2.The Expressions of Matrix Metalloproteinase-9 and Tissue Inhibitors of Metalloproteinase-1 in Scar Endometriosis and Full Term Endometrium.

OBJECTIVE: Endometriosis in a surgical scar following cesarian section is rare condition. And its pathogenesis has not been well established. The purpose of this study is to observe the expressions of matrix metalloproteinase-9 (MMP-9) and tissue inhibitors of metalloproteinase-1 (TIMP-1) in scar endometriosis and full term endometrium during cesarian section. METHODS: Nineteen scar endometriotic tissue and ten full term pregnant endometrium during cesarian section were investigated by immunohistochemical stain for MMP-9 and TIMP-1. RESULTS: The MMP-9 was expressed in eleven of nineteen scar endometriotic tissue (57.9%) and eight of ten full term pregnant endometrium (80.0%). The TIMP-1 was not expressed in both scar endometriotic tissue and full term pregnant endometrium. CONCLUSION: These results suggest that the relationship between MMP-9 and TIMP-1 might play a role of pathogenesis of scar endometriosis.

4.Biomechanical evaluation of dental implants with different surfaces: Removal torque and resonance frequency analysis in rabbits.

STATEMENT OF PROBLEM: Macroscopic and especially microscopic properties of implant surfaces play a major role in the osseous healing of dental implants. Dental implants with modified surfaces have shown stronger osseointegration than implants which are only turned (machined). Advanced surface modification techniques such as anodic oxidation and Ca-P application have been developed to achieve faster and stronger bonding between the host bone and the implant. PURPOSE: The purpose of this study was to investigate the effect of surface treatment of titanium dental implant on implant stability after insertion using the rabbit tibia model. MATERIAL AND METHODS: Three test groups were prepared: sandblasted, large-grit and acid-etched (SLA) implants, anodic oxidized implants, and anodized implants with Ca-P immersion. The turned implants served as control. Twenty rabbits received 80 implants in the tibia. Resonance frequencies were measured at the time of implant insertion, 2 weeks and 4 weeks of healing. Removal torque values (RTV) were measured 2 and 4 weeks after insertion. RESULTS: The implant stability quotient (ISQ) values of implants for resonance frequency analysis (RFA) increased significantly (P < .05) during 2 weeks of healing period although there were no significant differences among the test and control groups (P > .05). The test and control implants also showed significantly higher ISQ values during 4 weeks of healing period (P < .05). No significant differences, however, were found among all the groups. All the groups showed no significant differences in ISQ values between 2 and 4 weeks after implant insertion (P > .05). The SLA, anodized and Ca-P immersed implants showed higher RTVs at 2 and 4 weeks of healing than the machined one (P < .05). However, there was no significant difference among the experimental groups. CONCLUSION: The surface-modified implants appear to provide superior implant stability to the turned one. Under the limitation of this study, however, we suggest that neither anodic oxidation nor Ca-P immersion techniques have any advantage over the conventional SLA technique with respect to implant stability.

6.Relationship between Serum Lipids and Psychiatric Symptoms.

OBJECT: Many studies found that psychological stress produced significant increases in cholesterol concentration. But recent study results suggested that lowering cholesterol increased non-illness mortality. Suicide rate were higher in low cholesterol groups. Also, depression and violence were related to low cholesterol. It is the purpose of this study to determine which psychiatric symptoms are correlated with serum concentration of cholesterol, triglyceride and HDL-C. METHOD: The subjects we included were 61 neurotic outpatients. Their psychiatric symptoms were assessed with SCL-90-R and additional symptoms check list and the results of their fasting serum concentration of cholesterol, triglyceride and HDL-C were obtained. The correlation between serum concentration of lipids and psychiatric symptoms were analyzed using Pearson's correlation test. RESULTS: The result showed significant positive correlation between somatization symptom dimension and serum cholesterol concentration. Serum cholesterol concentration were also negatively correlated with suppressed emotion of anger Serum concentration of triglyceride and HDL-C were not correlated with any symptom dimensions of SCL-90-R. CONCLUSIONS: These results suggest that somatization symptoms have some relationship with serum cholesterol and the role of cholesterol in psychiatric symptoms need to be included in future study.

