1.Congenital Pulmonary Lymphangiectasis.

Congenital pulmonary lymphangiectasis is a rare cause of severe respiratory distress in the newborn period and most of these patient die, usually within the first 24 hours of life. The light microscopic characteristics of the lungs are a network of partly tubular, partly cystically enlarged lymph vessels within large areas of connective tissue and they have thin wall lined by endothelium. Also congenital pulmonary lymphangiectasis is closely associated with cardiovascular malformation, and most frequently occurs in the clinical setting of congenital heart disease with or without pulmonary venous obstruction(total anomalous venous return, atresia of large pulmonary veins) The authors describe three typical cases of congenital pulmonary lymphangiectasis.

2.bcl-2 and MIB-1 Expression in Basal Cell Carcinoma.

BACKGROUND: bcl-2 is a newly characterized proto-oncogen that has been shown to suppress programmed cell death(apoptosis), which is involved in tumorigenesis, and its expression has been demonstrated within tumor cells in a variety of neoplasms. OBJECTIVE: We examined whether there are differences in bcl-2 expression in basal cell carcinoma and several epidermal keratinocytic tumors. In addition, we evaluated bcl-2 expression according to histological types of basal cell carcinoma as well as the interaction of expression of bcl-2 and MIB-1 protein in BCC subtypes. METHODS: Routine paraffin sections of formalin-fixed sixty tissues(20 BCC, 10 Bowen disease, 10 keratoacanthoma, and 20 SCC) were labelled with anti-bcl-2 monoclonal antibody and MIB-1 using a labelled streptavidin-biotin complex. RESULTS: 1. Twenty cases of basal cell carcinoma were examined and all expressed cytoplasmic bcl-2. Three out of 20 cases of squamous cell carcinoma were focally positive. None of the 10 Bowen disease cases and the 10 keratoacanthoma cases expressed bcl-2. 2. In BCC, bcl-2 immunoreactivity was great in the superficical subtypes which had an indolent growth variant, moderate in the circumscribed types, and weak in the infiltrative types which had aggressive growth variants. 3. Evaluation of the distribution of bcl-2 immunoreactive categories and MIB-1 grades revealed a negative correlation tendency, but no statistical significance. CONCLUSION: Our results suggest the presence of a protection mechanism from apoptosis mediated by bcl-2 protein involved in the neoplastic growth mechanism of BCC. In addition, the observed findings in the expression pattern of bcl-2 and MIB-1 in the BCC subtypes may be due to interaction between bcl-2 and other apoptotic-related oncogens.

3.Ulnar Nerve Entrapment Syndrome by epitrochleoanconeus Muscle: A Case Report

A forty-seven-year-old male miner complained of paresthesia over right hypothenar area and ring and little fiugers for 3 months. On physical examination and electromyography, ulnar nerve dysfunction below elbow was noted. Intraoperatively, an anomalous muscle, the epitrochleoanconeus muscle, was found between the triceps muscle and flexor carpi ulnaris muscle, but it was clearly distinguished from them. Resection of the muscle and medial epicondylectomy were performed. The epitrochleoanconeus muscle is an anomalous muscle which arises from the medial epicondyle of the humerus and inserts into the medial border of the olecranon. The reports on ulnar nerve entrapment syndrome by this muscle were rare. On 14 months follow up, the symptoms and signs were improved.

4.Effect of Ginseng on the Blood Pressure and Lipid Metabolism, during Development of Hypertension in Spontaneously Hypertensive Rat.

Korean Ginseng (Panax C.A. Meyer) has survived empirical efficacy as tonic and geriatric agents for several thousands of years in oriental herbal medicine. But there has been numerous controversial reports about its use in aged hypertensive men due to its allegedly hypertensive effects. Therefore, the author conducted the pharmacological studies of Korean Ginseng extracts on the blood pressure and blood lipid metabolism, during development of hypertension, using SHR(Spontaneously Hypertensive Rat), counterpart model of human essential hypertension. The results obtained were as follows: 1. SHR could be grouped, according to their age, as prehypertensive stage(<6th week after birth), labile hypertensive stage(6th 14th week), early established hypertensive stage(14th-20th week), and late established hypertensive stage(>20th week). 2. Plasma renin activity tended to rise steadily, until established hypertensive stage was reached. Thereafter, plasma renin activity tended to decline. 3. The changes of blood pressure due to Ginseng extract, depended on the amount, route and duration of its administration. Low dose of Ginseng tended to increase blood pressure but high dose of Ginseng tended to decrease blood pressure, until 1 week of intraperitonal administration. But prolonged administration of Ginseng extract beyond 1 week, showed delayed hypertensive effect in the intraperitoneal administration but not in oral administration. 4. SHR seemed to have abnormalities in the lipid metabolism. SHR showed lowered level of serum cholesterol and phospholipid, whereas slightly higher level of triglyceride, and showed lower alpha-lipoprotin fraction but higher pre beta-lipoprotein fraction, as compared with NCR.(=Normal Control Rat). 5. When high fat cholesterol salt diet was fed on, it was possible to indece hyperlidemia and increment of hypertension in SHR. but when high fat cholesterol salt diet and Ginseng extract were fed on concomitantly, Ginseng showed significant inhibiting effect on the development of hyperlidemia and hypertension.

6.Correlation between Microalbuminuria Checked by RIA & Micral-Test®.

It is evident that an elevation of airway albumin excreation rate without clinical proteinuria strongly predicts a later progression on diabetic renal disease. So we studied the correlation between Microalbumin checkly RIA & Micral-Test®. We collected urine between 08:00 h and 08:00 h next day and then checked microalbuminuria by radioimmunoassay method and Micral-Test® The results are as follows: 1. There was significant correlation between microalbuminuria checked by RIA & Micral-Test® 2. There was poor correlation between diabetes duration or HV-A1c and maximal change in albumin excreation rate. 3. So we concluded that Micral-Test® can be used in laboratory instead of RIA.

10.Tendon Transfer with a Miarovascular Free Flap in Injured Foot of Children.

We performed tendon transfer with a microvascular free flap for recovery of handicapped function and reconstruction for the skin and soft tissue loss. We review the clinical data of 11 children who underwent these operation due to injured foot by pedestrian car accident from January, 1986 to June, l994. The mean age of patients was 5.6 years old (3-8). Five cases underwent tendon transfer and microvascular free flap simultaneously. Another 6 cases underwent operations separately. The time interval between tendon trasnfer and microvascular free flap was average 5.8 months (2-15 months). The duration between initial trauma and tendon transfer was average 9.6 months (2-21 months). The anterior tibial tendon was used in 6 cases. Among these, the technique of splitting the anterior tibial tendon was used in 5 cases. The posterior tibial tendon was used in 3 cases and the extensor digitorum longus tendon of the foot in 2 cases. Insertion sites of tendon transfer were the cuboid bone in 3 cases, the 3rd cuneiform bone in 3 cases, the 2nd cuneiform bone in I case, the base of 4th metatarsal bone in I case, and the remnant of the extensor hallucis longus in 3 cases. The duration of follow-up was average 29.9 months (12-102 months). The clinical results were analyzed by Srinivasan criteria. Nine cases were excellent and 2 cases were good. The postoperative complications were loosening of the tranferred tendon in 2 cases, plantar flexion contracture in l case, mild flat foot deformity in I case and hypertrophic scar in 2 cases. So we recommend the tendon transfer with a microvascular free flap in the case of injured foot of children combined with nerve injury and extensive loss of skin, soft tissue and tendon.
