1.A Case of Marfan Syndrome with Left Atrial Myxoma Confirmed by Echocardiography.

The Marfan syndrome, a rare disease causing a marked decrease in life expectancy by involving the skeletal, ocular, and cardiovascular systems, is known as a connective tissue disorder that is inherited autosomal dominant. The cardiovascular complications directly related to the cause of death are associated with more than 90% of the Marfan syndrome. Aortic aneurysm with rupture could occur because of dilatation of aorta due to defect of media. Aortic and mitral insufficiency, mitral valve prolapse, bacterial endocarditis, arrhythmia, and aneurysm of interatrial septum are also frequently observed. A few literatures were reported domestically pertaining to the Marfan syndrome thus far, and there are increased discoveries of cardiovascular complications of the syndrome with the application of echocardiogram. However, the Marfan syndrome with LA myxoma has never been reported both domestically and internationally(INDEX MEDICUS, 1966-1989). Therefore we hereby report a case of the Marfan syndrome with LA myxoma from the observation of a patient who was admitted to Eulji General Hospital at Taejeon i August of 1989.

2.Nursery Room Nurses' Role Performance for Maternal Role Attainment of Mothers at Early Postpartum Period.

The early postpartum period is crucial toward in recovery form childbirth and attainment of the maternal role. Maternal role attainment is a complex social and cognitive process of stimulus-response accomplished by learning. Helping for maternal role attainment is one of nursing goals in the early postpartum period. Based on King's conceptual framework for nursing, this study was planned as descriptive correlation study to determine the significant differences of the degree of nursery room nurses' role performance according to several variables of personal, interpersonal, and working system of nurses in nursery room, The purpose of this study was to contribute to the planning of nursing care to help maternal role attainment of the early postpartum period of mothers and to the development of relevant nursing theory, The data were collected from R3b. 3 to 28 by questionnaires with 273 nurses in nursery room. The instruments for this study were consisted of four parts: 21 questions for roll performance of nurse ; 37 questions for personal system of nurse including 31 questions for role perception of nurse ; 65 questions for interpersonal system including 63 questions for job stress of nurse ; 18 questions for working system of nurse. The toes to measure role performance and role perception, and job stress of nurse were tested for internal reliability. Cronbach's Alphas were 0.9612, 0.9058, and 0.9649. The data were analysed by using in S.A.S. computerized program and included percentage, t-test, ANOVA, Pearson Correlation Coefficient, and Duncan multiple range test. The conclusions obtained from this study are summarized as follows : 1. The mean score of the items of role performance was 2.12(SD=0.55) in Likert's 4 points scale. 2. The degree of role performance was significantly different according to role perception(p=0.0001), age(p=0.006), educational background(p=0.002), and certificate of midwife(p=0.03) among variables of personal system of subjects. 3. The degree of role performance was significantly different according to job stress(p=0.0001) and numbers of children(p=0.006) among variables of interpersonal system of subjects. 4. The degree of role performance was significantly different according to having opportunities for baby(p=0.03), the degree of flexibility to bring baby to mother's room(p=0.046), the scope of visitor for baby(p=0.016), the degree of flexibility of visiting for baby(p=0.049), the degree of participation of nurse in establishing visiting rules(p=0.017), existence and/or nonexistence of rules for breast feeding(p=0.010), existence and/or nonexistence of education for breast feeding(p=0.009), existence and/or nonexistence of breast feeding room(p=0.013), concret methods for breast feeding(p=0.003), working place(p=0.0001), and career(p=0.019) among variables of personal system of subjects.

4.The Pattern of Self Reported Assessment(Alcohol Use Inventory and Michigan Alcohol Screening Test) according to Receptive and Awareness of Drinking Problem and the Relation to the Alcoholic Type.

OBJECTIVES: This Study was designed to investigate the pattern of AUI(Alcohol Use Inventory) and MAST(Michigan Alcohol Screening Test) according to the awareness of drinking problem and receptiveness to treatment, and to know whether the type(type1 and type 2 according to Barbor's typology) has effects on the RECPAWAR(acknowledgement and awareness of use problem and receptive to treatment of these problem) score. METHODS: Subjects were 134 alcoholic patients from Alcoholic treatment ward. They were requested to fill up the questionnaires of Korean version of AUI and MAST(MAST for patient and MAST for caretaker). At first, based on the RECPAWAR, they divided into high RECPAWAR group(above 30 percentile) and low RECPAWAR group(below 30 percentile). According to typology, they divided into type 1 and type 2. The data was analyzed by T-test, Correlation, and Regression Analysis. RESULTS: 1) Significant negative correlation was observed between RECPAWAR score and MAST difference score(patient's MAST-caretaker's MAST). 2) High RECPAWAR group had significantly higher scores in the most scales than low RECP-AWAR group, except ENHANCED scale in AUI. 3) There was not significant difference on RECPAWAR score between type 1 and type 2. CONCLUSION: Except ENHANCED scale, the high RECPAWAR group had higher score in the second level scale. As compared with the low RECPAWAR group, the high RECPAWAR group responds more negative in drinking, and is more anxious about their drinking problems. So, they seem to have the objective view about their drinking problems. However the type was not related to receptive and awareness of drinking problem.

