1.Comparative, randomized, double-blind clinical study of alveolar ridge preservation using an extracellular matrix-based dental resorbable membrane in the extraction socket.

PURPOSE: The aim of this study was to radiographically and clinically compare the effect of extracellular matrix (ECM) membranes on dimensional alterations following a ridge preservation procedure. METHODS: One of 2 different ECM membranes was applied during a ridge preservation procedure. A widely used ECM membrane (WEM; Bio-Gide, Geistlich Biomaterials, Wolhusen, Switzerland) was applied in the treatment group and a newly developed ECM membrane (NEM; Lyso-Gide, Oscotec Inc., Seongnam, Korea) was applied in the control group. Cone-beam computed tomography (CBCT) scans and alginate impressions were obtained 1 week and 6 months after the ridge preservation procedure. Results were analyzed using the independent t-test and the nonparametric Mann-Whitney U test. RESULTS: There were no significant differences between the ECM membranes in the changes in the dimension, width, and height of the extraction socket or the quantity of bone tissue. CONCLUSIONS: The NEM showed comparable clinical and radiographic results to the WEM following the ridge preservation procedure.

2.Characteristics of contact and distance osteogenesis around modified implant surfaces in rabbit tibiae.

PURPOSE: Contact and distance osteogenesis occur around all endosseous dental implants. However, the mechanisms underlying these processes have not been fully elucidated. We hypothesized that these processes occur independently of each other. To test this, we used titanium (Ti) tubes to physically separate contact and distance osteogenesis, thus allowing contact osteogenesis to be measured in the absence of possible triggers from distance osteogenesis. METHODS: Sandblasted and acid-etched (SLA) and modified SLA (modSLA) implants were used. Both types had been sandblasted with large grit and then etched with acid. The modSLA implants then underwent additional treatment to increase hydrophilicity. The implants were implanted into rabbit tibiae, and half were implanted within Ti tubes. The bone-to-implant contact (BIC) ratio was calculated for each implant. Immunohistochemical analyses of bone morphogenetic protein (BMP)-2 expression and new bone formation (Masson trichrome stain) were performed. RESULTS: The implants outside of Ti tubes were associated with good bone formation along the implant surface. Implantation within a Ti tube significantly reduced the BIC ratio (P<0.001). Compared with the modSLA implants, the SLA implants were associated with significantly higher BIC ratios, regardless of the presence or absence of Ti tubes (P=0.043). In the absence of Ti tubes, the bone adjacent to the implant had areas of new bone formation that expressed BMP-2 at high levels. CONCLUSIONS: This study disproved the null hypothesis and suggested that contact osteogenesis is initiated by signals from the old bone that undergoes distance osteogenesis after drilling. This signal may be BMP-2.

Dorsal perilunar dislocations are rare traumatic entities. Associated fractures such as carpal bones and radial styloid processes can occur. However, the dorsal perilunar dislocation associated with dislocation of distal radioulnar joint is extremely rare. The authors herein report the case of a 34-year-old man who was presented with transscaphoidal perilunar dislocation which is associated with dislocation of distal radioulnar joint.

5.Intraosseous Ganglion with Pathologic Fracture of the Lunate.

Although intraosseous ganglion of the carpal bones is an uncommon lesion, it can be a cause of chronic wrist pain. Especially, pathologic fracture as complication of idiopathic intraosseous ganglion of the lunate is extremely rare. We report a rare case of idiopathic intraosseous ganglion with pathologic fracture of the lunate that was successfully treated by curettage and autogenous cancellous bone graft.

6.Trapezoid Stress Fracture in a Weight Lifter.

Trapezoid fracture is a rare condition and especially, isolated trapezoid fracture is most rare condition of carpal bone fracture. And principle of treatment has not yet been established. Most of trapezoid fractures can be treated using cast but displaced fracture or fracture combined with other carpal bone fractures may require operative treatment. There have been reports about case of conservative treatment in trapezoid stress fracture but no report about case of operative management. We treated a trapezoid stress fracture in a weight lifting athlete by operative treatment and report the results with literature review.

