1.Variations of the Straight Sinus in Korean Adults.

The complexily in development of the dural sinus may result in many variations. Studies are being done on the variation of the straight sinus for clinical application of neurological diagnosis and operation. However, no data is available on Koreans, which made this study necessary. The present study was made on 50 Korean adult cadavers (average age of 67). The results were as followlng. 1. A bulbous formation of the rostral aspects of the straight sinus was present in 22 cases(44%). 2. Complete double straight sinus was present in 3 cases(6%). 3. Partial septum was observed in 8 cases (16%) with varied position and length. 4. The straight sinus terminated into both transverse sinuses(52%), the left transverse sinus(24%), the confluence of sinuses (18%), and the right transverse sinus(6%). 5. Average length and width of the straight sinus were 48.5mm and 5.9mm, respectively. 6. Nineteen cases(38%) showed no tentorial sinus drainage into the straight sinus on both sides. 3.Partial Purification and Characterization of 41 kDa Serine Proteinase from Culture Filtrate of Trichophyton tonsurans.

Dermatophytes infect the human hair, skin, nail and cause the dermatophytosis. The extracellular and intracellular proteinases of the dermatophytes commonly occur in the genus Trichophyton like T. rubrum, T. mentagrophytes, and T. granulosum. These enzymes play a prominent role in growth, multiplication and infection of the host tissue. Extracellular proteinases have been purified from the species of Trichophyton and Microsporum. We purified the proteinase partially from the culture filtrate of the Trichophyton tonsurans through Mono-Q and Superose 12 column and investigated its biochemical and enzymatic characters. The molecular size of the proteinase was estimated to be 41 kDa by SDS-PAGE. And pI was 3.2. The optimal temperature and pH for an enzymatic activity were 27C and 7.5, respectively. The purified porteinase degraded the keratin, bovine serum albumin, hemoglobin. The serine proteinase inhibitor like PMSF and DFP inhibited the proteolytic activity of the purified enzyme whereas the cysteinase inhibitor did not. These results demonstrated that the purified proteinase is a serine proteinase and can contribute the tissue invasion.

5.A case of pemphigus erythematosus.

We present here, a case of pemphigus erythematosus developed in a 31-year-old woman, who had been suffering from repeated episode, of remission and exacerbation with several erythematous, and somewhat oozing patche accompanying a mild pruritus or her face, upper part of the back and anterior chest. In serological studies, she had antinuclear antiboc@lies at. a titer of 1:160. Histopathological finding showed subcorneal bullae. Immunofluorescent study revealed IgG and C, deposition in the intercellular space of the epidermis and along the de rmoepidermal junction.

7.A Case of Acute Suppurative Thyroiditis.

Acute suppurative thyroiditis is a rare because of high resistance to bacterial infection of thyroid gland, which is rich vascularity and lymphatic drainage, a high iodine content and complete encapsulation. The common clinical manifestations are fever, neck pain and localized mass of thyroid area. This thyroiditis is more common in left thyroid lobe. The most important causal microorganism are staphylococci and streptococci, with frequent isolation of mixed flora and anaerobes. Diagnosis was easily made by typical clinical manifestation, ultrasonography, thyroid imaging and fine needle aspiration. Treatment usually consist of appropriate antibiotic therapy and surgical drainage when abscess formation develops. We report a typical case of acute suppurative thyroiditis in 1 year old female infant with brief review of literatures.

9.Progression of Dupuytren's Contracture after Partial Fasciectomy.

PURPOSE: This study reported the clinical results of partial fasciectomy on Dupuytren's contracture and evaluated the factors associated with progression. MATERIALS AND METHODS: A retrospective analysis was performed on 32 hands in 28 patients who had subtotal fasciectomy for Dupuytren's contracture from 1992 to 2007. We evaluated complications and clinical results by Honner's classification. Through telephone interviews, we checked the progression of contracture including recurrence and extension in twenty three patients with a mean postoperative period of 5.9 years. The relationship between progression and several factors were analyzed using statistical analyses. RESULTS: Clinical results were excellent in 13 cases, good in 12, fair in 4, and poor in 3. Progression rate was 48% (11patients). It occurred at operated fingers in 3 cases, non-operated fingers in 7 cases and both in 1 case. Progression was correlated with smoking and was more common in patients with bilateral disease, but not with alcohol, age or severity. Complications occurred in 4 patients (1 complex regional pain syndrome, 1 nerve injury and 2 delayed wound healing). CONCLUSIONS: Partial fasciectomy is a suitable method for Dupuytren's contracture. Complication rate is not high if careful operation is performed. However, progression is common in patients with bilateral disease or smoking habitus.

10.Evaluation of 82 Cases of Oval Optic Intraocular Lens Implanted in the Bag.

The postoperative results of 77 paients:(82 eyes) who had undergone phacoemulsification with implantation of oval optic poly methylmethacrylate (PMMA) introoular intraowlar lens (IOL) (5 X 6mm) were evaluated retrospectively. Scleral pocket incision was done tangentially at the point 2mm. apart from the surgical limbus and 5mm in length. No suture in the group 1 (30 eyes), single horizontal suture in the group 2 (31 eyes) and single X suture in the group 3 (21 eyes) were used for the closure method. Average uncorrected visual acuity at postoperative one week was 0.5 or better in each group and there was no statistically significant difference among three groups (p>0.05). The change in mean keratometric astigmatism from postoperative one to eight weeks turned out to be less than 1 diopter (D) in each group and there was no statistically significant difference among three groups (p>0.05). When the diameter of continuous cirular capsulorhexis (CCC) was greater than 5mm or the position of CCC was decentered, it was found that optic was partially escaped form the bag due to adhesion between the remained anterior and posterior capsular structure. It could be suggested that oval optic PMMA IOL showes early visual recovery with less astigmatism regardless of the suture method and in order to place the optic of IOL properly in the bag postoperatively, the diameter of CCC should be less than 5mm and the position of CCC should be centered.
