1.Statistical analysis of the prevalence and trend of obesity in elementary school children in Kwangju.

To estimate the prevalence and trend of obesity in primary school-age children, obesity survey was performend in children aged 6 to 12 years(5,226 boys and 5,048 girls)in Kwangju in 1991. Obesity was defined as overweight if more than 20 per cent above the median weight for height and age(relative weight>120%). The following results were obtained: 1) The prevalence of obesity that exceeded the weight centiles at each height by more than 97th percentile was 8.78. Boys (9.70%)was not significantly higher than in girls(7.82%)(p>0.05). 2) The prevalence of obesity that exceeded the standard weight for height by more than 20% was 8.82%. Among them, 5.45% were mild obesity and 2.93% were moderate obesity and 0.44% were severe obesity. The prevalence rate of obesity was no significant differences by sex(p>0.05). 3) Regionally, the prevalence of obesity was a lower rate in Kwangiu in 1991 (8.82%)than that in Seoul in 1989(11.68%).

2.Relation between lymphocyte subpopulations of peripheral blood and immune responses of modified live hog cholera virus vaccine in pigs treated with an ionized alkali mineral complex.

Thirty-nine healthy pigs (28-32 days old) were purchased from a commercial swine farm and housed at swine pens of the College. The animals were vaccinated intramuscularly (1 ml) with an attenuated live hog cholera virus (HCV, LOM strain) and then boostered at 5 weeks after the first vaccination. The animals were divided into 4 experimental groups: 0.05% (w/w) PowerFeel-supplemented diet (T-1, n = 10); 3% (w/w) SuperFeed-supplemented diet (T-2, n = 10); diluted PowerFeel solution (1 : 500, v/v) as drinking water (T-3, n=9); control (n=10). PowerFeel is an original form of ionized alkali mineral complex (IAMC) and SuperFeed is a commercial product of IAMC. The subpopulation of lymphocyte in blood was assayed by a flow cytometry and HCV-specific antibody was determined by an indirect immunofluorescence assay. In IMAC-treated groups, the proportions of subpopulation expressing MHC-class II, CD2+, CD4+, CD8+, and surface IgM+ B lymphocytes were significantly decreased at 5-weeks after the first vaccination. Significant decreases were also observed in the proportions of MHC-class II, CD2+ and CD8+ lymphocyte at 3-weeks after the booster injection. The humoral immune responses in T-1 and T-2 groups were greater than those in T-3 or control group. These results suggest that IAMC-supplemented diets may have an HCV-specific immunostimulatory effect in pigs.

3.Percutaneous Balloon Mitral Vavuloplasty under General Anesthesia in Patient with Mitral Stenosis and Schizophrenia.

Since the introduction of percutaneous mitral valvotomy(PMV) in 1984, PMV was an effecive alternative to surgical commissurotomy in selected patients with severe mitral stenosis. Also PMV was an excellent palliative strategy in such patients who were high risk for operative management or longterm anticoagulation was not feasible. We performed PMV in a patient with very tight mitral stenosis and severe pulmonary hypertension under the general anesthesia because the patient was anticipated to be uncooperative due to mental problem, who was diagnosed as schizophrenia 16 years ago. Robinol was used for premedication and i.v fentanyl was used for maintenance of anesthesia. Inoue balloon was introduced into the LV and gradual ballooning was performed with favorable results. Total anesthesia time and interval from internal jugular vein puncture to the completion of valvuloplasty were 1 hour 45 min and 40 minutes respectively. Hemodynamic variables were improved immediately after intervention and mitral valve area was increased from 0.5cm2 to 1.3cm2.

7.Surgical Treatment for Trigeminal Neuralgia

Various treatments for trigeminal neuralgia (TN) are known to yield initial satisfactory results; however, the surgical treatment has excellent long-term outcomes and a low recurrence rate. Surgical treatment addresses the challenge of vascular compression, which accounts for 85% of the causes of TN. As for surgical treatment for TN, microvascular decompression (MVD) has become the surgical treatment of choice after Peter J. Jannetta reported the results of MVD surgery in 1996. Since then, many studies have reported a success rate of over 90% for the initial surgical treatment. Most MVDs aim to separate (decompress) the culprit vessel from the trigeminal nerve. To increase the success rate of surgery, accurate indications for MVD and management of the offender vessels without complications are critical. In addition, if there is no vascular compression, partial sensory rhizotomy or internal neurolysis can be performed to improve surgical outcomes.
8.A case of carbamazepine-induced toxic epidermal necrolysis.

Drug-induced toxic epidermal necrolysis (TEN) is a bullous erythematous disease that is characterized by the appearance of scaled lesions and large sheets of pilling on the skin. A caseof TEN occured in a 10 month old female patient. The characteristic skin lesions of TEN developed after oral administration of carbamazepine for a partial seizure. Thig case consists of prodrome of malaise, fever, anorexia, and conjunctivity followed by erythema & flaccid bullae formation. Diagnosis was confirmed by histologic findings. This patient was treated with a massive systemic corticosteroid, antibiotics, fluid and electrolytes, and topical measures. Authors experienced an extremely rare case of TEN dur to carbamazepine, So report it with a brief review of literature.

10.Clinical Experience of Adrenal Tumors Treated by Surgical Management.

Our experience of surgical management with 21 adrenal tumors between September 1980 and February 1993 is presented. The patients comprised 17 functioning adrenal tumors (5 primary aldosteronisms, 4 Cushing's syndromes, 6 pheochromocytomas and 2 neuroblastomas) and 4 nonfunctioning adrenal tumors (1 adrenal cyst, 1 adrenal cortical carcinoma, 1 adrenal adenoma and 1 metastatic adrenal carcinoma). The 21 patients in these series included 3 men and 18 women. Age was ranged from 11 years to 68 years at the time of presentation (average 35.8 years). Nine tumors occurred in the left adrenal gland, eleven in right and one in bilateral. Surgical approaches to the adrenal gland were transperitoneal in 11 cases and extraperitoneal in 10 cases. Among 17 functioning adrenal tumors, 13 cases underwent unilateral adrenalectomy, and 3 cases underwent unilateral adrenalectomy with nephrectomy. One case of pheochromocytoma underwent bilateral adrenalectomy. Among 4 non-functioning adrenal tumors, one case underwent unilateral adrenalectomy and 3 cases underwent unilateral adrenalectomy with nephrectomy. Operative complications occurred in 2 cases. One case with pneumothorax and the other case with electrolyte abnormality. The use of advanced radiographic and laboratory procedures results in the refinement of diagnosis and localization of tumors, enabling better surgical management of adrenal tumors. But some of underlying diseases should be differentiated by pathologic confirmation.
