2.Clinical Significance of MRI for Assessment of Bony Bridge of Epiphyseal Plate: A Case Report

It is essential to define the accurate location and extent of bony bridge in relation to the epiphyseal plate for operation of partial epiphyseal plate closure by bony bridge resection. Since magnetic resonance imaging(MRI) provides excellent tissue contrast and multiplaner image, it could be applied to the diagnosis and mapping of bony bridge of epiphyseal plate. A thirteen year old female with post-traumatic epiphyseal injury of ankle was diagnosed by MRI. We could assess the bony bridge accurately by direct sagittal and coronal images and excellent tissue contrast of MRI.

3.Brachial Plexus Injury

During the period from January 1980 to September 1988, 59 patients suffering from brachial plexus injury, have admitted to Seoul National University Hospital. Fifty-four patients were male and 5 patients were female, and age ranged from 6 to 58 years (mean:26.8 years). The most common cause of injury was traffic accident accounting for 66% (39patient), and among them 24 patients(62%) had the motor cycle accident. Birth injury, stab or gun shot wound, fall down and industrial hazard were following causes. Fracture and dislocation of involved limb accompanied in 19 patients(32%) and 2 patients of arterial injury were also found. EMG and cervicsl myelography with or without CT were performed in almost all the patients and MRI was performed in selected patients. Forty-five patients(76%) had supra-clavicular lesion and 14 patients (24%) had infra-clavicular lesion. In supra-clavicular lesion, upper roots or whole roots were involved in most of cases. Forty-nine cases were treated with conservative treatment for more than 8 months and operative procedures were performed in 36 cases. In operative cases, 30 patients were followed up for more than 1 year (average 24.3 months). Signs of root avulsion were found in 25 out of 31 cases of cervical myelography. MRI was performed in 7 cases, but it was inconclusive in diagnosis of root involvement. Infra-clavicular lesion showed better prognosis than supra-clavicular lesion. In supra-clavicular lesion, upper root involvement had the best prognosis. A pseudocyst detected by myelography usually precluded recovery in the root at the level of pseudocyst. Result of reparative treatment was better than that of conservative treatment. Overall satisfactory result in reparative treatment was 36%. However, if we include neurological improvement, positive result was 53%. Complete recovery was gained in half cases of neurolysis and other half showed no recovery, but all the cases of neurotization showed poor result. Reconstructive surgery including arthrodesis or muscle transfer can be effective treatment method, if it is performed properly on a selective patient.

4.The Effect of Ultraviolet lrradiation on Susceptibility of Listeria monocytogenes lnfection and the Production of Cytokines.

Although the effect of ultraviolet (UV) irradiation on immune response was reported to supress cellular immune response, the exact mechanism was not elucidated. As recent development in cytokine research progress, it is well-known that immune response is regulated by cytokines and especially cellular immune response is induced by interferon (IFN)-r and interleukin (IL)-12 which is mainly produced from lymphocytes and macrophages respectively. Therefore our purpose was to elucidate the UV effect on cellular immune response and its mechanism. We have investigated the changes of host resistance by injection of Listeria monocytogenes which is an intracellular parasite after UVB irradiation in C57BL/6 mice which is known to have relatively strong cellular immune response. In addition we also have investigated the changes in the production of IFN-r from lymphocytes and the production of tumor necrosis factor (TNF)-a and IL-12 from macrophages in mice by UVB irradiation. The increase of mouse spleen index and susceptibility of iisteria monocytogenes infection was correlated with the decreased production of IFN-r, TNF-a and IL-12, which was known to induce the suppression of cellular imrnune response.

5.The Effect of Monocular recession and Resection in Exodeviations: Comparison between Primary, Right, Left, Up, Down and Near Gaze.

In order to evaluate the incidence of lateral incomitance after monocular recession and resection and compare the amount of correction achieved by monocular recession and resection according to the gaze direction and distance in exodeviations, we measured the angle of deviation in primary, right, left, up, down and near gaze before and after monocular recession and resection. Lateral incomitance was developed in11.4%(9/79) after monocular recession and resection. The amount of correction of the deviation toward the side of operated eye was larger than that of the deviation in primary position(p=0.002), but the amount of correction of the deviation toward the side of non-operated eye, up, down and near gaze was all smaller than that of the deviation in primary position(all, p<0.01).

6.A Case of Gastric Tuberculosis.

Gastric tuberculosis is a rare disease and the diagnoeis of gastric tuberculosis is based on either positive histological or bacteriological study. We experienced a case of gastric tuberculosis with pleural effuaion in 59 year-old woman, diagnosed by the histological study of the endoscopic biopsy specimen. Follow up endoscopic finding revealed marked improvement of gastric lesion after anti-tubercadoua medication. We reported the case with review of literature.

