1.In vitro and In vivo Inductino of Osteogenesis in Cultured Mesenchymal Stem Cells Isolated from Rat Bone Marrow.

OBJECTIVES: To induce osteogenesis of cultured mesenchymal stem cells(MSCs) isolated from rat bone marrow in vitro and in vivo. METHODS: Bone marrow cells were isolated from Spraugue-Dawley rats and seeded in culture medium. MSCs were isolated from bone marrow cells and cultured. For in vitro induction of osteogenesis, MSCs were cultured in osteogenic medium. After 2 weeks, evaluation of osteogenesis was performed with alkaline phosphatase(ALP) activity, ALP immunochemical stain, von-Kossa stain for mineral deposition, and Northern blot analysis for osteocalcin m-RNA. For in vivo induction of osteogenesis, MSCs were seeded on Collagraft strips. The Collagraft strips seeded with MSCs were grafted in the subcutaneous tissue of the back of immunocompromised rats. Grafted Collagraft strips were harvested 3, 6, 9, 12 weeks after graft and stained with H-e ad toluidine blue-O stai. RESULTS: In vitro assay ALP activity was 121.27 p-nitrophenol nmol/106cell in osteogenesis induced MSCs and 75.53 p-nitrophenaol nmol/106cell in control groups. Osteogenesis induced MSCs were stongly stained by ALP immunochemical stain. Von Kossa stain showed black mineral deposition. There were strong bands in Northern blot analysis for osteocalcin m-RNA. H-E stain showed penetratin of cellular component into the pores of Collagraft strip in all specimen and showed formation of osteoid from 6 weeks after graft. Toluidine blue-O stain showed blue staining cartilagious component from 3 weeks after grafting. CONCLUSION: We could induce osteogenesis from mesenchymal stem cells in vitro and in vivo. Based on the results of this study, bone graft might be able to replaced by a new method in the near future.

2.Association of cesarean delivery with increases in maternal body mass index.

OBJECTIVE: Our purpose was to evaluate whether maternal body mass index measured before pregnancy are associated with an increased risk of cesarean delivery. METHODS: Maternal weight and height were prospectively collected on 819 women who delivered between 37 weeks and 42 weeks in the College of Medicine, Catholic University of Taegu-Hyosung. Statistical analysis was done using Chi-square tests, prepregnancy maternal weight and height were used to calculate the body mass index, and its contribution to the risk of cesarean delivery was determined. Women with congenital anomaly and pregestational diabetes were exclude from analysis. RESULTS: The analysis of risk factors for cesarean delivery in the 819 women revealed a decreased risk of cesarean delivery with maternal age < or =20 years and multiparity; increased risk of cesarean delivery was noted with maternal age >30 years. Increase in prepregnancy maternal body mass index and total weight gain were significantly associated with increase in the odds of cesarean delivery. CONCLUSION: The risk of cesarean delivery is associated with incremental changes in maternal weight and body mass index before pregnancy after adjustment for potential confounding factors. Prepregnancy counseling about optimizing maternal weight and monitoring weight gain during pregnancy to decrease the risk of cesarean delivery are supported by this study.

3.A Clinical Study of Vascular Injuries in Extremities

Thirty years ago a major arterial laceration was devasting injury, threatening survival of the limb in almost every case. Increased awareness of vascular injuries and of feasibility of immediate and more advanced arterial repair techmique reduced the amputation rate. The authors reviewed 163 patients of vascular injuries in extremities, who were admitted to Severence Hospital of the Yonsei University College of Medicine from January, 1979 to December, 1986. The results were follows : 1. Among the 163 patients, 146 patients were male patients. 2. Most of the victims were in their twenties. 3. The most common cause in the upper extremity was glass injury. The most common cause in the lower extremity was being struck by moving vehicles. 4. More complications in vascular injuries were associated with severe soft tissue crushing and comminuted fracture of the bone. 5. More favorable results can be gained through the use of microvascular techniques than through conventional techniques in vascular reconstruction.

