1.Analysis of the Factors Affecting Consumer Acceptance of Accredited Online Health Information.

With the increasing use of the internet and the spread of smartphones, health information seekers obtain considerable information through the internet. As the amount of online health information increases, the need for quality management of health information has been emphasized. The purpose of this study was to investigate the factors affecting the intention of using accredited online health information by applying the extended technology acceptance model (Extended-TAM). An online survey was conducted from September 15, 2016 to October 3, 2016, on 500 men and women aged 19–69 years. The results showed that the greatest factor influencing the acceptance of the accredited health information was perceived usefulness, and the expectation for the quality of the accreditation system was the most important mediator variable. In order to establish the health information accreditation system as a means to provide easy and useful information to the consumers, it is necessary to carry out quality management and promote the system through the continuous monitoring of the accreditation system.

2.Electrophysiologic characteristics of multiple accessory pathways.

BACKGROUND: The purpose of this study was to investigate the clinical and electro physiologic characteristics of patients with multiple accessory pathways. Recently as endocardial mapping has become more and more accurate, multiple accessory pathways which were considered to be rare in the past, have become more commonly reported in patients with Wolff-Parkinson-White syndrome. METHODS: From February 1993 to June 2000, there were a total of 452 patients, who were confirmed to have accessary pathway mediated-tachyarrhythmias diagnosed by electrophysiologic study. Among those, 19 patients had multiple accessory pathways, and were enrolled in this study. RESULTS: Among the 19 patients, thirteen patients were male and six were female, and their mean age was 36.5+/-16.17 years. All patients had 2 accessory pathways. The distribution of the location of the accessory pathways was at the left free wall (71.1%), right free wall (18.4%) and posteroseptal wall (10.5%). The most common combination pattern was the left free wall and left free wall (57.9%) and the most common anatomical areas were the left lateral wall and left posterior wall (36.8%). The success rate of the catheter ablation was 84.2% (16/19). The recurrence rate after the radiofrequency catheter ablation was 31.3% (5/16) and the most common recurrence site was left free wall (60.0%, 3/5). CONCLUSION: These results indicated that the clinical and electrophysiologic characteristics of the multiple accessory pathway patients with WPW syndrome in our study were similar to those of western countries.

3.The Role of CD44 in the Adhesion of Human Osteosarcoma to Hyaluronic Acid.

Osteoblast expresses a sequence of extracellular matrix during differentiation, suggest that multiple adhesion mechanisms regulate osteoblast differentiation and bone development. Hyaluronic acid (HA) is a glycosaminoglycan (GAG) which mainly distribute in cartilage and extracellular matrix (ECM). HA has very high molecular weights and their structures occupy large solvent domains which give solution of high viscosity. During early development and before tissue differentiation, HA can constitute the major structural macromolecule in the ECM, where it can promote both cell proliferation and migration. CD44 is a cell surface receptor for HA. The polymorphic family of integral membrane glycoproteins CD44 is found on a wide variety of cells. CD44's function in the cell membrane is transmembrane signalling between extracellular matrix and acin filament. In present study, HOS was used as a model to examine whether HOS adhere to HA, CD44 and GAGs on cell surface participate in the adhesion to HA and there is difference in CD44 expression at that time. HOS adhered to HA in the manner of time -dependent. After incubation for 180 minutes, about 90% of cells adhered to HA. When HOS was pretreated with anti -CD44 antibody, hyaluronidase and genistein, the adhesion rate was significantly decreased. CD44 was more expressed in HOS plated on HA -coated wells than BSA -coated wells. Taken together, HOS has a adhesive affinity to HA. CD44, GAGs on cell surface and tyrosine kinases play an important role in the adhesion of HOS to HA.

4.Induction of Apoptosis by Genistein in Human Melanoma Cells.

