3.Experimental Study on the Destruction and Recovery of the Blood Retinal Barrier after Photocoagulation and Cryotherapy in the Rabbits.

The primary purpose of the study is to investigate the destruction and recovery of the blood retinal barrier after photocoagulation and cryotherapy in the rabbits. The 110 pigmented rabbits were used in this experiment. After photocagulation and cryotherapy we injected intravenousely a dose of 25 mg/kg of fluorescein sodium, and then sampled blood and vitreous and measured the concentration of fluorescein sodium by means of High Performance Liquid Chromatography. The calculated penetration ratio which indicates the destruction of the blood retinal barrier is obtained by dividing fluorescein sodium concentration of vitreous by integral of fluorescein sodium concentration of the plasma from 3 min to 60 min. Subsequently, fundus photography and enucleation for flat preparation were performed in each experimental rabbit. The fluorescein concentration of vitreous of the normal rabbit is 3.60 +/- 4.75 X 10(-9)gm/ml and its penetration ratio is 0.20 +/- 0.23 X 10(-6) min. After measuring the correlation between the frequency of photocoagulation and penetration ratio and between the frequency of cryotherapy and penetration ratio, we found out that the correlation coefficient was 0.885 and 0.909 respectively. And this experiment showed that penetration ratio was higher in cryotherapy group than in photocoagulation group. In addition, we divided these experimental animals into 4 groups, mild and severe cryotherapy groups and lighter and heaver photocoagulation groups, and assessed the penetration ratio of these four groups at 1,3,7,14,28, and 42 days after treatment. As a result, penetration ratio was highest at 3 days after treatment and was almost back to normal by 42 days in all experimental groups except in severe cryotherapy group. Compared with photocagulation groups, cryotherapy groups showed more extensive destruction and delayed recovery of the blood retinal barrier. In fundus photography, in the photocoagulation groups white patch was developed after treatment, at 7 days white patches disappeared and were replaced by pigmented and scarred tissue; by 42 days the margin of these lesions was indistinct and the lesions were changed into relatively small scarred patches. On the other hand, in the cryotherapy groups the thick round white patch wasdeveloped after treament, at 7 days large round white patches disappeared and were replaced by pigmented and scarred tissue; by 42 days the lesions were replaced by large scarred patches with pigmentation and depigmentation. In flat preparation, in the photocoagulation groups central necrotic zone and intermediate zone with hyperpigmentation and peripheral zone with hypopigmentation was presented at 1 day. In the cryotherapy groups the diminished density, in the center and white ring at the margin was revealed at 1 day. From 7 days in photocogulation groups and from 14 days in the cryotherapy groups, retinal pigment epithelium began to show proliferation and by 42 days, retinal pigment epithelial layer of both groups except severe cryotherapy group was replaced by relatively normal retinal pigment epithelium.

4.Surgical Treatment of the Upshoot and Downshoot in Duane's Rtraction Syndrome.

The upshoot or downshoot that occurs when an eye is adducted in Duane's retraction syndrome is believed to be related to a taut lateral rectus muscle. When an eye is adducted, the lateral rectus muscle is slipped over the globe, producing this abnormal eye movement. Seven patients with Duane's retraction syndrome demonstrated this findings and underwent an Y-splitting operation on the lateral rectus muscle. Y-splitting and recession of the lateral rectus muscle is safe and simple procedure. All 7 patients demonstrated markedly decrease in the upshoot or downshoot after surgery.

5.A case of pemphigus erythematosus.

We present here, a case of pemphigus erythematosus developed in a 31-year-old woman, who had been suffering from repeated episode, of remission and exacerbation with several erythematous, and somewhat oozing patche accompanying a mild pruritus or her face, upper part of the back and anterior chest. In serological studies, she had antinuclear antiboc@lies at. a titer of 1:160. Histopathological finding showed subcorneal bullae. Immunofluorescent study revealed IgG and C, deposition in the intercellular space of the epidermis and along the de rmoepidermal junction.

6.Evaluation in Various Aspect in Strabismus Patient.

The author studied the 240 cases of strabismus patients in our OPD from 1972 to 1976. And the author observed the results of occlusion therapy in 55 cases of amblyopia with strabismus. The results are as follows: The sex distribution in exotropia patients showed a preponderance in female sex, the ratio male to female being 2:3. Concerning the refractive status, there were more hyperopia in esotropia and more myopia in exotropia. The amount of deviation was about the same at the near and distance, most patients had a basic type and distributed between 21-40 prism. Of all patients in whom a determination of the retina correspondence could be made, 70% showed NBC and 25 % showed ARC 5% showed variable. Ninety percent of the strabismic amblyopia patients was successfully treated by occlusion therapy. A positive correlation was not found between the duration of occlusion and the age at which occlusion was started. Amblyopes showed arrest of improvement after a certain period, about 3 to 4 months. The exotropia patients over the age of 7 showed good results by occlusion. The shorter the time between the age at which occlusion was started and the onset of strabismus, the better was the result. Strabismic amblyopia responded well to conventional occIusion.

