The effects of hyperbaric oxygen on radiation cystitis have been reported in patients with radiation-induced hemorrhagic cystitis refractory to conventional therapy. Cessation of gross hematuria and reversal of cystoscopic bladder changes were seen in response to a series of hyperbaric oxygen therapy of 3 atmosphere absolute pressure for 2 hours. We report the experience of 2 cases of hyperbaric oxygen therapy for radiation-induced cystitis.

The purpose of this study is to investigate the clinical and histologic changes in distraction osteogenesis according to different distraction rates in the rat's tibia. Eighteen adult rats underwent open osteotomy and attachment of an external unilateral distraction device in the middle of left tibia. Latency was allowed for 7 days before distracton began. The distraction device was activated with varying distraction rates of 0.5mm, 1mm, 2mm and same rhythm of twice a day until 5mm length gain was achieved. The animals were sacrificed at post-distraction 4, 8 weeks to observe the bony healing states. At each group, clinical, radiographic and histologic studies were done. The results obtained from this study were as follows: 1. The 0.5mm group showed excellent osteogenesis than other groups. The new bone was formed by intramembranous bone formation mostly and endochondral bone formation partly. 2. The 1mm group showed delayed osteogenesis and incomplete bony healing at 8 weeks. 3. The 2mm group showed weak osteogenesis and fibrous union or nonunion at 8 weeks. From these results, it could be stated that distraction rate of 0.5mm per day was most useful in rat's tibia. The rate of 1mm showed delayed bony healing and needed more consolidation period. Distraction osteogenesis is a excellent clinical method for regenerating local bone deficiencies in limbs and craniofacial area. The more studies needed for the higher animals and human about distraction rates and other biomechanical factors on the basis of this study.

Takayasu Arteritis is a systemic disease of unknown cause characterized by occlusion of the thoracic and aMominal aorta and its branches. Although it is typically a disease of young oriental women, the disease is occasionally reported in other groups. A 29-year-old pregnant woman was admitted for emergency cesarean section. Anethesia was induced with thiopental and succinylcholine and maintained with vecuronium-nitrous oxide-oxygen. After delivery, anesthesia was supplemented with fentanyl and droperidol. After delivery we couldnt detect radial arterial blood pressure in both side. Our experience would provide basic information regarding possible emergency surgery for patients with this intractable disease.

STUDY DESIGN: A retrospective study. PURPOSE: To evaluate the prevalence and risk factors of asymptomatic cervical or thoracic lesions in elderly patients who have undergone surgery for lumbar spinal stenosis. OVERVIEW OF LITERATURE: Concurrent multiple spinal lesions have been reported in many studies with a varied prevalence, and described the characteristics of the disease and its treatment options. However, the cervical or thoracic lesions without apparent symptoms in patients with symptomatic lumbar stenosis had not been evaluated. METHODS: A total of 101 elderly patients (aged 65 or more), who had undergone surgery for lumbar spinal stenosis from January 2005 to December 2005, were enrolled in this study. All patients underwent lumbar magnetic resonance imaging (MRI) along with T2-weighted cervical and thoracic sagittal MRI prior to surgery. The concurrent cervical or thoracic lesions were classified according to the disease entity, and the severity of the lesions was graded from grade 0 (no lesion) to grade 4 (any lesion compressing the cord with a signal change). The prevalence of concurrent cervical and thoracic lesions was then analyzed. In addition, the risk factors for the development of concurrent lesions were evaluated, and the risk factors affecting the severity of the concurrent lesion were analyzed individually. RESULTS: Seventy-seven (76.2%) and 30 (29.7%) patients had a concurrent cervical and thoracic lesion, respectively. Twenty-six patients (25.7%) had both a cervical and thoracic lesion. There was a positive correlation between the symptom duration of lumbar stenosis and the prevalence of both cervical (p = 0.044) and thoracic (p = 0.022) lesions. CONCLUSIONS: The incidence of asymptomatic cervical or thoracic lesions is apparently high in elderly patients who have undergone surgery for lumbar spinal stenosis, particularly in those with longer symptom duration. This highlights the need for a preoperative evaluation of the cervical and thoracic spine in these patients.

Evans syndrome refer to autoimmune hemolytic anemia accompanied by thrombocytopenia. The etiologic factors of Evans syndrome are unknown but viral infections such as Epstein-Barr virus and cytomegalovirus are suggested. Epstein-Barr virus (EBV) infection is an acute illness characterized clinically by fever, sore throat, and lymphadenopathy; serologically by the transient positive heterophil antibody; and hematologically by mononuclear leukocytosis with atypical lymphocytosis. We experienced a case of Epstein-Barr virus infection presented as Evans syndrome. Epstein-Barr virus infection was diagnosed by positive heterophil antibody and EBV viral capsid antigen IgM. Autoimmune hemolytic anemia and immune thrombocytopenic purpura were completely resolved by steroid therapy.