5.The Clinical Study of the Proximal Shaft Fracture of the Femur in Children

In general, satisfactory results can be obtained by nonoperative means in the management of proximal femoral shaft fracture in children. Howerver, it is sometimes difficult to maintain the fracture alignment by the nonoperative means because of different muscle pulls in the thigh. The proximal fragment tend to be displaced and roentgenograms are difficult to be taken during traction. These problems have led some authors to advocate open reduction and internal fixation which facilitate overall care. We report our experiences on nonoperative and.operative treatment for proximal femoral fracture at the Chon-ju Presbyterian Medical Center from February 1987 to January 1991. The results are as follows ; 1. There were 20 cases in male, 7 cases in female. Average age at injury was 6.9 years (range, 6 months to 12 years and 8 months). Traffic accident was the most common cause (70.3%). 2. Average immobillization time was 9.2 weeks (range, 6.0 weeks to 12.1 weeks) in the nonoperative treatment group(17 cases), while 8.1 weeks(range, 6.1 weeks to 11.4 weeks) in the operative treatment group (10 cases). 3. Average bony union time was 9.6 weeks (range, 6.3 weeks to 12.5 weeks) in the nonoperative treatment group(17. cases) and 11.2 weeks (range, 9.0 weeks to 13.4 weeks) in the operative treatment group (P < 0.05). 4. 6.Percutaneous Balloon Mitral Vavuloplasty under General Anesthesia in Patient with Mitral Stenosis and Schizophrenia.

Since the introduction of percutaneous mitral valvotomy(PMV) in 1984, PMV was an effecive alternative to surgical commissurotomy in selected patients with severe mitral stenosis. Also PMV was an excellent palliative strategy in such patients who were high risk for operative management or longterm anticoagulation was not feasible. We performed PMV in a patient with very tight mitral stenosis and severe pulmonary hypertension under the general anesthesia because the patient was anticipated to be uncooperative due to mental problem, who was diagnosed as schizophrenia 16 years ago. Robinol was used for premedication and i.v fentanyl was used for maintenance of anesthesia. Inoue balloon was introduced into the LV and gradual ballooning was performed with favorable results. Total anesthesia time and interval from internal jugular vein puncture to the completion of valvuloplasty were 1 hour 45 min and 40 minutes respectively. Hemodynamic variables were improved immediately after intervention and mitral valve area was increased from 0.5cm2 to 1.3cm2.

9.Evaluation of mechanical and projective standardization in digital subtraction radiography.

The following conclusions were obtained from the non-reconstructed and reconstructed subtraction images of the standard intraoral radiographs which were taken with paralleing technique with Rinn XCP only and with occlusal bite registration for geometric standardization using bilateral mandibular premolar and molar regions of two dry humad skulls. 1. The SD of the overall subtraction images of the premolars and molars of the non-reconstructed, tnat is, the manual superimposition showed statistically significant difference between the non-registered and registered groups. 2. In non-reconstructed and non-registered cases, the quality of the subtraction images were improved when superimposition was focally done and this was more evident in areas where the radiographic imaged tend to be distorted due to anatomic readons. 3. In non-restructed and registered cases, the subtraction images were consistent regardless of the anatomic site or the focus superimposition. This means that the geometric standardization with only occlusal bite registration could produce serial radiographs which is suitable for subtraction. 4. The SD of the overall subtraction images of the premolars and molars of the reconstructed, that is, the automatic superimposition showed statistically insignificant difference between the non-registered and registered groups. This means that using reconstruction, subtraction radiography is possible without occlusal bite registration. 5. In reconstructed and non-registered cases, compatible quality of the subtraction images were obtained regardless of the anatomic site or area of the corresponding points. 6. In reconstructed and registered cases, best subtraction images whose quality showed sensitivity to the areas of corresponding points were obtained.
