2.A Case of Sporotrichosis developed after Double Eyelid Operation.

A case of sporotrichosis, developed on left upper eyelid in 2 weeks after double eyelid operation, was presented. Eruptions were distributed along the operation site and similar to pyoderma or keloid. Painless subcutaneous nodule appeared near the lateral angle of eye in 8 months after onset. Sporotrichum schenckii was cultivated from the tissue fluid of the lesion. PAS positive spores were found extra-cellularly and in histiocytes and multi-nucleated giant cells.

3.Three Cases of Sporotrichosis Confined to the Face.

We experienced 3 cases of sporotrichosis confined to the face. They were lymphatic type, pustular type and fixed ulcerative type of cutaneous sporotrichosis respectively. Two types of them except the lymphatic type were very difficult to be diagnosed correctly with clinical features. So, we reviewed the types of sporotrichosis and the problems in establishing the diagnosis. 3 cases are as follows: Case l. A 57 years old female second hand dealer had a ulceration measuring 2-2cm in diameter on the right dorsum of the nose after injuring with sharp rusty iron material. Soon after ulceration healed, 5 beaded nodules measuring 0.5-1cm in diameter newly developed on the right side of the maxillary area. Case II. A 55 years old female house wife had a suppurated ulcer with serous exudate, which looked like impetigenous lesions, on the left mandibular area, without any apparent trauma history. Case III. A 52 years old female had a rice grain sized papule, which was grown slowly in size, on the left maxillary area without any apparent trauma history. Skin graft was performed after excision of the nodule, because it was diagnosed as skin cancer at a local clinic. A large ulceration with necrosis of the tissues developed on the very site of skin graft performed, accompanied by a bean sized nodule below the ulceration.

4.Classification of the Carbohydrate using pectin Binding Characteristics and PAS Reaction in Human Gingiva.

This study was performed in order to recognize the identifications of the glycoproteins containing oligosaccharides in human gingiva. After made paraffin sections of human gingiva at 4µm, the sections were incubated with 7 lectins (UEA-I, BS-I, SBA, DBA, WGA, PNA, PNA after neuraminidase treated, Con-A). In order to increase specificity of reactions, the sections were applicated with ABC system. And then the sections were incubated with DAB and were counterstained with hematoxylin. Using the same sections, the sections were done H-E and PAS stains. In WGA, DBA and Con-A, plasma membranes of the layers of all epithelium and connective tissue were stained. In BS-I ; In the epithelium of marginal gingiva, plasma membranes of upper layer of the spinous cell layer and granular cell layer were stained. And in epithelium of sulcular gingiva, plasma membranes of the all spinous cell layer and granular cell layer were stained. In SBA ; Plasma membranes of the granular cell layer were stained. In PNA ; In the epithelium of marginal gingiva, plasma membranes of the basal cell layer and lower layer of spinous cell layer were stained. But lectin reactions were not occurred in thc sulcular gingiva. In PNA treated neuraminidase, plasma membranes of the all epithelial layer except basal cell layer membranes especially cytoplasms of upper layer at the sulcular gingiva and connective tissue were reacted. 1. By the above results, authors could know the identification of oligosaccharides existing g1ycoproteins in the human gingiva. 1) All epithelial layer ; α-D-N-Acetyl-Galactosamine, Sialic acid, D-Glucosamine, α-D-Mannose 2) Basal cell layer ; Galactose-β-(1-3)-N-Acetyl-Galactosamine 3) Spinous cell layer ; α-D-Galactose, Galactose-β-(1-3)-N-Acetyl-Galactosamine 4) Granular cell layer ; α-D-Galactose 5) Connective tissue ; α-D-N-Acetyl-Galactosamine, Siallic acid, β-(1-4)-D-Acetyl-Glucosamine, α-D-Glucosamine, α-D-Mannose 2. The Galactose-β-(1-3)-N-Acetyl-Galactosamine was not existed in the basal cell layer and spinous cell layer in the sulcular gingiva.

5.Treatment of Obstructive Colorectal Cancer.

The occurrence of the colonic obstruction secondary to colorectal carcinoma (CRC) has been reported in 7~30% of the CRC patients. It is generally believed that obstructive CRC is associated with a poor prognosis with respect to operative mortality and five-year survival. A series of 1064 cases of the CRC treated surgically at Asan Medical Center from June 1989 to December 1996 has been analyzed to compare clinicopathological findings between obstructive and non-obstructive CRC and to evaluate surgical treatment options in obstructive CRC. Complete obstruction was present in 49 cases (4.6%). There were no differences between obstructive and non-obstructive CRC in tumor location, size, Dukes' stage, and differentiation. In forty-nine obstructive CRC cases, primary resections were performed in 29 cases after peri-operative bowel decompression. In this group, right colon cancer was more prevalent than staged operation group (45% vs. 5%, P<0.05) and hospital stay was significantly short (16 days vs. 38 days, P<0.05). Postoperative complication rate was higher in staged operation group (65% vs. 28%, P=0.01). It may be due to stoma related wound complication. In obstructive left colon cancer, there was a significant difference in complication rate between primary resection and staged operation (P<0.05). Overall 5-year survival rate were 66% and 53% in non-obstructive and obstructive group, respectively. Survival rate according to the Dukes' B and C stages did not show statistical differences, either. Conclusively, primary resection is preferred to the obstructive CRC when supportive care, preoperative bowel decompression, and intraoperative colonic irrigation were performed adequately.

