1.The Effects of Long-Term Training of Aerobic Exercise on Some Risk Factors of Cardiovascular Disease.

25 males, aged 40-60 years, who participated in aerobic-type exercise on moderate intensity(60~85% of MHR), 3~5 days a week for 6~12 months were selected. Data were obtained at pre- and post-participation of tranining period for height, weight, girths of chest and abdomen, body fat, BP, HR, serum cholesterol, triglyceride, HDL-cholesterol, glucose, uric acid, BP and HR during graded treadmil exercise, and VO2max. After training for average 38.4 weeks, significant reductions occured in percent of body fat, HR and SBP both at rest and exercise, triglyceride, but not in cholesterol, LDL-cholesterol. And DBP, HDL-cholesterol were elevated. VO2max was increased apparantly by 16.4%. From the results was concluded that training of aerobic exercise could reduce the risk of coronary heart disease by 1) decreasing myocaridal oxygen demand, 2) improving on lipid profile, 3) increasing cardiac work capacity.

2.A Case of Rieger's Syndrome Associated with Controlled Glaucoma.

Rieger's syndrome is a rare autosomal dominant developmental anomaly characterized by ocular and systemic abnomalities. These disorders are typically bilateral and usually diagnosed at birth or in the childhood. The most common ocular defects are hypoplasia of the iris, a prominent Schwalbe's line with iris strands and corectopia. A large number of patients have glaucoma due to devel-opmental defects of the anterior chamber angle structure. The authors experienced a case of Rieger's syndrome which was found in 31-year-old female, who had characteristic ocular and other systemic abnormalities. Associated glaucoma was well controlled by filtering surgery.

3.Analysis of Fractures of the Foot and Ankle

The results of the treatment upon total 132 cases of ankle and foot fractures treated during the period from January 1969 to August 1978 at the Department of Orthopaedic Surgery, National Medical Center have been analysed and reported in this paper. 1. Sex ratio between male and female was 3.7:1. 68 cases (51.5%) cases out of 132 cases were in the age groups of 2nd and 3rd decades. remarkably higher than any other group. 2. 85 cases out of 132 cases were closed fractures and 47 cases were open fractures, but the incidence of the open fracture in higher in the fractures below the metatarsal bones and multiple fractures. 3. 59 cases out of 132 cases were ankle fractures 34 tarsal bone fractures 14 metatarsal bone fractures, 8 phalangeal bone fractures and 20 multiple fractures Accordingly, the incidence of the ankle fracture is higher than other fractures. 4. Ankle fracture was classified in 4 types according to the fracture mechanism (Lauge-Hansen classification). 20 cases out of 59 cases were pronation-abduction type, 15 cases were pronation-external rotation type, 14 cases were supination adduction type and 9 cases were supination-external rotation type. 15 cases out of 59 cases of the ankle fractures were treated by tension band compression internal fixation. The result of this method was much better than other methods. 5. 25 cases out of 35 cases of calcaneal fracture involved subtalar joint according to Rowe classification. 12 cases out of 25 cases of calcaneal fracture involved subtalar joint were Tongue type-C. In the treatment, no relation between restoration tuber angle or Gissane angle and the incidence of complication was obtained. 6. The incidence of the complication, such as secondary oateoarthritis, non union. malunion, occurred higher in the open and multiple fractures than closed and single fractures.

4.Replantation of Severed Thigh: A Case Report

During the last 18 years only a small number of successful replantation of lower extremities severed by trauma have been reported. In this paper we report the first successful case of replantation of completely amputated thigh In Korean literature. A 31-year-old man was admitted to Kyung Hee Unlversity Hospital on February 27th 1976, approximately 1 hour after accident in which the left thigh was completely amputated 15 cm above the knee joint. Circulation was restored 6 hours after amputation through anastomosis of the femoral vein, artery, great saphenous vein, profunda femoris artery and vein. The continuity of sciatic nerve was reestablished by secondary furnicular suture 7 week after injury. About 4 years and 7 months after injury the patient could walk without pain and returned to his occupation. Thre was a almost complete return of motor and sensory function of the posterior tibial nerve.

