PURPOSE: Healey divided segment IV of the live as the 'superior portion (IVa) ' and the 'inferior portion (IVb) '. On the contrary, Couinaud suggested that there was no useful purpose in dividing segment IV because of several reasons. Our goal is to evaluate the safety of the 'isolated IVb (inferior) resection of the liver' via performing the dissection of cadavers. METHODS: There were ten total cadavers. Cadaver dissection proceeded with respect to the Glissonian pedicle, the portal vein and the bile duct, respectively. The total number of Glissonian pedicles at segment IV was measured. The distance between the origins of the IVa and IVb branches was measured. Additional pedicles that were known to exist at segment IVa were also evaluated. RESULTS: The mean number of Glissonian pedicles in segment IV, IVa, and IVb was 5 (+/-1.3), 1.6 (+/-7), and 3.4 (+/-0.9), respectively. The mean distance between the origins of the IVa and IVb branches was 5.6 mm (+/-3.9 mm). Two of 10 cases had a very short distance between the origins that were considered as having common origin. Additional pedicles were identified at the Lt. main Glissonian pedicle in all the cases (8 cases: 1 each, 2 cases: 2 each). CONCLUSION: Considering the possible existence of a common origin of segment IVa and IVb Glissonian pedicles, there is the risk that the segment IVa will be injured during 'iso lated IVb resection'. Inevitable ligation of the additional pedicle of segment IVa from the Lt. main Glissonian pedicle can be made during the 'isolated IVb resection'. Therefore, we think that 'isolated IVb resection of the liver' can be safe only when the surgeon divides the branches of segment IVb with meticulous preservation of the IVa branches.

PURPOSE: This study was aimed at measuring the changes in the hyoid bone position, tongue position, and pharyngeal airway space in subjects with mandibular setback osteotomies. METHODS: Twenty patients were evaluated retrospectively for their changes in pharyngeal airway space, tongue and hyoid bone positions. All patients underwent surgical mandibular setback using bilateral sagittal split osteotomies. The cephalometric analysis was performed preoperatively, and 1 week, 3-6 months, and 1 year postoperatively. RESILT: The hyoid bone moved inferiorly and posteriorly immediately after surgery, and it returned to the preoperative position during follow-up period. The nasopharyngeal airway space was not significantly changed after surgery. A considerable decrease in the oropharyngeal and hypopharyngeal airway spaces following mandibular setback surgery was found. The upper and lower tongue was posteriorly repositioned immediately after surgery. During follow-up period, the hypopharyngeal airway space and lower tongue posture returned to the preoperative positions, but the oropharyngeal airway space and upper tongue posture were not significantly changed. The position of pogonion remarkably changed to backward immediately after surgery, but slightly anterior advancement was found during follow-up period. CONCLUSION: Immediately after mandibular setback surgery, the oropharyngeal and hypopharyngeal airway spaces obviously decreased due to posterior and inferior repositions of the tongue and hyoid bone. During follow-up period, lower tongue and hyoid bone returned to the preoperative positions, it was related to advancement of the pogonion in this period. The narrowing of the oropharyngeal airway space and posterior movement of the upper tongue posture were relatively permanent after mandibular setback surgery. We suspected this phenomenon had an influence on maintaining the total volume of oral cavity against mandibular setback.

There are various surgical methods for reconstruction of the nasal defect. Among them, there is some difference in the choosing the proper reconstruction method according to defect size and position. When the defect involved the tip, the columella, and the alar, the local flaps may be preferred, because they can provide sufficient amount of tissue. However, the composite grafts from the ear have been effectively used in reconstructions of smaller sized defects of the columella and ala. We excised total external nose because of squamous cell carcinoma on the nasal tip, columella, and nasal septum. We reconstructed the nasal tip, both alae, and columella with forehead flap. After division of the regional flap, we found partial necrosis of the columella and narrowness of the nostril. So, we used chondrocutaneous auricular composite grafts for reconstruction of the columella and both nostrils. We used the file-folder designed auricular composite graft for reconstruction of columella and the wedge shaped ear helical composite grafts for widening of nostrils. 6 months later, there were no significant problems, except some mismatched dark color in the grafted alar tissues. Here, we report a successful reconstruction of large nasal defect using combined two different reconstructive methods.

The patient who was 50 year old man had been affected generalized hyperpigmentation with pnpillomatosis especially on forehead, neck, axilla, umbilical area, anogenital area, palm and soles and associated with highly malignant adenocarcinoma of stomach. Gastric tuberculosis is a rare disease and the diagnoeis of gastric tuberculosis is based on either positive histological or bacteriological study. We experienced a case of gastric tuberculosis with pleural effuaion in 59 year-old woman, diagnosed by the histological study of the endoscopic biopsy specimen. Follow up endoscopic finding revealed marked improvement of gastric lesion after anti-tubercadoua medication. We reported the case with review of literature.
