2.A case report of orthodontic treatment of Angle's Class III malocclusion by rapid midpalatal expansion.

The patient, 18-year-old girl, had a class III malocclusion with the lateral compression of the maxilla due to the premature loss of the canines, mild mandibular prognathism, and mouth breathing habit. The treatment plan consisted of 1) rapid maxillary expansion 2) a period of retention 3) extraction of the first mandibular premolars instead of surgical correction 4) completion of orthodontic treatment with multibanded system. The maxilla was separated in the midline by the application of orthopedic forces via a cemented rapid maxillary expansion device. After 18 months, She gained functional overbite-overjet relationship, good interdigitation of buccal segments, and facial harmony due to the retraction of lower anterior teeth.

4.Limb Lengthening by Gradual Elongation Intramedullary Nail (Albizzia ).

Leg length discrepancy or short stature is a significant problem to patient psychosocially, cosmetically, and there has been many efforts for limb lengthening. There are many report about lower limb lengthening since the first description of femoral lengthening by Codivilla in 1905. Although limb lengthening using external fixator was popularized by Anderson, the result was poor because of many difficulties in techniques and complications. Recently, a great progress in lower limb lengthening was made by distraction osteogenesis by llizarov and callotasis by De Bastiani. But as such exter- nal fixator has some problem in technique, cosmesis or complications. Therefore gradual elongation intrameduilary nailing(Albizzia) developed by Guichet in 1986 has gained attention for more stability and relatively less discomfort. We reviewed 18 cases in 11 patients(5 males and 6 females) who visited the department of Orthopedic Surgery of Severance Hospital at Yonsei University with limb leg length discrepancy due to sequeale of poliomyelitis, familial short stature and Turner syndrome. They all underwent lower limb lengthening using Albizzia technique between December 1995 and January l997. The average age at the time of the operation was 22.7 year. Famiiial short stature was in 12 cases(67%), Turner syndrome in 2 cases(11%), and leg length discrepancy due to the sequelae of poliomyelitis in 4 cases(22%). The site of lengthening were 5 cases of femur(28%) and l3 cases of tibia(72%). The latency period was average of 7 days. During the distraction period, 15 ratchetings per day(1 mm/day) were performed. In case of bilaterai femoral lengthenings, average length of gain(LG) was 6.0cm and percentage of increase(PI) l6.8%(16.2-17.4), lengthening index(LI) 1.2 month/cm(0.75-2.2). In case of unilateral femoral lengthening, LG was 3.8cm, Pl 7.8 %, Ll 1.3 month/ cm. In case of bilateral tibial lengthening, average LG was 5.5cm(3.5-6.0~) and Pl 18.8%(12.0-22.9), LI 1.3month/cm(0.67-2.3). In case of unilateral tibial lengthening, LG was 2.9cm(2.3-3.5), P1 9.4 %(7.7-11.7), LI 2.8 month/cm(2.2-3.1). Lengthening index in poliomyelitis by llizarov was 3.l month/cm, but it was 1.3 in femur and 2.8 in tibia hy Albizzia method. There are some advantages and disadvantages or contraindications and indications of the Albizzia technique. We suggest that the Albizzia technique is more stahle, more comfortable and no longer lengthening time compared to external fixator such as Ilizarov.

6.Screening of Antiviral Activities of Korean Medicinal Herbs and Traditional Prescriptions Against Herpes Simplex Virus Type-1.

In order to search for anti-Herpes simplex virus (HSV) type-1 agents from Korean medicinal herbs and Korean traditional prescriptions (herb complexes), we selected 80 medicinal herbs and 45 prescriptions, based on a review of the Korean traditional medicine books. Both methanol extracts and boiling-water extracts were tested by means of the MTT assay (tetrazolium based colorimetric assay). Ten of the 125 methanol extracts: CM-11, CM-18, CM-19, CM-21, CM-22, CM-39, MM-3, MM-18, MM-29, MM-73 (see explanation of nomenclature below), showed efficacy against HSV-1. Twelve of the water extracts: CW-2, CW-3-I, CW-3-II, CW-18, MW-3, MW-5 MW-6, MW-12, MW-47, MW-69, MW-73 and MW-75 were active. #3 (individual herb) and #73 (individual herb) were interesting because both water and methanol extracts were active. Especially, #3 is a part of composition of Hong-il-laksamd ngbang and Hojanghaedokt'ang which have anti-HSV-1 activitives. The SI value of MW-69 and CW-18 was relative high as 10.2+/-0.7 and 11.8+/-2.2. The cytotoxic effect on Vero cells of Panch' n-ch' ngbang, Taraxacum platycarpum H. Dahlst. and acycloguanosine was determined by MTT assay. Water extracts of Panch' ngch' ngbang (prescription) and Taraxacum platycarpum H. Dahlst. showed very weak cytotoxic effects on Vero cells at > 100 mug/ml but acycloguanosine showed strong cytotoxic effects on Vero cells at > 100 mu g/ml. As a result, #3, #73, MW-69 and CW-18 are considered as potentially useful for anti-HSV-1 agent and will be the focus of further research. 7.A roentgenographic study on apical root resorption of human permanent teeth.

