2.Pulmonary Metastasis of Malignant Meningioma.

We report a case of 41-year-old male with pulmonary metastasis from a recurrent intracranial malignant meningioma, which had two more recurrence, multiple intracranial metastasis. Pulmonary metastasis was developed five years after resection of primary lesion. The metastatic routes and treatment of pulmonary metastatic malignant meningiomas were described.

3.Immunomodulators Extracted from Korean - style Fermented Soybean Paste and Their Function . 1 . Isolation of B Cell Mitogen from Korean - style Fermented Soybean Paste.

Responses of mouse lymphocytes to the soybean paste fermented by Korean traditional fashion was examined to clarify its effects in cytokine production in vitro. A fraction of the soybean paste (KFSP-100) was prepared by precipitation with ammonium sulfate and by filtration through ultrafiltration membrane. KFSP-100 were added into cultures of fresh mouse splenic cells in vitro. KFSP-100 significantly enhanced the amount of IL-6 and TNF-a produced by macrophages and IL-6 and IFN-r produced by lymphocytes. Production of IL-12 by macrophages was not much affected by KFSP-100 treatments. The most noticeable finding was the fact that lymphocytes treated with KFSP-100 proliferated to an exceeding numbers (more than 10 times to the control) in 72 hours. The KFSP-100-induced proliferative response was specific to B cells since almost all of the KFSP-100-induced cells in the cultures of splenic cells were B cells. Furthermore, such a proliferative responses were equally observed only in cultures of purified B cells but not in cultures of T cells. In thermostability test, the biologically active components of the KFSP-100 is assumed to be either linear protein or glycoprotein. KFSP-100 did not induce agglutination of lymphocytes demonstrated by lectins in the same cells. These observations suggest that KFSP-100 may be a novel mitogen for B lymphocytes. The component (s) responsible for the B cell proliferation in KFSP-100 might be a factor gained by natural fermentation. None of the fractions of not fermented soybean paste prepared by the same methods demonstrate the same effect.

5.In Vitro Release of L-dopa and Dopamine by Genetically Modified Fibroblasts of the Rat.

A construct DNA containing the cDNA for rat tyrosine hydroxylase(TH) and another construct DNA containing the cDNA for bovine L-dopa decarboxylase(DDC) were made and those genes were transferred by calcium phosphate transfection method into the immortalized rat fibroblasts. A both TH- & DDC-positive clone was identified by immunocytochemical staining. These cells produced L-dopa and dopamine and released them into the cell culture medium in vitro, which was confirmed by high performance liquid chromatography. These results have general implications for the application of gene therapy and specific implications for Parkinson disease.

7.Microsurgical Treatment of Middle Cerebral Artery Aneurysms.

The authors analyzed 50 microsurgically treated middle cerebral artery(MCA) aneurysms among 266 intracranial aneurysms in the Department of Neurosurgery, Yonsei University Hospital from September 1975 through September 1982, in order to study the differences from those of other intracranial aneurysms, and analyze the surgical outcome versus preoperative clinical status of the patients, surgical timing, and the microsurgical techniques. The results of the analysis were summarized as follows ; 1) The middle cerebral artery aneurysms occupied 19.5% of the microsurgically treated intracranial aneurysms. 2) The brain CT scan showed subarachnoid hemorrhage in 77.5%, hemorrhage in the unilateral Sylvian cistern in 39.7%, and intracranial hematoma in 48.4%. 3) The most common location of middle cerebral artery aneurysms was the M1 M2 junction in 84%, and MCA bifurcation aneurysms occupied 44%, and trifurcation ones 40%. 4) Clipping with reinforcement of the aneurysms were performed in 74% of total cases due to difficult complete clipping of the aneurysmal neck. And the rest were wrapped. 5) The surgical timing was around the 3rd week after the last bleeding in most cases. 6) Patients in Botterell's grade I to III showed no motality and good surgical outcome. Only one case of grade V expired postoperatively, and morbidity was 10%.

8.MR Finding of the Pleomorphic Xanthoastrocytoma in a Child: Case Report.

Pleomorphic xanthoastrocytoma(PXA) is a benign brain tumor with favorable prognosis in spite of its marked pleomorphism. We report a case with review of the literature. An eight-year-old boy presented with headache and vomiting. Plain skull X-ray showed findings of increased intracranial pressure. The MR imaging demonstrated superficially located large cystic mass with a Iobulated mural nodule in the left frontal lobe. The mural nodule abutting leptomeninges showed intense enhancement on postcontrast Tl-weighted images, but the cyst wall was not enhanced. Preoperative diagnosis of PXA with the MRI findings could lead to avoidance of more aggressive brain resection and therefore more favorable neurological outcome
