1.Tibial Lengthening in Familial Short Stature ( Classic Ilizarov Method v . s . Combined Intramedullary Nailing ).

Ilizarov technique has been successfully applied to limb lengthening for several decade, one of its main drawback is, however, long application of external fixator over 6 months. To lessen this time, simultaneous intramedullary fixation which convert later to static fixation by insertion of interlocking screws has been proposed. The indication for surgical limb lengthening includes familial short stature below the 3rd percentile. We compared the results of thirteen tibial lengthening with Ilizarov method to that of eleven hybrid methods. The hybrid system is composed of an unreamed AO intramedullary nail (IM) and ring fixator. Once the lengthening is completed, distal interlocking screws were inserted. In the group who were treated by hybrid system, the mean lengthening of tibia was 7.1cm (range, 6.3-9.5), the mean of external fixation period 7 months (range, 5-9), the mean healing index 1.2 month/cm (range, 1.1-1.4). On the other hand, the mean lengthening of tibia was 7.5cm (range, 5-10.3), the mean of external fixation period 9.8 months (range, 6-15), the mean healing index 1.4 month/cm (range, 1.1-1.8) in the group treated by Ilizarov method. Mean follow-up period was 23 months (range 14-47). There was no rotational or angular deformities and no loss of lengthening in the hybrid group. However we experienced three cases of angular deformity at the distraction site during lengthening in Ilizarov group. There was no deep infection in both groups. In the hybrid group we found several advantages such as no deformity, no loss of lengthening, and less scar by early removal of fixator, but some disadvantages such as need of subsequent procedures and limitation in intramedullary nailing.

2.The Clinical Usefulness of Cervicovaginal Fetal Fibronectin as a Prediction of the Time of Induction.

OBJECTIVES: Fibronectin is a high-molecular-weight glycoprotein found in the plasma and extracellular matrix. The presence of fetal fibronectin in cervicovaginal secretion of pregnant women during the second and the third trimester has been suggested as a means of identifying women in preterm labor who be delivered prematurely. Therefore, our purpose was to determine the clinical efficacy of the presence of fetal fibronectin in cervicovaginal secretion of patient in full term as biochemical predictor of Time of Induction. METHODS: Fetal fibronectin was obtained at external os and posterior vaginal fornix in 30 women in full term who had no uterine contraction with intact membrane. The cervix was assessed by digital vaginal examination and scored with a modified Bishop score. RESULTS: There was a good correlation in delivery time after induction between the fetal fibronectin and the modified Bishop score (r=0.695,P<0.005) CONCLUSION: The presence of fetal fibronectin is better than Bishop score that is dependent on clinical experience as biochemical predictor of Time of Induction.

3.Surgical Treatment of Upper and Lower Eyelid Retraction.

A combined technique for lid retraction, that includes recession of retractors and Mullerectomy with or without spacers was presented, Twenty two procedures on eleven patients were reviewed, Of 22 procedures, 14 were for upper eyelid retraction and 8 for lower eyelid retration. Preserved sclera as a spacer was used on 5 upper eyelids and 6 lower eyelids in selected cases, Eight patients with lid retraction had hyperthyroidism, two patients showed lid retraction due to previous lid or orbital syrgery and one patient revealed lagophthalmos caused by facial palsy, All patients showed excellent cosmetic appearance during the follow-up period of 6 to 32 months (average 15months), but two patients required reoperation with happy results.

4.A Case of Rotor Syndrome.

Rotor syndrome is a rare disease of hereditary hyperbilirubinemia transmitted with autosomal recessive trait. In general, Rotor syndrome shows direct hyperbilirubinemia and there has been several reports since Sons's report in 1966, in Korea. A 34-year-old female was admitted with the chief complaint of intermittent icteric sclera for 24 years. There was no family history of jaundice. Rotor syndrome was diagnosed by oral cholecystogram, BSP retention test, 99mTc-DISIDA scan, liver biopsy and electron microscopy study of liver biopsy specimen. We report this case with brief review of the literature.

5.Fine Structure and Glycoconjugate Complex Pattern of the Rat Vaginal Epithelium during Pregnancy.

The present study is designed to study the light and electron microscopic structure of the rat vaginal epithelium during pregnancy. Furthermore, lectin histochemistry is used to know the changes of the terminal sugar of the glycoconjugate in the vaginal epithelium during pregnancy. The 0 day of pregnacy is defined as the day of presence of sperm in a vaginal smear. At 0 day of pregnancy, most of cells are flat in morphology except basal cell. Lightly-stained superficial cells had few cell organelles, and all the other cells had many intermediate filament bundles, microvilli-like processes and desmosomes. During pregnancy, the thickness of the vaginal epithelium was increased. The morphology of the mucous cells are changed from a cuboidal shape to a columnar one. The intermediate filament bundles are decreased in the mucous cell after first week of pregnancy. Lectin histochemistry showed the presence of alpha-L-fucose, alpha-D-galactose, beta(1, 4)-N-acetylglucosamine, alpha-D-N-acetylgalactosamine and galactosyl-beta(1, 4)-N-acetylglucosamine in the mucous cells. The basal cells also contained the same terminal sugars except galactosyl-beta(1, 4)-N-acetylglucosamine. Approaching to the birthday, the thickening of the mucous layer of the vaginal epithelium suggests that the mucous containing several glycoconjugates may play an important role to make the appropriate environment in the vaginal lumen during pregnancy and parturition.

