1.A Case of Twin Pregnancy with Ine Fetal Demise(Turner's Syndrome).

Chromosomal abnormalities are higher in twin gestations than in the singleton population. Turner's syndrome(gonadal dysgenesis) variety may result from chromosome loss during gametogenesis in either parent or a mitotic error during one of the early cleavage divisions of the fertilized zygote. The vast majority of 45, XO conceptions result in first or second-trimester miscarriage. Fetuses with Tumer's syndrome commonly exhibit posterior nuchal cystic hygromas and generalized edema. Recently we experienced one fetal demise in twin pregnancy. The affected fetus was associated with Turner's syndrome which was diagnosed by amniocentesis and karyotyping. The fetus was associated with cystic hygroma which was antenatally diagnosed by ultrasonogram. The unaffected fetus had normal karyotype and was delivered through cesarean section without any abnormalities. we report this case with brief review of literatures.

2.The Usefulness of Fetal Fibronectin as a Diagnostic Method of Preterm Ruptere of Membrane.

OBJECTIVE: The purpose of this study was to evaluate the fetal fibronectin as a diagnostic method of preterm rupture of membanes and its clinical usefulness compared with standard tests. STUDY DESIGN: Seventy-eight women who had recieved antenatal care and complained of fluid leakage prior to 37 weeks were enrolled in this study. Standard tests-vaginal peculum examination, Nitrazine test, transabdominal sonography-were perfomed. Rupture of membranes was diagnosed if any two of the standard tests were positive. Fetal fibronectin in posterior vaginal fornix was determined qualitatively by ROM kit (Adeza Biochemical, Sunnyvale, USA). RESULTS: The sensitivity of fetal fibronectin for prediction of rupture of membranes in the women who complained of fluid leakage was 90.6% and its specificity, positive and negative predictive value were 65.7%, 76.4% and 85.1% respectively. 12 women were not diagnosed as rupture of membrane on standard tests but were positive for fetal fironectin. CONCLUSION: Fetal fibronectin would be a useful diagnotic method for rupture of membranes, especially in the women who complained of fluid leakage but not diagnosed by standard tests.

3.A case of vulvar adenocarcinoma associated with extramammary Paget's disease.

Extramammary Paget's disease of the vulva is a rare intraepithelial neoplasm with a high recurrence rate. Extramammary Paget's disease of the vulva is rarely associated with an underlying vulvar adenocarcinoma, but underlying malignant tumor must be evaluated. We experienced a case of vulvar adenocarcinoma associated with Paget's disease and present with a brief review of literature.

4.Cyclooxygenase-2 Expression in Ovarian Tumors.

OBJECTIVE: A large body of evidence suggests that cyclooxygenase-2 (COX-2) is important in tumorigenesis of gastrointestinal and other cancers. This was the study to determine whether COX-2 was also expressed in benign and malignant ovarian tumors. METHODS: We studied the expression of COX-2 in various ovarian tumors [ Benign epithelial tumors (n=10), borderline epithelial tumors (n=12), malignant epithelial tumor (n=12), nonepithelial tumors(n=10) ] by immunohistochemistry. Results of immunoreactivity was classified semiquantitatively based on the proportion and intensity of tumor cell immunostaining. RESULTS: In the epithelial ovarian tumors, the intensity of immunostaining was stronger in the malignant tumors than the benign and borderline tumors (p<0.001). Especially all serous and mucinous malignant tumors (n=9) showed weak and strong immunoreactivity, with 66.7% strong reactivity. None of the nonepithelial ovarian tumors expressed COX-2 immunoreactivity. CONCLUSION: These preliminary data indicate that COX-2 may have a role in carcinogenesis of epithelial ovarian tumors, especially in the serous and mucinous types. COX-2 maybe a target for future research in the tumorigenesis of the epithelial ovarian malignancies.

In colon, smooth muscle tumors are extremely rare and represent only 3% of digestive tract leiomyomas. A 51-year- old women was admitted with complaints of epigastric pain and weight loss. Colonoscopy revealed a large pedunculated polyp covered with almost normal mucosa in the proximal transverse colon. After clipping of the stalk, we performed polypectomy successfully by colonoscopic snare electrocoagualtion. We report a case of endoscopic removal of colonic pedunculated leiomyoma with a review of literature.

6.Expression of Phosphorylated ERK1/2 and cFos Proteins in Vestibular Nuclei by Selective Stimulation of Horizontal Semicircular Canal.

BACKGROUND: There is a little information about the effect of selective vestibular stimulation on the expression of activity-dependent metabolic markers in the vestibular nuclei. The purpose of this study was to evaluate effect of afferent excitation of the horizontal semicircular canal on expression of phosphorylated ERK1/2 (pERK1/2) and cFos proteins in the vestibular nuclei. METHODS: The horizontal semicircular canal of Sprague-Dawley rats was selectively stimulated by using the sinusoidal horizontal stimulator with 10-minute duration of stimulation. Conventional immunohistochemical method was used to visualize pERK1/2 or cFos immunoreactive neurons in the vestibular nuclei following rotation. RESULTS: Five minutes after stimulation of the horizontal semicircular canal there was a high expression of pERK1/2 protein in the medial vestibular nucleus among 4 major subnuclei of the central vestibular nuclear complex. On the contrary, immunoreactivity of cFos protein was observed in the medial and inferior vestibular nucleus 2 hours after horizontal sinusoidal rotation. The lateral vestibular nucleus was free from the expression of pERK1/2 and cFos proteins in response to excitation of the horizontal semicircular canal. However, in the vestibular nuclei of unilaterally labyrinthectomized rats expression of pERK and cFos proteins was markedly suppressed in ipsi-lesional side as well as contra-lesional side following stimulation of the horizontal semicircular canal. Furthermore no expression of pERK1/2 and cFos protein in the bilateral vestibular nuclei of bilaterally labyrinthectomized rats was noted after stimulation of the horizontal semicircular canal. CONCLUSIONS: Therefore these results of present study suggest that excitatory afferent signals from the peripheral vestibular receptors are essential for protein translation for pERK1/2 and cFos in response to stimulation of the semicircular canal.

The cytoplasmic polyadenylation element (CPE)-binding protein (CPEB) binds to CPE containing mRNAs on their 3' untranslated regions (3'UTRs). This RNA binding protein comes out many important tasks, especially in learning and memory, by modifying the translational efficiency of target mRNAs via poly (A) tailing. Overexpressed CPEB has been reported to induce the formation of stress granules (SGs), a sort of RNA granule in mammalian cell lines. RNA granule is considered to be a potentially important factor in learning and memory. However, there is no study about RNA granule in Aplysia. To examine whether an Aplysia CPEB, ApCPEB1, forms RNA granules, we overexpressed ApCPEB1-EGFP in Aplysia sensory neurons. Consistent with the localization of mammalian CPEB, overexpressed ApCPEB1 formed granular structures, and was colocalized with RNAs and another RNA binding protein, ApCPEB, showing that ApCPEB1 positive granules are RNA-protein complexes. In addition, ApCPEB1 has a high turnover rate in RNA granules which were mobile structures. Thus, our results indicate that overexpressed ApCPEB1 is incorporated into RNA granule which is a dynamic structure in Aplysia sensory neuron. We propose that ApCPEB1 granule might modulate translation, as other RNA granules do, and furthermore, influence memory.
