2.A Clinical Study on Free Flaps in Young Children

The treatment of extensive soft tissue injury is a chsllenge to Orthopedidic Surgeon especially in young children and at present reconstructive surgery with free flap is being employed under microscopy actively. The author present 27 cases of free flaps in young children below 6 years old, which were performed from March 1983 to December 1986. The results are as followes ; 1. The mean age was 4.9 years old. 2. The mechanism of injuries were all pedestrian injury. 3. We performed 27 cases of free flaps, among them 24 cases of flap were survived(88.9 %) with good functional and cosmetic result. 4. The vessels were larger in these young children than we expected. It is apparent that in a healthy child even though below 6 years old, the careful application of microsurgical principle is an excellent method for salvaging devascularized parts that are vital for function or appearance.

4.Therapeutic Efficacy of Topical 5-Fluorouracil for Zosteriform Porokeratosis.

The patient, 23 year old female, with the Zosteriform Porokeratosis on right side of trunk and extrimity and a superimposed Disseminated Superficial Actinic Porokeratosis on face, was treated at Dept. of Dermatology, Catholic Medical College. The lesions appeared at the age of 7. Her mother had similar lesions on the face and extrimities. 3% 5-Fluorouracil in Vanishing cream base was applied topically under occlusive dressing for 40 days, and the dressing was changed every 24 hours. On seventh day erythema and papular lesion appeared, and oral administration of steroid and intermittent topical betamethasone dressing was enable to continued the occlusive dressing. One year after the treatment the lesion did not recurred and shallow atropic scar were left with normal skin color.

5.Two Cases of Hyperlippoproteinemia (Type II).

Two cases of hyperlipoproteinemia is reported, Hyperlipoproteinemia is characterized by various kinds of cutaneous xanthomatosis associated with or without arteriosclerotic changes due to increasing lipoprotein fraction of the serum. Case I . 37-year old housewife had multiple, pea to nut sized, various shaped, cutaneous and subcutaneous nodular masses has been developed last 3 years. Some of her family member have similar symptoms. Serum cholesterol level was 790 mg% and serum triglyceride level was 270 mg% after 14 hour fasting. Case II. 52-year old housewife was suffered from pea sized, multiple, subcutaneous nodular and cutaneous eruptions last 2 years. There was no family histories. Serum cholesterol level was 895 mg% and serum triglyceride level was 350 mg% after 14 hour fasting. Typical Touton giant cells were found in biopsy specimens taken from cutaneous lesons. ECG, X-ray, glucose toleranc test and other laboratory examinations showed within normal limits. Tne agarose electrophoresis pattern of serum lipoprotein fractions are very similiar with that of hyperlipoproteinemia type II. These two patients were treated with nicotinic and 1. 0 gm and clofibrate 2.0 gm daily by mouth and restricted animal fatty foods. Total serum cholesterol level was markedly decreasea 2 months after treatment in case I.

6.Two Cases of Malignant Melanoma Diagnosed by MRI.

A 10 month-old female visited our out-patient clinic, whose chief complaint was an enlargement of right eyeball since 5 month ago. Her physical examination disclosed Cafe-au-lait spots on abdomen and back and a tumor-like mass was seen on right vitreous cavity by slit lamp biomicroscopy, indirect ophthalmoscopy, and orbit CT. The working diagnosis was either neurofibromatosis or retinoblastoma, but choroidal hemangioma was also another possibility tomographically. However, MRI suggested the possibility of choroidal malignant melanoma unexpectedly. Subsequently the eye was enucleated and the tumor was confirmed to be a choroidal malignant melanoma. We report this interesting case and the other case of choroidal malignant melanoma diagnosed by MRI in 57 year-old male.

