1.Impact of Group Dynamic Imagery Therapy on Job Stress and Psychosocial Stress of Female Nurses.

PURPOSE: Purpose of this study was to examine the impact of group dynamic imagery therapy on job stress and psychosocial stress of female nurses. METHODS: The research questions were tested through a quasi-experimental research with a non-equivalent pretest-posttest design. Seventy-seven female nurses from G University Hospital in J City participated in this study; 38 of them were assigned to the experimental group, and the rest were assigned to the control group. Once a week for 8 continuous weeks, participants in the experimental group had 90-minute group dynamic imagery therapies. RESULTS: 1) The level of job stress was significantly lower in the experimental group than the control group, and 2) The level of Psychosocial stress was also significantly lower in the experimental group than the control group. CONCLUSION: The results of the study indicate that group dynamic imagery therapy can be an effective intervention against job stress and psychosocial stress of female nurses.

2.A Pattern on the Male LUTS Related with Chronic Kidney Disease.

PURPOSE: We examined an occurrence of the lower urinary tract symptoms (LUTS) within the chronic kidney disease (CKD) comparatively analyzed implications with components of the CKD. MATERIALS AND METHODS: We surveyed eighty-two CKD patients who were over than 50-year-old, micturated upward of 1,000cc a day. The check list concerned with the LUTS consists of International Prostate Symptom Score (IPSS), quality of life, volume of prostate, maximal flow rate and residual urine volume and we evaluated GFR, serum creatinine, prevalence period, causative disease and diabetes. We analyzed correlations between the CKD and components of the LUTS and examined an aspect of LUTS concerned with the degree of CKD, prevalence period and diabetes. A control group composed of twenty-eight male adults who visited to the general medical examination center and they were examined by the same checklist. RESULTS: The group of CKD is significantly worse than the control group over the whole items of LUTS beside the volume of prostate; they were on the average 57.3+/-3.8 years old, 17.1+/-2.3 of IPSS, 3.8+/-1.2 of quality of life, 12.6+/-3.2ml/sec of maximal flow rate and 38.9+/-4.3ml of residual urine, 25.9+/-3.4g of prostate in the group of CKD. Furthermore, a group of patients who had suffered from the CKD for over than 3 years and combined with diabetes is significantly worse than control group. CONCLUSIONS: The study revealed that a number of CKD have LUTS and they seem to be influenced by prevalence period, stage and causative disease of CKD rather than the volume of prostate.

3.Effects of Hospital Ethical Climate and Communication Self-Efficacy on Nursing Cares Left Undone among Nurses

Purpose:
This study aimed to identify the effects of hospital ethical climate and communication self-efficacy on nursing care left undone.
Methods:
The participants were 142 nurses working in a general hospital. Data were collected from July 18 to August 30, 2021. Data were analyzed by t-test, ANOVA, Pearson correlation, and multiple regression analysis using the SPSS/WIN 25.0 program.
Results:
(a) The mean value of hospital ethical climate was 3.54±0.41, communication self-efficacy was 5.03±0.81, and the sum of nursing cares left undone was 3.68±3.14.(b) Nursing cares left undone had a negative correlation with hospital ethical climate(r=-.25, p=.003) but not with communication self-efficacy (r=-.13,p=.116). (c) Factors that affected nursing cares left undone included education (≥master) (β=.23, p=.005), marital status (single) (β=-.19, p=.018), age (26~27) (β=-.18, p=.022), and hospital ethical climate (β=-.18, p=.029); the explanation power was 18.0% (F=8.66, p<.001).
Conclusion
Our study shows that hospital ethical climate plays a significant role in nursing cares left undone of nurses. It is important to strengthen hospital ethical climate to lower the incidence of nursing cares left undone. These results may serve as basic data to help develop strategies for reducing the incidence of nursing cares left undone.
4.Long Term Outcome of Endoscopically Clipping the Upper Part of R4 Sympathetic Block and R4 Sympathetic Block for the Treatment of Palmar Hyperhidrosis.