The purpose of this study was to investigate the resorption potential in human permanent teeth by analyzing the frequency and the degree of root resorption as revealed by routine intraoral roentgenograms. The following conclusions were obtained: 1) The distribution of root resorptions was as follows; a) 65.3 per cent were no root resorption. b) 7.0 per cent showed questionable root shortening. c) 25.2 per cent were definite root shortening. d) 2.5 per cent showed very severe resorption. 2) The order of susceptibility of teeth to resorption in this study was consistent with other studies. 3) The maxillary teeth showed a much higher incidence (52.2 per cent) than the mandibular teeth (17.1 per cent) 4) The sex ratio of the sample in this study was 1.3 female to 1 male.

8.A Clinical Study on Collateral Ligament Injuries of the Knee

A clinical analysis on the 33 cases of the collateral ligament injuries of the knee joint was made. Those cases were classified in three groups depend upon the width of the joint space by means of stress-radiogram as one plue (less than 5 mm), two plus(5~10 mm), and three plus(more than 10 mm). Twenty one cases were three plus group and were treated operatively and remaining had conservative treatment. This injury is more prevalent in male with the peak in male with the peak incidence of 21~30 years old age group. The most common site of injury of medial collateral ligament is the mid-portion of the ligament where cross the joint line by the 42.3% on operated cases. The lateral collateral ligament has no specific location of injury but more even on its whole length of femoral or fibullar attachment and mid-portion of the ligament. Results were evaluated in three groups of excellent, good and poor. Six out of 12 cases of conservative treatment and nine out of 21 cases of operative group were excellent in result without joint instability, pain and muscle weakness on walking and flexion was available more than 120 degress. Four cases of conservative treatment group and ten of operative group were good in result with occasional pain on walking, mild degree of instability and range of joint motion between 100 to 120 degrees. Four cases had poor result with pain on walking, marked joint instability and less than 100 degrees of joint motion. Overall result of more than good were 90.5% in operative group and 83.3% in conservative treatment group.

9.Cyst-like Destructive Lesions of Calcaneus

Calcaneus is very important in weight bearing and maintaining the normal contour of the foot. Many diseases can cause the cyst-like destructive changes of the calcaneus. In most cases, histological confirmation and bacteriological examination are necessary for the final diagnosis. In recent years, four cases of the cyst-like destructive lesions of the calcaneus were treated and had satisfactory results. They were confirmed with biopsy and bacterial culture as followings; acute osteomyelitis, chronic osteomyelitis, tuberculous osteomyelitis and simple bone cyst. Operations on the osteomyelitis were curettage with or without sequestrectomy and the simple bone cyst the curettage and bone graft. After operations, proper antibiotics and plaster immobilization followed routinely.

10.The Statistical Study of Bone Tumors

A statistical analysis was made on 59 patients of bone tumors who were treated at medical center from March, 1971 to February, 1978. The results were as follows. 1. Thirty nine cases(66%) were benign and twenty cases (34%) were malignant 2. Osteochondroma was the most common benign bone tumor (38.5%) and followed by simple bone cyst(17.9%), enchondroma(10.3%), osteoid osteoma(10.3%) and giant cell tumor(7.7%). 3. The knee joint area was the most common site of osteochondroma(95%) and its peak incidence was in the second decade. 4. Two of three cases of giant cell tumor were recurred at six months and three years after surgical intervention. 5. 11 cases(55%) out of 20 malignant bone tumor were primary tumors of osteogenic sarcoma(9 cases), Ewing's sarcoma(1 case) and multiple myeloma(1 case). Rest of 9 cases(45%) were metastatic bone tumors. 6. The primary organs of the metastatic bone tumor were kidney(2 cases), thyroid and lung(each 1 case), but the 5 cases were undetectable. The most common site of metastasis were spine and pelvis.
