted were independent factors influencing progression free survival time and overall survival time after surgery for advanced nasal and sinus malignancies(P<0.05).CONCLUSION The surgical efficacy of advanced nasal and sinusoidal malignancies is satisfactory and the clinical prognosis is related to age,marginal status and adjuvant therapy.
5.Correlation between symptoms and their contribution to syndrome based on association rule combined with Bayesian network: syndrome of lung damp-heat accumulation in coronavirus disease 2019

Objective:To explore the correlation between symptoms and their contribution to syndrome based on syndrome of lung damp-heat accumulation in coronavirus disease 2019 (COVID-19), thus to provide methodological basis for the syndrome diagnosis.Methods:Based on 654 clinical investigation questionnaires data of COVID-19 patients, a model based on syndrome of lung damp-heat accumulation was set. Using SPSS Modeler 14.1 software, association rules and Bayesian network were applied to explore the correlation between symptoms and their contribution to syndrome.Results:There were 121 questionnaires referring to syndrome of lung damp-heat accumulation in total 654 questionnaires. The symptoms with frequency > 40% were fever (53.72%), cough (47.93%), red tongue (45.45%), rapid pulse (43.80%), greasy fur (42.15%), yellow tongue (41.32%), fatigue (40.50%) and anorexia (40.50%). Association rule analysis showed that the symptom groups with strong binomial correlation included fever, thirst, chest tightness, shortness of breath, cough, yellow phlegm, etc. The symptom groups with strong trinomial correlation included cough, yellow phlegm, phlegm sticky, anorexia, vomiting, heavy head and body, fever, thirst, fatigue, etc. Based on SPSS Modeler 14.1 software, with syndrome of lung damp-heat accumulation (yes = 1, no = 0) as target variable, and the selected symptoms with frequency > 15.0% as input variables, the Bayesian network model was established to obtain the probability distribution table of symptoms (groups), in which there was only one parent node (the upper node of each input variable) of fever, and the conditional probability was 0.54. The parent node of cough had yellow phlegm and syndrome of lung damp-heat accumulation, indicating that there was a direct causal relationship between cough and yellow phlegm in syndrome of lung damp-heat accumulation, and the conditional probability of cough was 0.99 under the condition of yellow phlegm. The common symptom groups and their contribution to syndrome were as follows: fever and thirsty (0.47), cough and yellow phlegm (0.49), chest tightness and polypnea (0.46), anorexia and heavy cumbersome head and body (0.61), yellow greasy fur and slippery rapid pulse (0.95).Conclusions:It is feasible and objective to analyze the correlation between symptoms and their contribution to syndromes by association rules combined with Bayesian network. It could provide methodological basis for the syndrome diagnosis.
6.Risk factors of urinary tract infection after renal transplantation

【Objective】 To investigate the independent risk factors of urinary tract infection (UTI) in recipients under-going renal transplantation of donation after brain death (DBD), so as to provide a theoretical basis for the prevention and control of postoperative UTI. 【Methods】 A retrospective study was conducted for recipients who received renal transplantation of DBD in our hospital during Jan.2021 and Dec.2021. The recipients were divided into the infection group (n=26) and non-infection group (n=74) according to the incidence of UTI 3 months after operation. The risk factors of UTI were identified with univariate and multivariate analyses. 【Results】 The incidence of UTI was 26%. Univariate analysis showed that gender, postoperative urinary fistula, time of indwelling catheter and time of indwelling double J tube were the influencing factors of UTI (P<0.05). Forward stepwise regression analysis showed time of indwelling double J tube (OR=1.086,95%CI:1.003-1.177,P=0.042) and time of indwelling catheter(OR=4.687,95%CI:2.064-10.645, P<0.010) were the independent risk factors of UTI (P<0.05). 【Conclusion】 The time of indwelling catheter and time of indwelling double J tube are the independent factors of UTI after renal transplantation of DBD.