8.Influence of the central benzodiazepinergic system on peripheral cardiovascular regulation.

Diazepam is known to have cardiovascular depressive effects through a combined action on benzodiazepinergic receptor and the GABA receptor-chloride ion channel complex. Moreover, it is known that barbiturates also have some cardiovascular regulatory effects mediated by the central GABAergic system. Therefore, this study was undertaken to delineate the regulatory actions and interactions of these systems by measuring the responses of the cardiovascular system and renal nerve activity to muscimol, diazepam and pentobarbital, administered intracerebroventricularly in rabbits. When muscimol (0.03~-0.3 microgram/kg), diazepam (10~100 microgram/kg) and pentobarbital (1-10 microgram/kg) were injected into the lateral ventricle of the rabbit brain, there were similar dose-dependent decreases in blood pressure (BP) and renal nerve activity (RNA). The relative potency of the three drugs in decreasing BP and RNA was muscimol > pentobarbital >diazepam. Muscimol and pentobarbital also decreased the heart rate in a dose-dependent manner; however, diazepam produced a trivial, dose-independent decrease in heart rate. Diazepam (30 microgram/kg) pentobarbital (3 microgram/kg) did not. Bicuculline (0.5 microgram/kg), a GABAergic receptor blocker, significantly augmented the effect of muscimol (0.1 microgram/kg) in decreasing blood pressure and renal nerve activity, but of pentobarbital in decreasing BP and RNA, either alone or with muscimol. We inferred that the central benzodiazepinergic and barbiturate systems help regulate peripheral cardiovascular function by modulating the GABAergic system, which adjusts the output of the vasomotor center and hence controls peripheral sympathetic tone. Benzodiazepines more readily modulate the GABAergic system than barbiturates.

Squamous cell carcinoma of the ovary is very rare malignant tumor originating from the malignant transfomation of mature cystic teratoma, usually being reported in about 1-3% of cases. Of malignant transformation, squamous cell carcinoma is the most common type and its prognosis is poor. Because of its rarity, the clinicopathologic characteristics are not well established. We have experienced a case of squamous cell carcinoma arising in mature cystic teratoma of ovary and present it with brief review of the literature.

10.Predictive Values of Gated Myocardial SPECT for Wall Motion Improvement After Bypass Surgery.

We studied to investigate the predictive values of gated SPECT for the improvement of wall motion after bypass surgery. As we compared postoperative SPECT with preoperative ones, we defined viability as wall motion improvement. We performed rest 71-201/stress Tc-99m-MIBI gated SPECT in 25 patients before and 3 months after bypass surgery. Myocardial wall motion was graded as normal, hypokinesia, akinesia, and dyskinesia by pair-wise visual analysis of gated pre and postoperative SPECT's on the same monitor screen. Myocardial wall thickening was determined good or poor Among 92 segments with wall motion abnormalities before operation, 69 (75%) improved and 23 did not. Before operation, we could find segments with good systolic thickening in 64 segments among total 92. Thickening of the remaining 28 was poor. Wall motion improved postoperatively in 45 segments (70%) among 64 with good thickening. Twenty four(86%) among 28 segments with poor thickening had also improved. We grouped segments into mild(hypokinetic) and severe(akinetic/dyskinetic) ones. Among 33 segments with severe motion abnormalities, 14 had good thickening and 19 did not. Nine(60%) improved out of 14 segments having severe abnormality with good thickening. However, 16(84%) segments out of 19 having severe abnormality with poor thickening also improved. Neither degree of perfusion decrease nor severity of wall motion abnormalities could explain the high rate of false negatives. In conclusion, as we defined viability as wall motion improvement by comparing pre and postoperative SPECT, systolic thickening observed by gated Tc-99m-MIBI SPECT in myocardial segments with wall motion abnormalities predicted wall motion improvement after bypass surgery. However, poor thickening could not be referred as evidence of nonviable myocardium both in mild and severe contractile dysfunction, so that we might need stimulation study such as dobutamine echocardiography or dobutamine gated SPECT.
