2.Bladder Management after Total Hip Arthroplasty under Spinal Anesthesia.

We conducted a retrospective study of 104 patients to examine the efficacy of two methods of urinary bladder management after total hip arthroplasty under spinal anesthesia. Patients who had total hip replacement was randomly assigned either to Group I, in which indwelling catheters were placed during the operation and removed the next morning, or Group II, in which urinary retention was managed by intermittent catheterization as needed. After the removal of the indwelling catheter, the patients in Group I had a lower incidence of urinary retention than those in Group II. Bladder distention(volume above 700ml) was more common in Group II and was associated with an increased need for subsequent long-term catheterization. There was no significant difference between the groups in the rates of urinary tract infection. We conclude that the short-term use of an indwelling catheter after extended surgery, such as joint replacement, reduces the incidence of urinary retention and bladder overdistention, without increasing the rate of urinary tract infection.

3.Functional Leg Length Inequality Following THA.

This is a follow up study on consecutive series 50 patients of total hip arthroplasty to assess for transient leg-length inequality. The authors report a clinical experience of 50 patients who have been pelvis tilt before total hip replacement. Retrospectively reviewed for the presence of pelvic obliquity preoperative and at the 1, 3 and 6 months follow-up visits. The results were as follows. Fourty-eight patients(96%) with functional leg length inequality related pelvic obliquity were improved within 6months postoperative follow up period after total hip arthroplasty. Two patients(4%) had persistent functional leg length inequality that affected factors-old age, female and soft tissue tightness due to pelvic obliquity. We believe that in most of these patients, the transient functional leg length inequality has a self-limited course, whereas in a small number, the profoud sense of inequality remains.

5.Results of Treatment in Childhood with High Hyperopic or Astigmatic Amblyopia.

We analized the results of treatment in 28 children with high hyperopic amblyopia or astigmatic amblyopia among 65 amblyopic children ranging from 3 to 10 years of age from Jan. 1985 to Dec. 1989. The results are as follows: 1. The final corrected visual acuities of both eyes above 0.8, according to Han's visual acuity chart, were attained in 7 cases (50.0%) among children with high hyperopic amblyopia and 4 cases (28.6%) among children with astigmatic amblyopia. 2. As the degree of hyperopic errors increased, initial corrected visual acuity decreased (p<0.05). 3. The initial corrected visual acuity was not associated with the degree of hyperopic errors, but could be correlated with the initial visual acuity examination under 5 years of age (p<0.025). 4. The final corrected visual acuity recovered was above 0.8, (with Han's visual acuity chart) in 11 eyes (78.6%) among 14 eyes with high hyperopic amblyopia under 5 years of age However above 6 years of age, only one eye (12.5%) among 8 eys was correctable. 5. As the degree of astigmatic errors increased, the final corrected visual acuity decreased (p<0.025).

6.Tendon Transfers in Traumatic Foot

A clinical review was performed of 11 cases of traumatic foot treated by tendon transfer at Severance Hospital during the period from April 1979 to March 1988 and the results are summarized as follows: l. Among 11 patient, there were 8 males and 3 females, ranging from 4 to 36 years of age. 2. Anterior tibial tendon was used in 6 cases, among these, split anterior tibial tendon was used in 3 cases. Posterior tibial tendon was used in 2 cases and extensor digitorum longus tendon of the foot in 3 cases. 3. In cases of soft tissue injury where tendon transfer was impossible, microvascular free cutaneous flap transplantation was combined with the tendon transfer in 6 cases. 4. Follow-up results of the 11 cases were obtained from 5 months to 9 years. Excellent results were obtained in 8 cases and satisfactory in 3 cases. For the correction of function loss in traumatic foot, tendon transfers were useful method for the recovery of active motion of the foot.

7.Orbitotemporal neurofibromatosis: a case report.

Neurofibromatosis is an autosomal dominant abnormality that may affect multiple organ systems. The eyelids, the orbits, the adjacent tissues and bones may be involved with varying frequency and severity. The management of orbitotemporal neurofibromatosis depends very much on the type and severity of the orbital involvement and on the functional state of the eye. Experience with surgical management of orbitotemporal neurofibromatosis involved in the orbit, the temporal soft tissue and bone with blind eye is reported. The goal of surgery is tumor resection, reconstruction of the orbital socket, aesthetic eyelids, and insertion of the artificial prosthesis. A two stage approach is recommended. In the first stage, tumor is resected and the orbital socket is reconstructed with titanium mesh plate and cranial bone graft. After reconstruction of the orbital socket, galeal flap is rotated posteriorly to cover the mesh plate and canthopexy is accomplished. Mask lift is performed to enhance aesthetics. In the second stage, correction of the bulky eyelids is achieved and orbital space for insertion of the artificial prosthesis is reconstructed. Authors have managed a orbitotemporal neurofibromatosis with blind eye of a 41-year-old male using titanium mesh plate and bone graft with satisfactory results.

10.Tinea Capitis of Adults in Taegu City for 11 Years ( 1978 ~ 1988 ).

Epiderniologic description of the tinea capitis of adults was made for the cases detected in Taegu city for 11 years from January 1978 to December 1988. 1. Totalnumber of adult patients(20 years or older) were 41(2.0%) out of 2,029 cases of all tiena capitis. Among 41 patients, 34 cases were detected between 1983 and 1988. 2. Clinically 24 patients were superficial tinea capitis and 17 patients were kerion celsi. 3. Age of the patients ranged from 21 to 85 years with an average of 58.5 years and mode interval of 61-70 years. Patients over 50 years of age accounted for 70.7% of all csses. 4. Sex distribution of the cases was 39 females(95.1%) and 2 males(4.9%). 5. Causative agents isolated from the patients were Microsporum canis in 36 cases, Trichophyton verrucosum in 2 cases, T. mentagrophytes, T. ferrugineum and T. rubrum in 1 case, respectively. 6. Solitary lesion was most common and alopecia was not prominent in most of the cases. The most frequent site of involvement was frontal and posterior scalp. At initial exarnination, 5 cases were misdiagnosed as seborreic dermatitis and 1 case as multiple folliculitis. 7. Sources of infection were presurned to be cat in ll cases, cattle in 2 cases and the rests were unidentified. Interfarnilial infection was observed in 5 cases.
