The question of infectivity and prevalence of Clonorchis sinensis in Cheju province (Quelpart Island) was arisen to authors since the several surveys on the endemic diseases were performed in this island. Therefore, authors decided to solute this question. Then the survey on the possible second intermediate host of Clonorchis sinensis was performed and reported already with negative finding. At present time, authors carried out the collection of all kinds of fresh-water snails through all areas of this island, to confirm the presence or not of Parafossarulus manchouricus, the only first intermediate host of Clonorchis sinensis in Korea . And the following results were obtained. The fresh-water snails collected in the survey were Semisulcospira libertina Gould and Lymnaea ollula Gould. Parafossarulus manchouricus Bourguigant was not collected in this island. It is confirmed that the Clonorchis sinensis can not be prevalent in this island.

4.Clinical Features of Anterior Ischemic Optic Neuropathy.

PURPOSE: To analyze the clinical features of anterior ischemic optic neuropathy (AION) and to evaluate the visual prognosis METHODS: Medical records of 52 eyes in 47 patients who were diagnosed with AION from January 1991 to December 2000 were retrospectively reviewed. The clinical features, associated systemic disease, visual fields findings, and fundus findings in the patients were analyzed. RESULTS: The mean age of the patients was 55.9 years (20~81 years). Twenty-six were men (55.3%) and 21 were women (44.7%). The initial visual acuity of less than 0.1 was 24 eyes (46.2%), that of 0.1 to 0.5 was 23 eyes (44.2%), and more than 0.5 was 5 eyes (9.6%). In seasonal onset, frequency of AION during the winter was 18 eyes (37.5%). As the associated systemic diseases, 15 had diabetes mellitus (31.9%), and 27 had hypertension (57.4%). In laboratory examination, erythrocyte sedimentation rate (ESR) was elevated in 4 patients (8.5%). In fundus examination, the optic disc swelling was found in 16 eyes (30.8%), and optic disc atrophy was found in 36 eyes (69.2%). In Goldmann perimetry, the altitudinal field defect was found in 23 eyes (44.2%), most commonly. The mean follow-up period was 1.7 years (2 months ~ 9 years), and the visual acuity improved in 13 eyes (25.0%), worsened in 6 eyes (11.5%), and did not change in 33 eyes (63.5%) at the final follow-up. CONCLUSIONS: In conclusion, patients with AION showed various clinical features and overall visual prognosis is poor. Because AION is frequentely associated with systemic diseases, careful evaluation on the systemic disease is mandatory.

5.Effect of Propofol on Neonate in Emergency Cesarean Section.

BACKGROUND: Many reports claim that propofol anesthesia in cesarean section is safe for neonates, but these studies concern cases of elective, or combined elective and emergency surgery. Our study was designed to investigate the effects of propofol on neonates in emergency cesarean section. METHODS: One-hundred patients for cesarean section were involved. They were divided into 2 main groups; (P) anesthesia with propofol (N = 50) and (E) enflurane (N = 50). Both P and E were subdivided into 2 groups each; (PE) propofol elective (N = 25); (EE) enflurane elective (N = 25); and 2 emergency subgroups, (PM) propofol emergency (N=25), and (EM) enflurane emergency (N = 25). Apgar score (1 min, 5 min), Neurologic Adaptive Capacity Score (NACS, 15 min, 24 hr), cord blood gas analysis (cBGA) were measured. Maternal venous and umbilical artery and venous blood concentration of propofol were measured at delivery. RESULTS: The values of the Apgar score, NACS, and cBGA of group P were not different from the corresponding values of group E. Also there were no differences between groups PM and EM, and between groups PE and EE. The blood concentration of propofol was 2.36 microgram/ml in the maternal vein, 1.13 microgram/ml in the umbilical vein, and 0.89 microgram/ml in the umbilical artery. CONCLUSIONS: From these results, it may be suggested that propofol is safe for neonates in emergency cesarean section surgery.

6.Chronotropic Actions of Higenamine in the Isolated Right Atrium of the Rabbit.

Higenamine is known to possess stimulatory activity on beta-receptor of the heart. Chronotropic actions of higenamic were studied in spontaneously beating right atrial muscle isolated from rabbits. The frequency of spontaneous beating and the relative threshold voltage of the right atrium were examined. The relative threshold voltage was defined as the minimal voltage of the given impulse above which the right atrium could be paced at the frequency of 20% of higenamine was also observed. Higenamine caused the postive chronotropic effect. This response became prominent as the ca2+ concentration in the bathing solution lowered. When tetrodotoxin was added to the bathing solution, the effect of higenemine altered and became similar to that of epinephrine. Higenamine reduced the relative threshold voltage of the right atrium in the bathing solution with [ca2+] of 0.5mM. Such effect was abolished by tetrodotoxin. The effects of verapamill on the spontaneous rate and the relativel threshold voltage were inhibited by higenemine. The above results suggest that, aithough the main action og higenamine is on the Ca channel, higenamine also have a minor effect of augmenting the Na channel.

9.Evaluation of the Singh index for Measurement of Osteoporosis.

PURPOSE: The Singh index which is based on a plain radiograph has commonly used in assessing the grade of osteoporosis. We studied value of the Singh index in assessing the grade of osteoporosis by obtaining reliability of the Singh index and correlation between the Singh index and bone mineral density. MATERIALS AND METHODS: Six observers assessed radiographs of 60 patients suspected osteoporosis. The interobserver and intraobserver agreement of the Singh index were obtained by using kappa statistics. The bone mineral density of proximal femur was measured by dual energy X-ray absorptiometry in all patients, and then the results of bone mineral density were compared with those of Singh index by using percentage of variance. RESULTS: The kappa values of interobserver agreement ranged from 0.28 to 0.76 (mean value: 0.45) and the strength of the agreement was moderate. The percentage of variance of BMD which was explained by Singh index was 7.1%. CONCLUSIONS: The results of this study suggest that the Singh index has low clinical value in assessing the grade of osteoporosis
