3.A Case of Trichilemmal Carcinoma.

Trichilemmal carcinoma is a rare adnexal tumor originating from outer root sheath of hair follicle. We report a case of trichilemmal carcinoma occurring on the scalp. A 71-year-old women presented with a nodule on the vertex of scalp. Histopathologically, tumor cells showed a lobular proliferation in continuity with the epidermis. The lobules showed peripheral palisading and trichilemmal keratinization. The prominent tumor cells were large cells with PAS-reactive, diastase-sensitive, clear cytoplasm. Striking cytologic atypia and high mitotic index were found. Total excision was done and the patient has been free of recurrence or metastasis for 1 year.

4.An Overview of the Package Insert and Contraindications of Filler Products in Korea.

Filler injection is an extremely popular cosmetic procedure in Korea, but surprisingly few dermatologists are aware of the package inserts in filler products. In the era of legal dispute, it is important that we fully understand the indications, contraindications, and side effects of the individual filler products. The purpose of this paper is to provide a general overview of the contents of package inserts of filler products that are commercially available in Korea, especially in terms of the contraindications of filler injection. The authors emphasize that greater improvement in the Korean package inserts of filler products can be expected. Currently, practicing physicians are usually blamed for filler complications. We also found many significant translational errors. We hope that dermatologists take more interest in filler package inserts by reading this overview.

5.High-Dose Preoperative Radiation and Sphincter-Saving Procedure in Distal Rectal Cancer.

A clinical study was made of 14 cases of distal rectal cancer treated at the Department of Surgery, St. Mary's Hospital, Catholic University Medical College, from January 1995 to April 1996. The patients were treated in a modified Thomas Jefferson program and received high doses of preoperative radiation followed by a sphincter saving procedure. The results are as follows: 1) There were 5 male patients (35.7%) and 9 female patients(64.3%). There was also a large number of patients in their 50s and 60s. 2) The preoperative pathologic type was a moderately differentiated adenocarcinoma. 3) Rectal bleeding was the most common symptom, followed by tenesmus, constipation, anal pain, and a tarry stool. 4) At diagnosis, the Thomas Jefferson (T.J.) system was used for the clinical stage of the patients. The clinical stage of disease showed a preponderance of T.J. stage I (10 patients), T.J stage II, III, and IV occurred in 2, 1, and 1 patient, respectively. 5) The most common site was 0-3 cm above dentate line (8 patients), followed by 4-6 cm (5 patients) and > 7 cm (1 patient) above the dentate line. 6) The complications after preoperative radiation therapy were diarrhea (3 patients) and perianal dermatitis (5 patients). 7) After preoperative radiation therapy, one case showed pathologic complete remission. 8) The procedure was a Transanal Abdominal TransAnal proctosigmoidectomy and coloanal anastomosis (TATA) in 11 patients, a low anterior resection in 1 patient, an abdominal perineal resection in 1 patient, and a colostomy in 1 patient. 9) The pathologic stage showed a preponderance of Duke's B2 and B1; 5 were B2, 4 were B1, 3 were C2, 1 was D, and 1 was O. 10) The postoperative complications after colostomy repair in TATA were frequent defecation, tenesmus, anal pain at defecation, and rectovaginal fistulas which subsided 3 months after colostomy repair.

6.A Case of Intradermal Nevus Combined with Epidermal Cyst.

The combination of an intradermal nevus with epidermal cyst or other adnexal tumors has often been described since 1952. Most of these reports have involved the coexistence of common epidermal cysts and melanocytic nevi. Herein, we report a 21-year-old Korean male, who initially presented to our department with a rapidly enlarging preexisting nevus on his back. The mass was surgically excised and histopathologically showed intradermal nevus combined with epidermal cyst.

7.Thoracocervicofacial Purpura Occurred After Epileptic Seizure.

The thoracocervicofacial pupura is an obstructive pupura, which occurs in the various conditions with elevated pressure in the thoracic cavity. This pupura can occur as an unusual consequence of epileptic seizure. A 28-year-old man visited due to the syncope and the thoracocervicofacial pupura. His creatine kinase level was elevated. Therefore the cutaneous manifestation is thought to be due to epileptic seizure. Here we report a case of thoracocervicofacial pupura as the unique manifestation of epileptic seizure.

8.Effect of Lovastatin(Mevacor(R)) on Serum Lipids of Patients with Primary Hyperlipidemia.

A new hypolipidemic agent, lovastatin, hydroxy-methyl-gultaryl coenzyme A reductase inhibitor was administred to 25 patients with primary hyperlipidemia 20 to 40 mg daily for 12 weeks and sequential changes of serum lipid profile were analysed as follow. 1) Mean average at baseline period of serum total cholesterol, triglyceride, HDL and low desity lipoprotein cholesterol were 271, 179, 51 and 185 mg/dl respectively. 2) Total cholesterol showed 20% decrease at 4th week and 23% decrease at the end of 12th week while low density lipoprotein cholesterol decreased 31% and 33% respectively. 3) Triglyceride dropped 7% at 8th week and 3% at 12th week. High density lipoprotein cholesterol increased 4% at 4th week and showed 2% decrease at the end of study. 4) Only one patient complained of moderate abdominal pain, which subsided after 2 weeks drug withdrawal. In conclusion, lovastatin was well tolerated and effective, in the treatment of primary hyperlipidemia.

9.A Sporadic Case of Punctate Palmoplantar Keratoderma.

Punctate palmoplantar keratoderma(PPK) is a rare disease, characterized by small, hard hyperkeratotic papules which are irregularly distributed on the palms and soles. PPK is an autosomal dominant disease with variable penetrance. We report a case of sporadic case of punctate palmoplantar keratoderma in a 26-year-old female patient.

10.The Korean Version of Skindex-29.

BACKGROUND: Most available dermatologic quality-of-life measures were written in English. So these must be translated for use in Korea. Our purpose was to translate and adapt culturally into Korean a skin-related quality-of-life measure Skindex-29, and to begin preliminary assessments of its reliability and validity. METHODS: Cross-cultural adaptation questionnaire studies were performed. Two hundred and sixty adult outpatients and healthy people responded to the Korean version of Skindex-29. Evaluations of the semantic equivalence of back-translated items, reliability, construct validity, and content validity of the Korean version were the main outcome measures. Comparison between the Korean version and other versions of Skindex-29 (Spanish and American) responses was also performed. RESULTS: Five problematic items required a second translation and back-translation to achieve satisfactory agreement with the original instrument. The final Korean version of Skindex-29 was internally reliable. The instrument demonstrated both construct and content validity. As hypothesized, scores for dermatologic patients were higher than those of healthy persons and scores for patients with inflammatory diseases were higher than those of persons with isolated skin lesions. Skindex scale scores of the Korean version and other versions of Skindex-29 were similar. CONCLUSIONS: We have developed a semantically equivalent translation of Skindex-29 in Korean. Our preliminary evaluation of its measurement properties suggests that it is a reliable and valid measure of the effects of skin disease on the quality of life in Korean patients.