7.Development of External Linkage Type of Spine Motion Analyer and Thoracic & Lumbar Motion Analysis in Normal Subjects.

STUDY DESIGN: External linkage type of spine motion analyzer and special programs for calculation of the relative angular motion and graphical display were developed and used to measure relative thoracic and lumbar motion patterns in healthy subjects. OBJECTIVES: To examine the dynamic relationship of thoracic and lumbar spines during various trunk motions and to measure main and coupling motions in three-dimensional spinal kinematics. SUMMARY OF LITERATURE REVIEW: Many kinds of methods, such as inclinometer, stereoradiography, electrogoniometer, cadaveric study etc., were introduced for measuring lumbar spine motion. But, there is no reliable method for measuring accurate dynamic three-dimensional motion patterns of whole thoracic and lumbar spines. MATERIALS AND METHODS: Thirty healthy men, ranging in age from 21 to 29 years, participated in the study. The developed device was tested to measure spine motion patterns in flexion, extension, lateral bending and axial rotation. RESULTS: The average range of motion for flexion was 71.30degrees/16.54degrees(lumbar/thoracic), for exten-zion 25.11degrees/12.67degrees, for one side lateral bending 27.42degrees/31.71degrees, for one side axial rotation 30.86degrees/24.38degrees. There were more coupling motions in lateral bending and axial rotation than in flexion and extension. Coupling motion pattern were found to be relatively consistent among subjects in active axial rotation, and the direction of coupling motion was flexion plus ipsilateral side bending. CONCLUSIONS: It was rather simple and reliable method thor spine motion analysis. There were similarities between motion patterns of lumbar and thoracic spine. But, range of motion in sagittal plane was much greater in lumbar spine than in thoracic

8.Chronotropic Actions of Higenamine in the Isolated Right Atrium of the Rabbit.

Higenamine is known to possess stimulatory activity on beta-receptor of the heart. Chronotropic actions of higenamic were studied in spontaneously beating right atrial muscle isolated from rabbits. The frequency of spontaneous beating and the relative threshold voltage of the right atrium were examined. The relative threshold voltage was defined as the minimal voltage of the given impulse above which the right atrium could be paced at the frequency of 20% of higenamine was also observed. Higenamine caused the postive chronotropic effect. This response became prominent as the ca2+ concentration in the bathing solution lowered. When tetrodotoxin was added to the bathing solution, the effect of higenemine altered and became similar to that of epinephrine. Higenamine reduced the relative threshold voltage of the right atrium in the bathing solution with [ca2+] of 0.5mM. Such effect was abolished by tetrodotoxin. The effects of verapamill on the spontaneous rate and the relativel threshold voltage were inhibited by higenemine. The above results suggest that, aithough the main action og higenamine is on the Ca channel, higenamine also have a minor effect of augmenting the Na channel.

9.Analysis of Biomechanical Properties of Whole Cervical Spine under Static Loading with 3-D Finite Element Model.

The biomechanical responses, such as nucleus pulposus pressure, facet contact force, stress distribution among ligaments in the cervical spine under static loading like flexion and extension, were examined with 3-D nonlinear finite element model. Finite element model consisted of whole cervical spines with intact discs and ligaments was developed from 1mm thick CT-cross sections of a 25-year-old healthy young man. Geometrical nonlinearity was considered for the large deformation and ABAQUS package was used for calculation. Results were verified comparing with those of existing in vivo and in vitro experiments. Results indicated that, developed cervical spine FEM was well consistent with other experiments. Nucleus pulposus pressure increased in flexion and extension more in lower cervical region. Facet contact force increased more in lower cervical spine in extension. In addition, capsular ligament was the most important one among 7 cervical ligaments surrounding cervical spine both in flexion and extension. We could better understand the biomechanics of whole cervical spine with developed finite element model and it might be applied to dynamic or postoperative study.

10.Development of Spine Motion Analyzer and Comparison of Motion in Normal and Lumbar Fusion Cases.

Many kinds of lumbar diseases cause lumbar segmental instability and/or limitation of motion. But, there is no reliable method for measuring accurate trunk motion. In order to measure relative trunk motion respect to the pelvis in vitro, external linkage type of spine motion analyzer was developed. Special programs for calculation of the relative angular motion and graphical display were also developed. The developed device was tested to compare spine motion pattern between 15 normal volunteers and 18 lumbar fusion patients in flexion, extension, lateral bending and axial rotation. It was rather simple and reliable method for spine motion analysis. And it was possible to measure not only angular displacement but also angular velocity and angular acceleration. There were more coupling motions in lateral bending and axial rotation than in flexion and extension. Compared to the normal subjects, patients showed different angular motion, especially in the angular acceleration. Optimal trajectory of the trunk motion derived from mathematical model in flexion and extension matched well with measurement for normal subjects.
