1.The Clinical Study of Transient Synovitis of the Hip Joint in Children

Transient synovitis of the hip joint in children was first described by Lovett and More in 1892and is the commonest cause of painful hip in children. It is characterized by pain and limp of acute onset, generally shart-lived, unassociated with structural changes in the femoral capital epiphysis demonstrable roentgenographically.. The pathology is of interest, not because it is a disabling condition, but because of the difficulties in differentiating it from more serious disease of the hip such as Legg-Calve-Perthes disease, tuberculosis, osteomyelitis and etc. The 76 cases so diagnosed at the Han-II hospital from 1974 to 1983 were reviewed and reported as follows; 1. There was probably associated with predisposing factors such as infection, and trauma. 2. The diagnostic criteria were clinical symptoms, laboratory and radiological findings. 3. The treatment was bed rest, skin traction on the affected leg, non-weight bearing, antibiotics, sedatives and in a few cases, INH was administrated for the prevention of the tuberculosis. 4. Four hips among them revealed later as Legg-Calve-Perthes disease and tuberculous arthritis of the hip joint.

3.Screening of Antiviral Activities of Korean Medicinal Herbs and Traditional Prescriptions Against Herpes Simplex Virus Type-1.

In order to search for anti-Herpes simplex virus (HSV) type-1 agents from Korean medicinal herbs and Korean traditional prescriptions (herb complexes), we selected 80 medicinal herbs and 45 prescriptions, based on a review of the Korean traditional medicine books. Both methanol extracts and boiling-water extracts were tested by means of the MTT assay (tetrazolium based colorimetric assay). Ten of the 125 methanol extracts: CM-11, CM-18, CM-19, CM-21, CM-22, CM-39, MM-3, MM-18, MM-29, MM-73 (see explanation of nomenclature below), showed efficacy against HSV-1. Twelve of the water extracts: CW-2, CW-3-I, CW-3-II, CW-18, MW-3, MW-5 MW-6, MW-12, MW-47, MW-69, MW-73 and MW-75 were active. #3 (individual herb) and #73 (individual herb) were interesting because both water and methanol extracts were active. Especially, #3 is a part of composition of Hong-il-laksamd ngbang and Hojanghaedokt'ang which have anti-HSV-1 activitives. The SI value of MW-69 and CW-18 was relative high as 10.2+/-0.7 and 11.8+/-2.2. The cytotoxic effect on Vero cells of Panch' n-ch' ngbang, Taraxacum platycarpum H. Dahlst. and acycloguanosine was determined by MTT assay. Water extracts of Panch' ngch' ngbang (prescription) and Taraxacum platycarpum H. Dahlst. showed very weak cytotoxic effects on Vero cells at > 100 mug/ml but acycloguanosine showed strong cytotoxic effects on Vero cells at > 100 mu g/ml. As a result, #3, #73, MW-69 and CW-18 are considered as potentially useful for anti-HSV-1 agent and will be the focus of further research. 5.Characterization and Distribution of Dendritic Cells and Tissue Macrophages in the Rat Ovary.

This study was undertaken to investigate the characterization and distribution patterns of MHC class II positive dendritic cells[DCs] and ED2 positive tissue macrophages throughout the estrous cycle and during pregnancy in the rat ovary. The immunohistochemical characterization of the cells was carried out using the monoclonal antibodies OX6 and ED2 in cryostat-cut sections. DCs were distributed in the theca cell layer of the growing and mature follicles,stroma and corpus luteum. Tissue macrophages were distributed in the theca externa of the growing and mature follicles, stroma and corpus luteum but they were smaller in number than DCs. None of DC and tissue macrophage was found in the ovum, granulosa layer and follicular cavity of the ovarian follicle. However, DCs and tissue macrophages were present in the granulosa layer and follicular cavity in the atretic follicles. Degenerating corpus luteum contained a vast number of OX6 positive cells. On the contrary, fewer tissue macrophages were founcl in the degenerating corpus luteum. More macrophages tended to be observed in the former follicular cavity and theca lutein cell layer than in the granulosa lutein cell layer of the corpus luteum. In stroma,DCs and tissue macrophages were more frequently found around the blood vessels than in the other region, however, DCs were relatively greater in number than tissue macrophages. There was no estrous cycle and pregnancy dependent variation in the numbers and distribution patterns of DCs and tissue macrophages. In conclusion, the rat ovary contains rich networks of MHC class II positive dendritic cells and ED2 positive tissue macrophages. These findings suggest the existence of a well-developed system of immunological surveillance in the rat ovary. The results of this study have potentially important implications for the understanding not only of the ovarian immune system and the pathogenesis of various ovarian diseases but also of various physiologic functions of the ovary.

6.A Case of Giant Hydronephrosis.

A kidney containing more than 1,000ml fluid in its collecting system is generally defined as giant hydronephrosis. The clinical manifestations of giant hydronephrosis are indistinct. The patient seems to seek medical help late due to tolerable symptoms and particularly in the state of poor economic condition. We present a giant hydronephrosis of contents of 60 liters associated with ureteropelvic junction stricture on a 43-year-old woman. The final diagnosis was performed by exploration of abdomen and nephrectomy was done.

9.59 Cases of posterior Urethral Injury.

A clinical evaluation was made on 59 patients with posterior urethral injury treated in the Department of Urology, Chonnam University Hospital, during the period from January 1980 to December 1986. The results were as follows : 1. Of 157 patients with urethral injuries among 2,932 urological in-patients during the period, 59 sustained posterior urethral injury. These age ranged from 6 to 68 years. 2. The most common cause of posterior urethral injury was traffic accident(59.3%) which is followed by industrial accident(27.1%), falling down(10.2%), iatrogenic(1.7%) and inflammatory(1.7%) in decreasing order. 3. The managements were transperineal end-to-end anastomosis in 28 patients, primary urethral realignment with interlocking sounds in 12, indwelling of Foley catheter in 8, pull-through procedure in 3, cystostomy only in 4, transpubic urethroplasty in 3 and scrotal skin inlay urethroplasty in 1. 4. Of the 28 cases treated with delayed transperineal end-to end anastomosis, postoperative urethral stricture developed in 12(42.9%), impotence in 2(7.1%), incontinence in 2(7.1%) and urethrocutaneous fistula in 1(3.6%). 5. Of the 12 cases treated with primary urethral realignment, urethral stricture developed in 7(58.3%), impotence in 1(8.3%), incontinence in 1(8.3%) and epididymitis in 1(8.3%).
