3.Melanin Pigmentation In Gingiva.

Oral melanin pigmentation is common in some races and ethnic groups. The gingivae are the most frequently pigmented intra-oral tissues. Melanin pigmentation is the result of melanin granules, produced by melanoblasts intertwined between epithelial cells at the basal layer of the epithelium. We present one case of melanin pigmentation of the gingiva of 26-year old female. Melanin depigmentation method is applying a 90% phenol solution to deepithelize pigmented areas, gingivectomy, epithelial abrasion, bone denudation, and split thickess flap. We chose epithelial abrasion using round diamond bur. The patient satisfies the result and have almost no pain and discomfort. But repigmentation potential must be noticed to patient.

4.Effect of Transscleral Diode Laser Photocoagulation Applied Through Silicone Scleral Exoplants.

Authors performed transscleral diode laser retinopexy through the conjunctiva, muscle and silicone scleral exoplant to evaluate its safety and efficacy in experimental rabbit eyes. Transscleral applications required energies of average 325. 2 mWwith duration of 0.5 second. The mean power required to produce comparable lesions through the conjunctiva, muscle and the buckle(MIRA #276)was 27%, 20%, and 33%higher, respectively. These results demonstrate that the diode laser photocoagulation applied through a scleral buckle is capable to produce chorioretinal scars without damaging the scleral tissue nor the buckling elements employed.

5.The Effects of Artificial Tear Formulations and Anti-inflammatory Agents on the Cultured Keratocytes of Rabbit.

We investigated the biological effects and cytotoxicity of the artificial tear formulations and anti-inflammatory agents on the cultured keratocytes of rabbit in vitro by using lactate dehydrogenase (LDH) leakage assay and transmission electron microscopy. In the artificial tear fomualations, the LDH titer of the Tears naturale and Protagent were more increased with time, but the Tears naturale II and Tears naturale free was slowly progressed or has narrow range of LDH increasing. In the anti-inflammatory agent, the LDH titer of the all eye solutions was increased with time, especially Maxidex and Fluorometholone. Non-steroid, Decrol was less than other anti-inflammatory agents in Na+, K+, Cl- electrolyte compositon. Especially, the LDH titer of the artificial tear formulations and anti-inflammatory agents containing the benzalkonium chloride (BAC) or low Na+, K+, Cl- composition was more increased with time. The typical rabbit keratocytes were elongated and had not prominent nuclei. But the higher the titer of LDH, the keratocytes were visible round and swollen, and were injured with the disruption of plasma membrane, vacuole formation in cytoplasm, and damage of nuclei in transmission electron microscopy. As the use of topical drug containing low Na+, K+, Cl- or Benzalkonium chloride should be induced a particulary toxic effect on keratocyte, we should be carefully use these topical eye solutions, especially when the epithelium is denuded.

6.Drainage of Subretinal Fluid with the Diode Laser.

We used 810 nm diode laser endophotocoagulator to perforate the choroid and retinal pigment epithelium for draining subretinal fluid during scleral buckling procedure. The laser settings reqiured for perforation ranged from 0.3 to 0.5 second of duration and from 300 to 500 mW of power. The average number of laser bums per site was 2.2 times(1-11times). Drainage was successful in all 42 rhegmatogenous retinal detachment patients treated. The complications associated with laser drainage included small subretinal hemorrhage in one eye(2.3%) and a faint laser bum on detached retina in one eye(2.3%). We believe that laser drainage is safe and requires no special skill.

7.The Effect of Mitomycin C(MMC) on Inhibition of Cellular Proliferation and Type_I Collagen, Laminin Synthesis of Pterygial Mesenchymal Cell.

The purpose of this study is to investigate that the biological effect of mitomycin C(MMC) on inhibition of cellular proliferation, extracellular synthesis of type_I coolagen, lamini, and study of myofibroblast is derived directly from the primary and recurrent pterygial mesenchymal cell by MMC concentration and duration of exposure time used clinically. Human pterygial mesenchymal cells were exposed for 3 minutes, 5 minutes, and 10 minutes to MMC 0.01%, 0.03%, 0.05%, and DMEM(control). After cells were incubated for 24 hours, [H3] thymidine proliferative assay, immunoassay of type_I collagen and laminin, immunohistochemical and ultrastructual study of -smooth muscle actin were perfromed in vitro. Recurrent pterygal mesenchymal cells were more proliferated and stronger than primary pterygial cells in proliferation and inhibition of cellular proliferation assay. In immunoassay of extracellular matrix, the higher the concentration of MMC and longer the duration of exposure time, the inhibition of laminin are strong. However, there was a little effect of inhibition of synthesis of type-I collagen. Also the results of positive responsed immunohistochemical and ultrastructual finding such as a few pinocytosis, microfilaments, microtendon, and basal lamina like materal by TEM of myofibroblast were revealed. We think that the reccurent pterygial tissue have more effect on inhibition of cellular proliferation and laminin synthesis than primary pterygium. Therefore, reconsideration of MMC concentreation and duration time should be need in case of recurrent tissue, further experimental and clinical research on the myofibroblast and inhibition of type-I collagen also should be need.

8.The bone formation around anodic oxidized titanium implants in the tinbiae of ovarectomized rats.

Anodic spark deposition method(ASD) surface treated titanium implant possesses a considerable osteoconductive potential that promoting a high level of implant osseointegration in normal bone. The purpose of this study was to observe the ASD implant's osseointegration in the osteoporosis-induced animal model. Twenty four rats, 10 weeks of age, were ovarectomized and 5 weeks later divided into two groups : ASD implant group and control implant group. Titanium screw implants (diameter; 2.0 mm, length, 3.5 mm; pitch-height, 0.4 mm) were designed for this study. Experimental implants were ASD treated and no treatment on control implants. ASD implants and control implants were placed in to left tibiae of rats. The rats were sacrificed at different time interval(1, 2, 4 and 8 weeks after implantation) for histopathologic observation and immunohisto -chemistrical observation, with collagen type I, fibronectin, integrin alpha2beta1 and integrin alpha5beta1 antibodies. The results obtained from this study were as follow: 1. Histopathologic findings, overall tissue response and the pattern of bone formation in both groups were similar. In ASD group, more newly formed bone was seen at 1 week and 2weeks than control group. 2. The levels of type I collagen and fibronectin expression were the most abundant at 2weeks and decreased gradually in both groups. Fibronectin and type I collagen expression in ASD group were stronger than control group but no significance. 3. The levels of integrin alpha2beta1 and Integrin alpha5beta1 expression were most abundant at 2 weeks and decreased gradually in both groups. No significant difference was observed in both groups. From this results, anodic oxidized titanium implants were more advantages in early stage of bone formation than control group, but have no significance in tissue responses and late bone formations. It could be stated that although anodic oxidized titanium implant possesses considerable osteoconductive potential but in osteoporotic bone condition dental implant procedure should performed after improving or treating the osteoporotic bone condition.
