1.Studies on the parasitic helminths of Korea III. Nematodes and cestodes of rodents.

A survey for nematode and cestode parasites of rodents in Korea has been carried out at the areas of Chulwon, Kumwha, Pochon, Paju and Chungpyong, Kyunggi-Do. A total of 300 rodents of seven species was examined, comprising of 219 A. agrarius, 33 R. norvegicus, 8 R. alexandrinus, 14 Mus musculus yamashinai, Microtus fortis pellceus, 21 Crocidura russula and a Cricetulus trition nester. The following fifteen species belonging to thirteen genera were identified: NEMATODA: Heterakis spumosa, Syphacia obvelata, Nippostrongylus muris, Protospirura muris, Capillaria hepatica, Capillaria sp., Heligmosomum sp., Heligmosomoides sp., Rictularia sp. and Gongylonema sp. CESTODA: Hymenolepis nana, Hymenolepis diminuta, Raillietina coreensis, Paranoplocephala sp. and Cysticercus fasciolaris(the larva of Taenia taeniaeformis).

3.The Clinical Study of the Effective Treatment of Blepharospasm and Hemifacial Spasm with Botulinum toxin A (Oculinum(R))(II).

Two hundred sixty two patients of the essential blepharospasm and hemifacial spasm were treated with Botulinum toxin A (Oculinum(R)). A total of 620 treatments was given injections over a 3-year period. A reduction in spasm intensity was noted in most patients, and the mean response time of the essential blepharospasm and hemifacial spasm were 144.2 days and 172.3 days, respectively, There was no clear relationship between age, sex, toxin dose or preinjected spasm intensity, the amount of spasm reduction, and the mean response time. The mean respone time had no difference from the first through the fourth treatments; but in hemifacial spasm, the mean response time of the second treatment was longer than that of first, third and fourth treatments. The lagophthalmos and superficial punctate keratitis were the most frequent complications. As a result of the injection to avoid the center of the upper and lower eyelids, the frequency of complications could be minimized. There was no clear difference in the beneficial effect and the mcidence of complication (lagophthalmos) between toxin stored in the vaccum and in the non-vaccum state. Patients who were treated with lidocaine mixed toxin had a less effective result than those with saline-mixed toxin.

4.Action of Atropine and Glycopyrrolate on the Muscarinic Effect of Neostigmine.

Sixty patients anesthetized with nitrous oxide-oxygen-halothane, who received pancuronium for the maintenance of muscular relaxation were divided into three group of twenty each. Ath the end of anesthesia, the patients of Group l received in 1.0mg atropine travenously and neostigmine 2.0mg: those in Group ll glycopyrrolate 0.4mg and neostigmine 2.0mg. and those in Group lll atropine 0.5mg, glycopyrrolate 0.2mg and neostigmine 2.0mg. After that the changes in heart rate, arrhythmsia, blood pressure and oropharyngeal secretions were observed and evaluated. The results are as follows: 1) The safety and effectiveness of all groups in antagonizing the muscarinic actions of neostigmine were demonstrated. 2) Glycopyrrolate produced a significantly smaller change in heart rate than atropine. 3) There were no significant difference in the indicence of severe tachycardia in all groups respectively. 4) Glycopyrrolate is superior to atropine in the antisalivary action. 5) There were no more advantages in combined use of atropine and glycopyrrolate than that of atropine or glycopyrrolate separotely.

5.The Incidence and Severity of Venous Air Embolism Determined by Transesophaseal Echocardiography in Hepatic Resection Using a Cavitron Ultrasonic Surgical Aspirator Dong.

BACKGROUND: A new technique resecting the hepatic parenchyma without inflow occlusion using a Cavitron Ultrasonic Surgical Aspirator (CUSA(R) ) reduces intraoperative blood loss and perioperative morbidity. This study was designed to identify the incidence and severity of venous air embolism (VAE) using transesophaseal echocardiography (TEE) in hepatic resection using CUSA(R) . METHODS: Forty patients undergoing hepatic resection using CUSA(R) of ASA class 1 and 2 were selected. After insertion of an epidural catheter for postoperative analgesia, all patients were anesthetized with sevoflurane in 50% air/O2. After the induction of anesthesia, A TEE probe was inserted into the esophagus. Blood pressure, heart rate, central venous pressure, end tidal CO2, and arterial carbon dioxide tension were recorded after induction, and during and after hepatic resection. During hepatic resection, an anesthesiologist evaluated the degree of VAE by transesophageal echocardiography in the 4-chamber view. RESULTS: The mean time of using CUSA(R) was 65.3 +/- 24.4 minutes. Of 40 patients, 9 had VAE grade I, 14 grade II, 14 grade III, and 3 grade IV. However, no significant difference was observed in hemodynamics or PaCO2 after induction, or during or after hepatic resection. The mean amount of blood loss was 887.0 ml +/- 598.8 ml and the mean transfused amount was 123.1 +/- 351.3 ml. CONCLUSIONS: All patients showed air embolism during hepatic resection with CUSA(R) . Serious complications associated with air embolism would occur in patients with an undiagnosed intracardiac right to left shunt. Therefore, meticulous monitoring by transesophageal echocardiography might be recommended in hepatic resection with CUSA(R) .

6.The Clinical Analysis of 79 Cases of Indigenous Malaria in Myongji Hospital during 4 Years.

