2.The Effect of Intravenous Lidocaine Infusion on Succinylcholine - or Vecuronium - induced Neuromuscular Block in the Cat.

The effect of lidocaine on the neuromuscular junction and the interaction between lidocaine and the muscle relaxant, succinylcholine(SCC) or vecuronium(VEC), were investigated in 12 cats of either sex using the sciatic nerve-anterior tibialis muscle preparation. The degree of twitch depression with lidocaine itself was 16.43+/-14.94%, and the ED from dose-response curves of SCC and VEC(8.35+/-4.62 and 6.36+/-8.03 ug/kg, respectively) in group II (under lidocaine infusion) were smaller than those(20.57+/-15.80 and 8.98+/-7.31 ug/kg) in group I (without lidocaine). There was no difference in the duration of action of SCC and VEC between the two groups, but the onset of VEC in group II was longer than in group I(p<0.05), The mean arterial pressure(MAP) after lidocaine infusion was reduced 149.17 to 130.83mmHg and MAPs at the same dose of each agent were also lower in group II than in group L The authors conclude that lidocaine itself produces the twitch depression and potentiates the SCC-or VEC-induced neuromuscular block. However, there is no influence on the duration of SCC or VEC.

3.Unruptured Aneurysm of the Left Sinus of Valsalva Presenting as Acute Coronary Syndrome: A Case Report.

We report here on a case of a 34-year-old man with unruptured aneurysm of the left sinus of Valsalva, and he presented with acute coronary syndrome due to the putative dynamic compression of both the left main coronary artery and the left circumflex coronary artery. The cardiac multislice computed tomography scanning and coronary angiogram revealed the compression of the two coronary arteries by the aneurysm of the left sinus of Valsalva. Aneurysmectomy was performed for surgical repair. After the surgery, the patient stayed asymptomatic during the 6-months of follow-up.

4.Predictability of Acoustic Pharyngometry for the Obstructive Sleep Apnea.

BACKGROUND AND OBJECTIVES: Acoustic pharyngometry is a relatively new noninvasive method that quantifies the geometrically complex oropharyngeal dimensions. We aimed this study to investigate the predictability and usefulness of acoustic pharyngometry in the diagnosis of obstructive sleep apnea (OSA). SUBJECTS AND METHOD: We conducted a prospective clinical trial in consecutive 49 patients with snoring or sleep apnea. Polysomnography was used to determine whether the patient had OSA. All subjects were examined by acoustic pharyngometry in sitting, supine, and both lateral recumbent positions while awake with tidal breathing. Morphometric oral cavity measurements were obtained and the Kushida index was calculated for each patient. RESULTS: Upper-airway cross-sectional areas measured by acoustic pharyngometry were significantly smaller in patients with OSA than in subjects without OSA. Especially, the oropharyngeal junction area (OPJ) in supine position was the most prominent parameter to discriminate between them. CONCLUSION: Acoustic pharyngometry can be a clinically useful tool for localizing the upper-airway obstruction and for predicting obstructive sleep apnea.

5.Induction of Apoptosis by Genistein in Human Melanoma Cells.

Genistein is a naturally occurring isoflavone that has been identified predominantly in soybean. It has been found that genistein can inhibit the growth of various cancer cell lines. Melanoma continues to increase in incidence in many parts of the world and remains among the top six cancers as a cause of death and morbidity. Understanding and overcoming resistance mechanism(s) of melanoma to apoptosis would therefore facilitate identification of new therapeutic targets and development of new treatments. This study was undertaken to investigate whether genistein induced apoptosis on human melanoma cells (G361). Genistein had a significant dose- and time-dependent inhibitory effect on the viability of G361 cells. The death of cells was further demonstrated to be due to apoptosis characterized by chromatin condensation and apoptotic bodies by hoechst staining, and DNA electrophoresis. p53 levels were not altered by genistein treatment. Genistein treatment induced caspase-3 cleavage and activation. Poly (ADP-ribose)-polymerase (PARP) and DNA fragmentation factor 45 (DFF45), which are caspase-3 substrates, were cleaved during genistein-induced apoptosis. It was found that the caspase-6 substrate lamin A was cleaved, whose cleavage has been reported to be necessary for complete condensation of DNA during apoptosis. The expression level and phosphorylation of focal adhesion kinase (FAK) were reduced by genistein treatment. These results suggest that genistein may constitute a potential antitumor compound against melanoma occurring at oral mucosa and skin.

The spontaneous perforation of sinus of Valsalva without congenital anomaly such as aneurysm and trauma has not been reported. We report a 66-year-old female presenting with ST elevation myocardial infarction due to compressing the proximal part of right coronary artery by hematoma attributed to spontaneous perforation of sinus of Vaslava. The patient was stabilized after surgical primary repair.

Multiple primary colorectal carcinomas are not rare and occur more often than what can be ascribed to chance, but the frequencies on record vary. The tumors are usually classified as synchronous if they are present at the same time. It is often difficult to detect all these multiple lesions preoperatively:a correct diagnosis is frequently obtained only upon examination of the resected specimen. Triple synchronous primary colon cancer is an exceedingly rare disease. Despite of its rarity, multiple colon cancers should be investigated in the patients in whom clinical or laboratory evidence of colon cancer is suspected. Recently, we experienced one case of triple synchronous primary colon cancer diagnosed by colonoscopy before operation and report here with the review of the literature.