6.The intestinal parasite infection status of inhabitants in the Roxas city, the Philippines.

We carried out a small-scale survey to investigate the status of intestinal protozoa and helminthes infection of inhabitants in Roxas city, Mindoro, the Philippines. Total 301 stool samples were subjected to the formalin-ether concentration method for the detection of helminth ova and protozoan cysts. The overall positive rate was 64.5%, and that of male and female were 56.6% and 72.5%, respectively. The highest infected helminth was Ascaris lumbricoudes (51.2%), followed by Trichuris trichiura (27.6%), hookworm (8.0%) and Enterobius vermicularis (0.3%). The protozoa infection status revealed that Entamoeba coli was the most frequent (15.0%). Iodoamoeba buetschlii and E. histolytica were found but few. The multiple infection more than two parasites was 29.6%, and double infection with A. lumbricoides and T. trichiura was most common. The intestinal helminth infections were highly prevalent in this area, according to this result, and we concluded that anthelminthic drugs should be given to inhabitants, especially to children of 1 to 15 years-old.

8.Evaluation of Korean Corneal Storage Medium with New Formula in Cornea.

PURPOSE: We investigated the viability, the changes of thickness, and morphological changes of corneas stored in the Korean corneal storage medium (CS005) by comparing with those of corneas stored in Optisol GS (Chiron Co., Irvine, CA, USA). METHODS: Cat corneas preserved in the CS005 and Optisol GS were stored for up to 14 days at 4degrees C. The endothelial viability, ultrastructure, and the change of thickness were assessed by microscopy. In addition, the corneas preserved for 3 and 5 days were transplanted to cats and, after 3 months, the endothelial changes of transplanted corneas were evaluated using the scanning electron microscopy. RESULTS: Corneas preserved in CS005 or Optisol-GS showed the similar endothelial viability and cell density as the time of preservation. Cornea stored in both medium showed no significant difference in the change of corneal thickness by 7 days although the corneas stored in CS005 were slightly thicker by 14 days than tissues stored in Optisol-GS (193.6% vs. 164.7%, respectively). Ultrastructural examination of the corneas preserved in both medium showed the cytoplasmic vacuoles, nuclear changes, and edema with increasing the storage time. Transplanted corneas showed the similar structural changes in endothelial cell stored in CS005 or Optisol-GS. CONCLUSIONS: The endothelial viability and changes of corneas preserved in Korean corneal storage medium were similar to that in Optisol-GS. Therefore, this study suggests that corneal tissue can be safely stored in the Korean corneal storage medium, considering that donor corneas were generally used within 7 days after preservation.

Primary malignant melanoma of tbe esophagus is extremely rare and its incidence is below 0.1%. The tumor is polypoid and tend to be large, which is covered with false membrane, friable, hemorrhagic, and necrotic. Hematogenous and lymphogenic metastasis are common. Resection of the tumor with an anastomotic procedure seems to be the treatment of choice, and postoperative irradiation may be useful. But, despite these measures, prognosis is poor, with a 5-year survival of 4.2%. We report a case of 58-year-old man with primary malig- nant melanoma of the epophagus, and review of the literature in presented related studies.

10.Does Home Oxygen Therapy Slow Down the Progression of Chronic Obstructive Pulmonary Diseases?.

PURPOSE: As the National Health Insurance Service (NHIS) began to cover home oxygen therapy (HOT) services from 2006, it is expected that the new services have contributed to overall positive outcome of patients with chronic obstructive pulmonary disease (COPD). We examined whether the usage of HOT has helped slow down the progression of COPD. METHODS: We examined hospital claim data (N=10,798) of COPD inpatients who were treated in 2007~2012. We performed chi2 tests to analyze the differences in the changes to respiratory impairment grades. Multiple logistic regression analysis was used to identify factors that are associated with the use of HOT. Finally, a generalized linear mixed model was used to examine association between the HOT treatment and changes to respiratory impairment grades. RESULTS: A total of 2,490 patients had grade 1 respiratory impairment, and patients with grades 2 or 3 totaled 8,308. The OR for use of HOT was lower in grade 3 patients than others (OR: 0.33, 95% CI: 0.30~0.37). The maintenance/mitigation in all grades, those who used HOT had a higher OR than non-users (OR: 1.41, 95% CI: 1.23~1.61). CONCLUSION: HOT was effective in maintaining or mitigating the respiratory impairment in COPD patients.