Genistein is a naturally occurring isoflavone that has been identified predominantly in soybean. It has been found that genistein can inhibit the growth of various cancer cell lines. Melanoma continues to increase in incidence in many parts of the world and remains among the top six cancers as a cause of death and morbidity. Understanding and overcoming resistance mechanism(s) of melanoma to apoptosis would therefore facilitate identification of new therapeutic targets and development of new treatments. This study was undertaken to investigate whether genistein induced apoptosis on human melanoma cells (G361). Genistein had a significant dose- and time-dependent inhibitory effect on the viability of G361 cells. The death of cells was further demonstrated to be due to apoptosis characterized by chromatin condensation and apoptotic bodies by hoechst staining, and DNA electrophoresis. p53 levels were not altered by genistein treatment. Genistein treatment induced caspase-3 cleavage and activation. Poly (ADP-ribose)-polymerase (PARP) and DNA fragmentation factor 45 (DFF45), which are caspase-3 substrates, were cleaved during genistein-induced apoptosis. It was found that the caspase-6 substrate lamin A was cleaved, whose cleavage has been reported to be necessary for complete condensation of DNA during apoptosis. The expression level and phosphorylation of focal adhesion kinase (FAK) were reduced by genistein treatment. These results suggest that genistein may constitute a potential antitumor compound against melanoma occurring at oral mucosa and skin.

5.Predictability of Acoustic Pharyngometry for the Obstructive Sleep Apnea.

BACKGROUND AND OBJECTIVES: Acoustic pharyngometry is a relatively new noninvasive method that quantifies the geometrically complex oropharyngeal dimensions. We aimed this study to investigate the predictability and usefulness of acoustic pharyngometry in the diagnosis of obstructive sleep apnea (OSA). SUBJECTS AND METHOD: We conducted a prospective clinical trial in consecutive 49 patients with snoring or sleep apnea. Polysomnography was used to determine whether the patient had OSA. All subjects were examined by acoustic pharyngometry in sitting, supine, and both lateral recumbent positions while awake with tidal breathing. Morphometric oral cavity measurements were obtained and the Kushida index was calculated for each patient. RESULTS: Upper-airway cross-sectional areas measured by acoustic pharyngometry were significantly smaller in patients with OSA than in subjects without OSA. Especially, the oropharyngeal junction area (OPJ) in supine position was the most prominent parameter to discriminate between them. CONCLUSION: Acoustic pharyngometry can be a clinically useful tool for localizing the upper-airway obstruction and for predicting obstructive sleep apnea.

6.Two Cases of Deep Neck Infection with Mediastinal Involvement.

Deep neck space infections affect fascial compartments of the head and neck, and their contents. Acute mediastinitis occasionally occurs as a complication of neck infections resulting in neck sepsis, which spreads to the mediastinum via the cervical fascial planes, and this is best referred to as descending necrotizing mediastinitis (DNM). We recently experienced two cases of deep neck infection dissecting along cervical fascial planes into the mediastinum causing a virulent mediastinitis. Aggressive antibiotic treatment of the deep neck infections along with prompt complete mediastinal drainage are recommended for optimal outcome.

To estimate the incidence rate of coronary heart disease in Korea, of all residents of the Taegu city aged 25 or above, those who had an acute MI or fatal coronary event between 1 July 1996 and 30 June 1997 were registered. Seven hundreds and eight patients were registered during the study period(685 were identified at hospital and 23 were autopsy cases). Age-standardized annual incidence rate of men in city area was 93 per 100,000(95% CI; 61-142) and 33(95% CI; 16-67) in women(100 in men and 20 in women aged 35-64). The incidence was rapidly increased after age 40 in men, however, in women after age 60. Twenty-eight-days case fatality rate was 45% in men and 47% in women. However, in the age group of 45-59 case fatality rate in women was two times higher than that in men. In conclusion, crude annual incidence rate of CHD in city area was 73 per 100,000 in men and 33 in women. The age-standardized annual incidence of CHD in men(93 per 100,000) was 3.2 times higher than that in women (33 per 100,000) in Korea.