5.MR Finding of the Pleomorphic Xanthoastrocytoma in a Child: Case Report.

Pleomorphic xanthoastrocytoma(PXA) is a benign brain tumor with favorable prognosis in spite of its marked pleomorphism. We report a case with review of the literature. An eight-year-old boy presented with headache and vomiting. Plain skull X-ray showed findings of increased intracranial pressure. The MR imaging demonstrated superficially located large cystic mass with a Iobulated mural nodule in the left frontal lobe. The mural nodule abutting leptomeninges showed intense enhancement on postcontrast Tl-weighted images, but the cyst wall was not enhanced. Preoperative diagnosis of PXA with the MRI findings could lead to avoidance of more aggressive brain resection and therefore more favorable neurological outcome

6.Congenital Pseudarthrosisof the Tibia: Treated with Free Vascularized Fibular Graft

It is notoriously difficult to obtain a sound bony union of congenital paeudarthrosis of tbe tibia with conventional methods. This paper is the results of using the free vascularized fibular graft for congenital pseudarthorsis of the tibia in 7 patients since 1978 in this hospital, which is the first attempt in Korea. During the follow-up periods from 9 months to 32 months, 5/7 patients(71%) had good or excellent bony union, 2 patients had bone resorption at tbe distal site of grafted bone and required a second supplementary cancellous bone graft with electrode insertion. So it is thought that the free vascularized fibular graft is one of good methods of treatment for congenital pseudarthrosis of the tibia.

7.Peripheral Nerve Injuries of the Upper Extremity

Ninety-one nerve injuries (median; 36, ulnar; 43, radial; 12) involving the upper extremity in 66 patients were studied over a twelve-month period. The results for the median, ulnar and radial nerves after nerve repair were analyzed with respect to the suture method, the age, the level of injury and the time interval from injury. The results were obtained as follows: 1. Sympathetic nerve function returned from 6 weeks (average) after operation. 2. There are better results in perineurial suture than in epineurial suture. 3. There are better results in under 16 years old than over 16 years. 4. There are better results in high level injury than in low level. 5. There are the best results in the delayed primary suture, the next in the primary suture and the last in the secondary suture. 6. Good recovery in motor function was observed in radial, median and ulnar nerve in order.

8.Vascularized Fibula Graft for Restoration of the Large Bone Defect

We analyzed 34 cases of large bone defect caused by trauma, infection and tumor resection from July, 1979 to June, 1985. The bony defect were restored by vascularized fibular graft or vascularized fibular transposition. 22 cases were the traumatic segmental loss and 7 cases were infected ones and 5 cases were the loss following tumor resection. Vascularized fibular graft was performed in 19 cases and vascularized fibular transposition was performed in 15 cases. The 33 cases(97%) of 34 cases had obtained sound bony union and average union time had been 18.6 weeks. Even local infection in the bone, vascularized fibular graft or transposition can be performed. Grafted or transposed fibula had been hypertrophied about 1.2 times in anteroposterior and lateral film which had been measured in the center of the fibula. The most common complication was the clawing of first or second toe. If the ipsilateral fibula is intact, the vascularized fibular transposition is better and if not, vascularized fibular graft is preferred. Operation using the vascularized fibula is a excellent method for the restoration of large bone defect in long bone.

9.The Treatment of Acromioclavicular Separation

Of the shoulder injuries in trauma, none are more common than those involving the acromioclavicular joint. There are many procedures described for treatment of injuries of the acromioclavicular separation but difficulties continues to be encounted. To know the functional anatomy of this joint and mechanism of injuries makes more proper treatment. We operated the 16 cases of acromioclavicular separation in the Dept of Orthopedic Surgery, Kyung Hee College of Medicine from January 1975 to August 1978. The following results were obtained. 1. This injuries is more prevalent in male with peak incidence in the third and fourth decades. 2. The mechanism of injuries were traffic accident, falling down and the other direct injuries with in orders. 3. The difference of distance between coracoid process and clavicle gives the information of Type II and Type III. 4. The operative procedures in Type II and Type III are good treatment of acromioclavicular separation.