BACKGROUND: Thoracic sympathetic block surgery is a safe and effective procedure for palmar hyperhydrosis, and this maintains sufficient moisture and prevents compensatory hyperhidrosis. To avoid compensatory hyperhidrosis, the authors performed sympathetic block surgery just above the R4 level to maintain sympathetic tone affecting the caudal area. MATERIAL AND METHOD: A total of 71 subjects (45 males and 26 females) were categorized into two groups. Group 1 (31 patients, mean age: 25.5 years) had clips placed both on the upper and lower part of R4 sympathetic ganglion, and group 2 (40 patients, mean age: 25.9 years) underwent clipping of the upper part of R4. Telephone surveys were done to collect data on 8 categories, and the average follow up interval was 24.9 months (group 1) and 18.9 months (group 2). RESULT: For group 1, 41.9% experienced no sweating and 48.4% replied they experienced some sweating depending on the surrounding conditions. Group 2 showed that 60% experienced no sweating and 35% replied they experienced some sweating depending on the surrounding conditions. 58.1% in group 1 experienced sweating right after the surgery, and 40.0% in group 2 experienced the same. Group 1 (38.1%) and group 2 (37.5%) replied they experienced no hand dryness and more patients in group 2 than in group 1 had hand dryness, but without uncomfortable symptoms. 71.0% (group 1) and 62.5% (group 2) replied they had no compensatory hyperhidrosis or related symptoms. One patient in group 1 and two in group 2 reported they regretted undergoing the procedure. The regions of compensatory hyperhidrosis were the back, thigh and chest in group 1 and the group 2 reported the back, chest, and abdomen in the order of frequency. Fewer incidences of the gustatory hyperhidrosis were noted in group 2. Most of the patients were satisfied with their treatment. CONCLUSION: Clipping the upper part of the R4 ganglion or R4 sympathetic block are both effective for treating palmar hyperhidrosis and these treatments decrease the occurrence or symptoms of compensatory hyperhidrosis. The upper R4 sympathetic block procedure is easier and safer with fewer incidences of gustatory hyperhidrosis and a higher percentage of patient satisfaction.

5.Comparison of Expression of Endometrial Prolactin in infertile Women with Luteal Phase Defect According to Clomiphene Citrate Administration.

OBJECTIVE Clomiphene citrate is one of the most commonly used drugs in the treatment of infertility, but the pregnancy rate achieved with clomiphene citrate is significantly lower than the ovulation rate due to its antiestrogenic effect on the endometrium. Endometrial prolactin is considered to be a marker and an inducer of predecidualization that is characteristic of secretory endometrium. The purpose of this study was to evaluate the association of clomiphene citrate and unsatisfactory endometrial differentiation to secretory endometrium by examining the endometrial expression of prolactin in clomiphene citrate-treated infertile women with luteal phase defect. METHODS: The endometrial samples from infertIle women wIth luteal phase defect (n=27) were examined. Five cases during secretory phase and six cases during proliferative phase were obtained by biopsy. Sixteen cases were obtained by biopsy during secretory phase after clomiphene citrate treatment. By immunohistochemical staining for prolactin, all obtained endometrial tissues were examined. The differences in the endometrial expression of prolactin were evaluated between proliferative phase and secretory phase, and between clomiphene citrate treated group and no treatment group during secretory phase. RESULTS: The staining of endometrial prolactin was significantly more intense in the glandular epithelial cells and stromal cells in the secretory endometrium than in the proliferative endometrium. The glandular expression of prolactin in the secretory endometrium was not significantly different between the clomiphene citrate-treated group and no treatment group (p=0.719), but the staining of prolactin in the stromal cells was significantly less intense in the clomiphene citrate-treated group than no treatment group (p=0.019). CONCLUSION: in this investigation, we demonstrated that the endometrial stromal expression of prolactin in the secretory phase was significantly lower in the clomiphene citrate-treated group campared with no treatment group in infertile women with luteal phase defect. And our finding suggests that clomiphene citrate may have an adverse effect on the endometrial predecidualization in infertile women.

6.A Case of Cerebellar Venous Angioma Associated with Huge Intracranial Arteriovenous Malformation.