7.A Clinical Study on Trochanteric Fractures of the Femur

The increased incidence of the trochanteric fracture of the femur was accompanied by the development of the vehicle and the prolongation of the average life-span. Recently it has been also increased in an active person from thirties to forties. 87 cases of the trochanteric fracture of the femur in 86 patients who have been admitted and treated at the Department of Orthopedic Surgery of Hanyang University Hospital during 8 years and 10 months, from May, 1972 to February, 1981, were reviewed. The results of this study were as follows: 1. The trochanteric fracture of the femur was more common in male and was frequent in a person who was from thirties to forties and who had vigorous social activity. 2. An injury from traffic accident was the most common cause of the fracture from thirties to forties and an injury from slip down was most common in seventies. 3. The unstable fracture which was classified by Evans' classification, Boyd and Griffin Type II Fx. and Tronzo Type III Fx. were most common in the intertrochanteric fracture and the Fielding Type II Fx. was most common in the subtrochanteric fracture of the femur. 4. Other injuries were accompanied by the trochanteric fracture of the femur in 38 patients (44.2%) and the pelvic bone fracture was the most common associated injury in these patients. 5. The weight bearing was allowed earlier in the patient who had been given the surgical Tx. than in the patient who had been given the conservative Tx. 6. In adults, there was no significant differences in the duration to achieve the bony union between the conservative treatment and surgical treatment performed. 7. We considered that the diminution of the probable complications by early weight bearing after accurate open reduction in accordance with the Type of the Fx. followed by secure internal fixation with devices is an ideal method of the Tx.

8.Screening of Antiviral Activities of Korean Medicinal Herbs and Traditional Prescriptions Against Herpes Simplex Virus Type-1.

In order to search for anti-Herpes simplex virus (HSV) type-1 agents from Korean medicinal herbs and Korean traditional prescriptions (herb complexes), we selected 80 medicinal herbs and 45 prescriptions, based on a review of the Korean traditional medicine books. Both methanol extracts and boiling-water extracts were tested by means of the MTT assay (tetrazolium based colorimetric assay). Ten of the 125 methanol extracts: CM-11, CM-18, CM-19, CM-21, CM-22, CM-39, MM-3, MM-18, MM-29, MM-73 (see explanation of nomenclature below), showed efficacy against HSV-1. Twelve of the water extracts: CW-2, CW-3-I, CW-3-II, CW-18, MW-3, MW-5 MW-6, MW-12, MW-47, MW-69, MW-73 and MW-75 were active. #3 (individual herb) and #73 (individual herb) were interesting because both water and methanol extracts were active. Especially, #3 is a part of composition of Hong-il-laksamd ngbang and Hojanghaedokt'ang which have anti-HSV-1 activitives. The SI value of MW-69 and CW-18 was relative high as 10.2+/-0.7 and 11.8+/-2.2. The cytotoxic effect on Vero cells of Panch' n-ch' ngbang, Taraxacum platycarpum H. Dahlst. and acycloguanosine was determined by MTT assay. Water extracts of Panch' ngch' ngbang (prescription) and Taraxacum platycarpum H. Dahlst. showed very weak cytotoxic effects on Vero cells at > 100 mug/ml but acycloguanosine showed strong cytotoxic effects on Vero cells at > 100 mu g/ml. As a result, #3, #73, MW-69 and CW-18 are considered as potentially useful for anti-HSV-1 agent and will be the focus of further research. 9.A Case of Giant Hydronephrosis.

A kidney containing more than 1,000ml fluid in its collecting system is generally defined as giant hydronephrosis. The clinical manifestations of giant hydronephrosis are indistinct. The patient seems to seek medical help late due to tolerable symptoms and particularly in the state of poor economic condition. We present a giant hydronephrosis of contents of 60 liters associated with ureteropelvic junction stricture on a 43-year-old woman. The final diagnosis was performed by exploration of abdomen and nephrectomy was done.

10.Partial Purification and Characterization of 41 kDa Serine Proteinase from Culture Filtrate of Trichophyton tonsurans.

Dermatophytes infect the human hair, skin, nail and cause the dermatophytosis. The extracellular and intracellular proteinases of the dermatophytes commonly occur in the genus Trichophyton like T. rubrum, T. mentagrophytes, and T. granulosum. These enzymes play a prominent role in growth, multiplication and infection of the host tissue. Extracellular proteinases have been purified from the species of Trichophyton and Microsporum. We purified the proteinase partially from the culture filtrate of the Trichophyton tonsurans through Mono-Q and Superose 12 column and investigated its biochemical and enzymatic characters. The molecular size of the proteinase was estimated to be 41 kDa by SDS-PAGE. And pI was 3.2. The optimal temperature and pH for an enzymatic activity were 27C and 7.5, respectively. The purified porteinase degraded the keratin, bovine serum albumin, hemoglobin. The serine proteinase inhibitor like PMSF and DFP inhibited the proteolytic activity of the purified enzyme whereas the cysteinase inhibitor did not. These results demonstrated that the purified proteinase is a serine proteinase and can contribute the tissue invasion.