7.Transient Cerebellar Mutism after Total Removal of Medulloblastoma in a Child: Case Report.

Transient mutism resolving to cerebellar speech after posterior fossa surgery is a well recognized phenomenon, particularly in pediatric patients. The anatomic basis for this postoperative functional change is unclear, but may reside in the dominant superior cerebellar hemisphere or the medial deep cerebellar nuclei. We report a case of a 9-year-old boy who presented for surgical resection of a medulloblastoma. Preoperatively, his complaint consisted of headache, nausea, vomiting and cerebellar ataxia. He had normal speech. At one day after operation, suddenly he was unable to speech, however, communication through a variety of verbal cues, including sign language was possible. His mutism lasted 12 days and cerebellar dysarthria was slowly resolved.

8.Tension Pneumocephalus after Transsphenoidal Surgery for a Giant Pituitary Tumor: Case Report.

Tension pneumocephalus is a rare complication of craniotomy, however, it should be managed promptly due to rapid neurological deterioration. We report a case of tension pneumocephalus after transsphenoidal surgery for a giant pituitary tumor. It may have developed because of the cerebrospinal fluid rhinorrhea and presence of an external lumbar drain.

9.Intrahepatic Duct Dilatation Caused by Extrahepatic Billiary Obstruction' Morphologic Differentiation Between Benign and Malignant Disease on CT Scan.

PURPOSE: Retrospective studies were carried out to evaluate the usufulness of CT in differentiating benign or malignant intrahepatic duct(IHD) dilatation due to extrahepatic duct(EHD) obstruction. MATERIALS AND METHODS: Among 75 cases of IHD dilatation, 35 cases(47%) which did not show causative lesion on CT were classified into central or peripheral type according to the extent of the dilatation and into pruned or beaded form according to its pattern. We considered central type or pruned form as a benign feature while peripheral type or beaded from as a malignant feature. Predictions of benignancy or malignancy were made according to the pattern and the extent of IHD dilatation. In all 75 cases, maximum caliber of IHD at portal vein bifurcation were examined. RESULTS: In regard to the classification of the extent, 80% (28 out of the 35 cases:11 of 15 cases of benignancy and 17 of 20 cases of malignancy) of our prediction was correct. As to the classification of the pattern, similar results(82% correct prediction) were obtained(14 out of 17 cases):ln 15 cases of benignancy, we observed six cases of pruning and 1 case of beading. Among the 20 cases of malignancy, two cases of pruning and eight cases of beading of IHD dilatation were observed. Mean diameters of IHD at portal vein bifurcation were 10.1ram in benignancy and 15.1mm in malignancy.(Diameters less than 11mm was suggestive of benignancy(alpha =0.05). CONCLUSION: Evaluation of the extent, the pattern, and the degree of IHD dilatation is necessary in differential diagnosis of benignancy and malignancy of EHD obstruction. We conclude that CT is a useful tool for this purpose of differential diagnosis of the benignancy and the malignancy of extrahepatic biliary obstruction.

10.Diagnostic Value for Early Detection of Prostate Cancer of the Digital Rectal Examination, Serum Prostate Antigen and Transrectal Ultrasonography.

PURPOSE: There diagnostic modalities have been commonly used to detect prostate cancer to date: digital rectal examination (DRE), serum prostate specific antigen (PSA) and transrectal ultrasonography(TRUS). We evaluated the diagnostic values of these three modalities in the early detection of prostate cancer. MATERIALS AND METHODS: We analysed 215 patients with low urinary tract symptoms who had pathologic diagnosis for prostatic cancer was possible by biopsy or transurethral resection. Transectal ultrasonography guided sextant biopsies were performed if the PSA level was greater than 4.0 ng/ml or DRE was suspicious, even if TRUS revealed no ateas suspicious for cancer. TURP were performed without TRUS or biopsies if the PSA level was lesser than 4.0 ng/ml and DRE was negative. RESULTS: Of the 215 patients, 36 (16.7%) revealed prostate cancer. Positive predictive value, sensitivity and specificity was 37.0%. 83.3%. 71.5% for DRE, 30.0%, 91.7%, 58.3% for PSA and 27.6% 58.3%. 43.3% for TRUS respectively. Positive findings on serum PSA or DRE or both tests detected significantly more tumors(97.2%, 35of 36 cancers) than only PSA (91.7%, 33 for 36 cancers), DRE (83.3%, 30 of 36 cancers) and TRUS (60.0%, 21 of 35 cancers) alone. CONCLUSIONS: These result shows that DRE is more valuable and cost effective method for detection of prostate cancer than serum PSA and the use of DRE in conjunction with serum PSA enhances prostate cancer detection.