5.A Case of Steroid Treatment on Haze after LASIK Retreatment in eye with previous PRK.

PURPOSE: A corneal haze after LASIK retreatment in eye with previous PRK has not been reported in Korea. We experienced a case of myopic regression and haze after LASIK retreatment and treated it with fortified steroid eye solution. Herein we will present it. METHODS: A 38-year-old woman underwent the PRK for the correction of myopia in 1994 and the bilateral LASIK retreatment of myopic regression in January 2000. One year later, the left eye showed myopic regression and severe corneal haze but right eye was nearly emmetropia. PredForte(R) was dropped 4 times daily for 3 months, tapered, substituted with Flumetholon(R) for the next 3 months. We observed for 6 months. RESULTS: Three months later, refraction became nearly emmetropic and uncorrected visual acuity was 0.6. Corneal haze, central keratometric and pachymetric value were much decreased. It remained stable for 6 months.

6.Influence of Food Behavior and Life-Style Behavior on Health Status in Male Industrial Workers.

The aim of this study was to investigate the effects of food behavior and life-style behavior on the health-status of male industrial workers in the Masan region. The average age of the 173 male subjects was 42.9 years and 59% of the subjects were labor workers, 25% office workers, and 70% of them earned 1 - 1.5 million won monthly. The subjects were categorized into one of three groups : normal group, health-concerned group, and disease-suspected group classified by the criteria of the data (blood pressure, blood glucose, blood hemoglobin, serum total cholesterol, and serum triglyceride) obtained from a health examination. Forty seven belonged to the normal group, 71 to the health-concerned group, and 55 to the disease-suspected group. The health-concerned group, and disease-suspected group had significantly higher systolic and diastolic blood pressure, blood glucose, and serum total cholesterol than the normal group, and the disease-suspected group showed a significantly higher serum triglyceride level than the normal and health-concerned groups. The disease-suspected group consumed carbohydrate foods, such as cereals more frequently and protein foods such as beans and eggs less frequently than the normal group and health-concerned group. However, there was no difference in nutrient intakes among the three groups. The disease-suspected group and health-concerned group smoked more cigarettes and drank more frequently than the normal group, and the disease-suspected group exercised less as compared to the normal group. The kinds of diseases diagnosed in the disease-suspected group were hypertension, hyperlipidemia, liver disease, and neurological disease. The results of this study indicate that nutrition education and monitoring should be implemented for industrial workers to prevent chronic diseases and to reduce medical cost for the treatment of disease.

7.The Effects of a Group Educational Intervention on the Prevention of Osteoporosis in Breast Cancer Patients.

PURPOSE: This study was conducted to examine the effects of osteoporosis prevention education on Korean breast cancer patients' knowledge and health promoting behavior. METHODS: The participants in this study included breast cancer patients who were registered at a self-help group organized by the Breast Cancer Center of a university hospital in Seoul, Korea. The intervention group received 60 minutes'didactic instruction followed by 30 minutes' interactive session, and reinforcement education with leaflets was given three weeks after the group intervention. In order to measure the effects of the intervention, this study used valid and reliable scales on the knowledge of osteoporosis and health promoting behavior for preventing osteoporosis. A post-test was conducted 12 weeks after the intervention. RESULTS: The results showed significant improvement in three of the five factors of osteoporosis knowledge including bone physiology, the characteristics of osteoporosis, and preventive behaviors in the intervention group. Overall health promoting behavior for preventing osteoporosis had significantly increased in the intervention group compared to the control group. CONCLUSION: Osteoporosis preventive education improved breast cancer patients' knowledge of osteoporosis and health promoting behavior for preventing osteoporosis by inducing their voluntary participation in self-care activities.

10.Ossifying Fibroma: 3 Cases Report

Ossifying fibroma is one of a group of fibro-osseous lesion which arises typically within the jaw bone and only rarely affects the long bones. In 1966, Kempson described two patients with ossifying fibroma of the tibia, noting its unique, aggressive local behavior. This rare lesion resembles monostotic fibrous dysplasia, but can be differentiated from it on histologic and clinical ground. Histologically, ossifying fibroma is characterized by osteoblasstic rimming spicules of bone within a fibrous stroma with evidence of the lamella transformation of bone, and clinically by, frequent recurrence and aggressive progression of the lesion. We experieced three cases of ossifying fibroma, two cases in the tibia and one case in the fibula. In case 1, recurrence was developed after local curettage and bone graft, so reoperation was performed at 11 months after the first opecation. The 2nd case was healed after wide curettage and free vascularized fibula graft, and the 3rd case being followed up after segmental resection of the fibula.
