2.3 Cases Report of Myositis Ossificans Progressiva

Myositis Ossificans progressiva is a very rare disease and not more than five cases have been reported in this country. The symptoms usually appear before the age of six and no case has ever been reported in medical literature in which patient is older than the age of twenty. This author reports here that a 52 years old male was diagnoed as a myositis ossificans progressiva. The patient developed his symptoms when he was lynched by a club during the Korean conflict at the Korean conflict at the age of thirty. Another interesting fact is that is that his daughter has also developed similar symptoms which appeared following an accident when she was run over by an angry cow at the age of six.

3.Posteromedial Release in Infancy for Resistant Congenital Clubfoot

Ten feet from 6 idiopathic congenltal clubfoot patients which had failed to respond to conservative mearsured were treated by aggreasive posteromedial release during infancy with generally good results. A modltication of Denis-Browne splint by splitting both aides and using dial lock to hold the foot still ln correeted position was found quite useful for post-operative maintenance.

4.“V” Shape Corrective Osteotomy of the Long Bone

French method of wedge osteotomy has been widely used to correct the cubitus varus deformity, but it is difficult to correct the rotational deformity, gunstock deformity effectively and to fix the distal fragment rigidly because the supracondylar area of the humerus has an anatomical characteristic of short distance in A-P plane. We can design a new method of “V” shape osteotomy to correct the cubitus varus and apply this method to the deformities of the tibia and femur. Ten cases of “V” shape corrective osteotomy were performed in the Department of orthopedic Surgery, Kyung Hee University. Of these, 5 cases were cubitus varus deformities, 3 cases were deformities of tibia and 2 cases were deformities of femur. The results were obtained as follows. 1. The cubitus varus and gunstock deformities are corrected effectively without unnecessary bony loss. 2. The rotational deformity can be corrected in stable position due to the “V” shape contact surface. 3. The contact area is so wide and oblique that it may be ease to manage the distal fragment and we may expect good union process with early ROM exercise due to rigid fixation. 4. Additional change of the corrective angle can be possible. 5. This method is also effective for the correction of the deformity caused by partial epiphyseal closure of the long boen, using partial open wedge osteotomy.

5.Niclosamide in the treatment of tetagonimiasis.

Anthelmintic effect of niclosamide [N-(2-chloro-4-nitrophenyl)-5-chloro-salicylamide)] on Metagonimus infection was carried out to the cases in area of Seomjin-river. The drug, one tablet contains 0.5 g of niclosamide was supplied from Bu-gwang pharmaceutical company in Seoul. Sixty five cases were divided into three groups by the regimen: Group l given 2 g(4 tablets) for one day, Group II for two successive days and Group III for three successive days. Stool examination was done before and 2-3 weeks after the drug administration by formalin-ether concentration and Stoll's dilution egg counting techniques. The results were as follows: Egg negative conversion (cured) and egg reduction (more than 80 percent decrease) rates were 28.6 percent, 10.7 percent in group I, and 31.8 percent, 27.3 percent in group II. But of those in group III treated for three successive days, 73.3 percent was cured and 26.7 percent showed egg reduction, and the general egg reduction rate was 95.1 percent. The results in the present trial suggested that the niclosamide has anthelmintic efficacy when it is used in 2 g for more than three successive days.

6.A Case of Oligodendroglioma of Cerebellar Vermis in a Child.

The incidence of oligodendroglioma in pediatric age is less than 1%, and about 90% of them are located supratentorially. The authors are reporting a case of oligodendroglioma at the cerebellar vermis in a 14 years old boy which was cured by surgical resection and shunt procedure.

7.Clinical Evaluation of Cervical Water-soluble Metrizamide Myelography via C1-2 Puncture.

A total of 32 cases of cervical myelography via lateral c1-2 puncture using water-soluble metrizamide was evaluated. Twenty seven cases were suspected to have herniated cervical disc and five cases, spinal cord tumor. Patients were placed in prone position with head and neck slightly extended on the radiolucent operating table. Puncture was performed using 22-guage spinal puncture needle placed at the junction of the middle and posterior one-third of the bony spinal canal and 4-6mm inferior to the arch of atlas on lateral projection. The needle was positioned posterior to the spinal cord into the subarachnoid space. The author could obtain more clear images with less amount of contrast medium than doses used in conventional cervical myelography via lumbar route. The were relatively few adverse reactions and no considerable complications have been encountered with a new cervical water-soluble metrizamide myelography via C1-2 puncture.

9.Usefulness of Gastroscope in Patients with Incomplete Colonoscopy.

BACKGROUND/AIMS: Conventional colonoscopy is limited in some patients with several causes, such as fixed angulation, extensive loop or adhesion. Therefore, small-caliber scopes are considered alternatives to unsuccessful conventional colonoscopy. The aim of this study is to evaluate the usefulness of gastroscope in patients with unsuccessful colonoscopy. METHODS: From May 2008 to April 2009, a total of 2,548 colonoscopies were performed in Wonkwang University Hospital. The gastroscope was used subsequently when conventional colonoscopy failed. RESULTS: There were 27 cases (1.06%) of unsuccessful colonoscopy. The causes of failure were assessed as fixed angulation of the sigmoid colon (59.2%, 16/27), excessive looping (14.8%, 4/27), stricture (14.8%, 4/27), sigmoid diverticulosis (7.4%, 2/27), and adhesion of transverse colon (3.7%, 1/27). The average time of intubation to cecum with the gastroscope was 7 minutes and 28 seconds (range, 2 to 20 minutes). With the gastroscope, 77.8% (21/27) were intubated to the cecum. The pain score of gastroscope was reduced, as compared with that of conventional colonoscopy (4.95 vs. 5.94, P<0.001). CONCLUSIONS: Gastroscope would be a useful alternative tool in patients with unsuccessful colonoscopy.

10.Malignant Fibrous Histiocytoma in psoas muscle: Case report.

Malignant fibrous histiocytoma (MFH) is a soft-tissue sarcoma that usually occurs in middle-aged or elderly patients, has a similar male-to-female ratio, and rarely develops in the retroperitoneum, especially in the psoas muscles. The lower extremity is the most common site of origin, followed by upper extremity, the trunk, and the retroperitoneum. Primary retroperitoneal involvement of a malignant fibrous histiocytoma is often diagnosed at an advanced stage as it has no specific symptoms. Primary retroperitoneal involvement of a malignant fibrous histiocytoma is very rare and there is no case report of MFH in the psoas muscle in Korea, as introduced here.