A case of cerebellar venous angioma incidentally associated with huge intracranial arteriovenous malformation is reported. This 50-year-old male patient was admitted presenting symptoms of subrachnoid hemorrhage. Brain CT and cerebral angiography showed huge AVM in the right parieto-occipital area and also right cerebellar venous angioma draining into the straight sinus. No clinical manifestation was correlated to this venous angioma and no specific treatment was made to this lesion. The AVM was removed totally and the patients is now in follow-up evaluation through OPD with slight left hemiparesis.

7.Effect of Oral Administration of Dioctahedral Smectite and Cholestyramine with Phototherapy in the Treatment of Neonatal Hyperbilirubinemia.

PURPOSE: Dioctahedral smectite is an alumina silicate of phyllitic structure and absorbs bile acid in the intestine, forming a non-absorbable complex preventing enterohepatic circulation. The purpose of this study is to clarify the value of dioctahedral smectite and the adequate dosage, in combination with phototherapy, as well as to confirm whether it shortens the duration of hospitalization and to compare dioctahedral smectite with cholestyramine. METHODS: Total 45 full-term neonate with a total bilirubin level greater than 12 mg/dl were studied. The neonate were randomly divided into three groups : 1) Only phototherapy group (A) 2) 3.0 g/day dioctahedral smectite with phototherapy group (B) 3) 1.0 g/kg/day cholestyramine with phototherapy group (C). RESULTS: The mean serum bilirubin level of group B and C decreased significantly compared to group A at 48, 72 and 96 hours after the beginning of the study. The duration of phototherapy and hospitalization significantly decreased in group B and C. CONCLUSION: The data revealed that oral administration of dioctahedral smectite not only increased the efficacy of phototherapy, but also shortened the duration of phototherapy and can substitute for cholestyramine.

Suprasellar meningiomas are uncommon intracranial tumors representing approximately 4-10% of intracranial meningiomas and the total excision is sometimes hazardous because of surrounding vital structures. We have experienced four cases of this lesion during last three years and the masses were approached through frontoparietal craniotomy and lateral subfrontal approach and were removed completely with microsurgical technique. Visual disturbances were improved in three cases and unchanged in one case. The literatures are also reviewed.

Various types CNS lesions bring about precocious puberty and mostly these lesions involve the hypothalamus. We experienced two cases of hypothalamic hamartomas and a case of hypothalamic astrocytoma, presenting sexual precocity as the initial symptom. Other clinical findings included gelastic seizure, behavioral disturbance, accelerated bony growth. MRI of the hamartoma showed high signal intensities on T2 weighted spin echo image. The two hypothalamic hamartomas were partially removed and the patients showed some improvements endocrinologically and clinically one month after the operation. Literatures about the hypothalamic hemartoma were reviewed and the problems especially in the extent of surgical removal in the hypothalamic hamartomas were discussed.

10.Role of Endovascular Treatment in the Management of Intracranial Arteriovenous Malformations: Experience of 15 Cases.

The authors report the results of treatment in 15 patients with intracranial arteriovenous malformations(AVM's) who underwent embolization with n-butyl cyanoacrylate(NBCA) and poly vinyl alcohol(PVA) particle between March, 1990 and August, 1992 at Seoul National University Hospital and discuss about role of endovascular treatment in the management of intracranial AVM's. Seven patients(46.7%) had pre-operative embolization, microsurgery was done in six patients and radiosurgery was done in one patient. In patients with pre-operative embolization, endovascular treatment had helped microsurgery in dissection, diminution of blood transfusion, and shorter time of surgery. AVM's of two patients(12.5%) were totally occluded by embolization alone. But they had morbidity after embolization due to inadvertent occlusion. AVM's of six patients(40%) were partially occluded and recurrence of previous symptoms, bleeding, revascularization, increase of AVM nidus developed during follow up. It is concluded that role of endovascular treatment is in intracranial AVM's adjuvant, definite, palliative. Except surgical resection after embolization, embolization alone should be done very cautiously indefinite and palliative treatment.