Transcarpal fractures and dislocations in children are rarely reported in the orthopedics literature. This is a case report of a 10-year-old boy who sustained a trans-scaphoid perilunate dislocation with fractures across the carpal structure: these included injuries to the capitate and triquetrum bones. Treatment consists of a closed reduction for the dislocation and using the dorsal approach, an open reduction with internal fixation of the fractures. The injury healed well with a full return of good wrist function. This unusual pattern of injury is described so that it may be more readily appreciated in the future.

Acromial fractures are well-documented complications subsequent to reverse shoulder arthroplasty (RSA), and most appear as stress fractures with no history of single trauma. To date, no study has reported the occurrence of acute displaced acromial fracture due to sudden strong deltoid contraction during heavy work. Displacement of the fracture results in a challenging surgery since it is difficult to obtain adequate fixation in thin and osteoporotic bones. We report a rare case of acute displaced acromial fracture after successful RSA treatment, using a novel technique of open reduction and internal fixation, applying two 4.5 mm cannulated screws and lateral clavicle precontoured plate.

9.Three-dimensional evaluation of the relationship between dental and basal arch forms in normal occlusion.

OBJECTIVE: The purposes of this study were to evaluate the relationship between the dental and basal arch forms; to analyze their differences in the tapered, ovoid, and square arch forms in normal occlusion by using three-dimensional (3D) virtual models; and to test the hypothesis that the overjet and maxillomandibular basal arch width difference have a significantly positive correlation. METHODS: Seventy-seven normal occlusion plaster casts were examined by 3D scanning. Facial axis (FA) and WALA points were digitized using the Rapidform 2006 software. The dimensions of the dental and basal arches and the overjet were measured. The samples were classified into 3 groups according to arch forms: tapered (n = 20), ovoid (n = 20), and square (n = 37). Analysis of variance (ANOVA) was used to compare the dental and basal arch dimensions. The Pearson correlation coefficients between the intercanine as well as the intermolar widths at the FA and WALA points were calculated. RESULTS: With regard to the basal arch dimensions, the tapered arch form showed a larger mandibular intermolar depth than the ovoid. Strong correlations were noted between the basal and dental intermolar widths in both the upper and lower arches (r = 0.83 and 0.85, respectively). Moderate correlation was found between the upper and lower intercanine widths (r = 0.65 and 0.48, respectively). CONCLUSIONS: The 3 dental arch form groups differed only in some dimensions of the skeletal arch. Moderate correlations were found between the basal and dental intercanine widths. These findings suggest that the basal arch may not be a principle factor in determining the dental arch form.

10.Interaction between Muscle and Bone.

The clinical significance of sarcopenia and osteoporosis has increased with the increase in the population of older people. Sarcopenia is defined by decreased muscle mass and impaired muscle function, which is related to osteoporosis independently and dependently. Numerous lines of clinical evidence suggest that lean body mass is positively related to bone mass, which leads to reduced fracture risk. Genetic, endocrine and mechanical factors affect both muscle and bone simultaneously. Vitamin D, the growth hormone/insulin-like growth factor I axis and testosterone are physiologically and pathologically important as endocrine factors. These findings suggest the presence of interactions between muscle and bone, which might be very important for understanding the physiology and pathophysiology of sarcopenia and osteoporosis. Muscle/bone relationships include two factors: local control of muscle to bone and systemic humoral interactions between muscle and bone. As a putative local inducer of muscle ossification, we found Tmem119, a parathyroid hormone-responsive osteoblast differentiation factor. Moreover, osteoglycin might be one of the muscle-derived humoral bone anabolic factors. This issue may be important for the development of novel drugs and biomarkers for osteoporosis and sarcopenia. Further research will be necessary to clarify the details of the linkage of muscle and bone.