7.Cultural Characteristics of Veiled Lady Mushroom, Dictyophora spp..

This study was carried out to obtain the basic data for artificial culture of veiled lady mushroom (Dictyophora spp). The optimal conditions for the mycelial growth were 25degrees C and pH 5.0 for all isolates except the optimal temperature of 30degrees C for D. echinovolvata ASI 32002 and Phallus rugulosus . The optimal medium for Dictyophora spp. was PBA (potato bamboo sawdust extract agar) medium. The strain ASI 32002, D. echinovolvata , grew faster than. D. indusiata ASI 32003 and Phallus rugulosus ASI 25007 on the medium. Carbon sources such as glucose, maltose and inuline were favorable for stimulating a mycelial growth of the two strains of ASI 32002 and ASI 32003. Asparagine and glutamine appeared to be favorable to the strain ASI 32002 and ASI 32003, where as alanine, one of nitrogen source also favorable to the strain ASI 32002. The optimum C/N ratio of the two isolates of ASI 32002 and ASI 32003 was about 25 : 1 when 2% glucose as carbon source was mixed with the basal medium. While, in the case of 4% as carbon source, the optimum C/N ratio was about 30 : 1.

9.Differential Findings of Ictal EEG Pattern between Mesial and Neocortical Temporal lobe Epilepsies.

BACKGROUND: Neocortical temporal lobe epilepsy (NTLE) appears to have different characteristics in clinical and electrophysiological aspects from mesial temporal lobe epilepsy (MTLE). Many semiological studies on TLE have been performed. We investigated ictal EEG patterns to differentiate NTLE from MTLE. METHODS: One hundred twenty nine scalp ictal EEG recordings came from 27 pathologically proven TLE patients were analyzed by 1) frequency of ictal discharge at EEG seizure onset (alpha, beta, theta & delta), 2) distribution patterns of ictal EEG onset (diffuse, lateralized & regionalized), 3) electrode of maximum amplitude at initial regionalized portion of ictal EEG, and 4) mesial to lateral and anterior to posterior amplitude ratio of ictal discharges in temporal electrodes. RESULTS: One hundred seven seizures came from MTLE (23 patients) and twenty-two seizures from NTLE (4 patients). Ictal EEG onset with delta wave was observed more frequently in MTLE (42.7%) than in NTLE (14.3%), but beta wave onset was noted more frequently in NTLE (14.3%) than MTLE (0%). Initial ictal EEG discharges were lateralized on ipsilateral hemisphere in 22.0% of MTLE and in 28.6% of NTLE, regionalized over ipsilateral temporal region in 36.6% and 23.8% respectively, and showed diffuse nonlateralized onset in 41.5% of MTLE and 47.6% of NTLE. Both TLE groups revealed most often their maximum negativity of ictal EEG onset at sphenoidal electrode (MTLE: 59.3%, NTLE: 42.8%). Mesial to lateral amplitude ratio of ictal EEG was significantly larger in MTLE group than NTLE group. The amplitude ratio of anterior to posterior temporal electrodes was greater in NTLE group than in MTLE group. CONCLUSION: Delta frequency of ictal onset EEG is more suggestive of MTLE while beta frequency may suggest NTLE. Initial ictal EEG patterns with their maximum on sphenoidal electrode do not differentiate MTLE from NTLE. NTLE has wide vertical and narrow horizontal electrical field, but MTLE has wide horizontal and narrow vertical electrical field.

10.CT and MR Findings of a Chordoid Meningioma: A Case Report.

A 25-year-old woman with focal seizure, intermittent morning headache and vomiting for 2 years showed microcytic hypochromic anemia on peripheral blood smear and a 6x7.5cm sized intracranial mass with cystic and solid portions at the right temporoparietal convexity on brain CT and MRI which was hypervascular on cerebral angiography. Histopathologic findings on light microscopy suggested chordoma, but it was confirmed as a chordold meningioma by immunohistochemical study. The present case suggests that the diagnosis of chordold meningioma shoud be considered in a juvenile or young adult who is presented with an extra-axial mass with typical location of meningiomas, findings of chordomas on light microscopy, and clinical findings of Castleman syndrome.