BACKGROUND: In the Republic of Korea, there had been no reports about indigenous malaria cases since 1984 until a vivax malaria case was detected in 1993. Thereafter vivax malaria has been a reemerging disease in Korea, the number of cases of malaria increasing recently and the prevalent area being more wider. We suggested that we should take malaria into consideration as the differential diagnosis whenever we meet the febrile patients. We analyzed the cases of malaria who were treated in Myoungji hospital located in Goyang-Si during 4years. METHODS: We retrospectively analyzed data of 79 confirmed cases of malaria treated in the Myoungji hospital from January 1, 1998 to December 31, 2001. All of the patients had no histories of traveling abroad, drug abuse or blood transfusion. The clinical manifestation, hematologic abnormalities and prognosis of patients were reviewed. RESULTS: Seventy-nine cases of malaria were diagnosed as vivax malaria by blood smears. Vivax malaria was developed in Goyang-Si from May through November. Common symptoms were fever (100%), chilling (84.8%), headache (64.6%) and myalgia (55.7%). Splenomegaly was detected in 77.2% of cases by sonography. In 1998, Tertian fever pattern was most frequent. Since 1999, irregular and atypical fever patterns, such as almost-daily high fever or the every fourth or fifth-day fever, were increased in numbers. Laboratory findings included thrombocytopenia (92.4%), anemia (29.1%), leukopenia (25.3%) and leukocytosys (5.1%). CONCLUSION: ICases of indigenous malaria have been progressively increasing in the Republic of Korea. Therefore early diagnosis, treatment and prevention of malaria are very important. Fortunately, patients were well responsed to treatment.

7.Unilateral Tension Pneumothorax during General Anesthesia - A case report.

A caae is preaented of unilataral tension pneumothorax in a 5 year old patieat who underwent a cleft palate. This complication is belived to have been due to ventilation of high intratracheal pressure with high fresh gas flow (8L/min). It is suggested to use a correct sise tube, ventilation of a proper intratracheal pressure with appropriate fresh gas flow and to monitor breathing sound with precordial stethosco during E.N.T. operation.
8.Delayed Tear Clearance Induced by Botulinum Toxin Injection in Essential Blepharospasm and Hemifacial Spasm.

Assuming that botulinum toxin injection can release the involuntary contraction as well as reduce the tear drainage function, mediated by paralysis of orbicularis oculi muscles, then give rise to effect on tear clearance, Authors have examined 18 blepharospasm and 32 hemifacial spasm patients to evaluate the pre- and post- botulinum toxin injectional values of BUT, Schirmer test, and fluolescein clearance test 1, 2, which evaluate possibilities of delayed tear clearanee induced by botulinum toxin injection. Following treatment with botulinum toxin, results of BUT were increased significantly (p<0.0001), values of Schirmer test tended to increase without statistical significance. In fluorescein clearance test 1, length and concentration values were increased significantly(p<0.05), but in fluorescein clearance test 2, only length measurements were increased significantly(p<0.05). And also in blepharospasm cases of BUT values less than 4 second, both Schirmer test and fluorescein clearance test results were increased with statistical significance(p<0.05). We concluded that delayed tear clearance induced by botulinum toxin injection could be effective treatment for facial dystonias, especially combined cases with dry eye.

9.Involvement of NMDA Receptor and L-type Calcium Channel in the Excitatory Action of Morphine.

We studied the excitatory action of morphine on the responses of dorsal horn neuron to iontophoretic application of excitatory amino acid and C-fiber stimulation by using the in vivo electrophysiological technique in the rat. In 137 of the 232 wide dynamic range (WDR) neurons tested, iontophoretic application of morphine enhanced the WDR neuron responses to N-methyl-D-aspartate (NMDA), kainate, and graded electrical stimulation of C-fibers. Morphine did not have any excitatory effects on the responses of low threshold cells. Morphine-induced excitatory effect at low ejection current was naloxone-reversible and reversed to an inhibitory action at high ejection current. NMDA receptor, calcium channel and intracellular Ca2+ antagonists strongly antagonized the morphine-induced excitatory effect. These results suggest that changes in intracellular ionic concentration, especially Ca2+, play an important role in the induction of excitatory effect of morphine in the rat dorsal horn neurons.

10.Clinical Characteristics of Patients with Rheumatoid Arthritis Who have Sustained High Erythrocyte Sedimentation Rates after Clinical Remission.

OBJECTIVE: The aim of this study is to determine the clinical characteristics of patients with rheumatoid arthritis (RA) sustaining high erythrocyte sedimentation rate (ESR) despite clinical remission. METHODS: This cross-sectional study involved 91 patients, who visited a tertiary medical center. Patients underwent laboratory tests and a physical examination by a rheumatologist. The disease activity score (DAS) was calculated and patients who were in remission (defined as DAS28-CRP <2.6) were selected. Patients were divided into two groups: those with high and low ESRs (> or =40 and <40 mm/hr, respectively). RESULTS: DAS 28-CRP scores revealed that 61 of the 91 patients were in remission. Of these 61 patients, 15 and 46 were allocated to the high and low ESR groups, respectively. Compared to the low ESR group, the high ESR group had a longer disease duration (99.2+/-60.2 vs. 59.1+/-48.9 months), significantly higher white blood cell counts, and CRP levels, total modified Sharp radiographic joint scores, and erosion scores, as well as significantly lower hemoglobin, albumin and alanine aminotransferase levels. CONCLUSION: Patients who have high ESRs despite their remission status may show progressive radiographic change. In such patients, additional treatments that decreases the inflammation and prevents radiological progression should be considered.
