1.Analysis on monitoring results of heavy metals in rural soils in Bole of Xinjiang, 2017-2019

Objective To monitor heavy metals Pb, Cd, Cr, As, and Hg in agricultural land in Bole of Xinjiang, in order to understand the overall pollution status of rural soil environment and determine the environmental health hazards in rural areas of Bozhou, Xinjiang. Methods From 2017 to 2019, 4 administrative villages in 5 townships in Bole City in Bozhou Region were selected as monitoring points. The 5 to 20 cm deep topsoil of monitoring points was collected for a total of 60 samples. According to "Environmental Quality Standard for Soils" (GB 15618-2018), the pollution levels of Pb, Cd, Cr, As and Hg were evaluated by single index method and Nemerow index method. Results The content of Pb, Cr, As, and Hg in the heavy metals in the soil of Bozhou region from 2017 to 2019 were all lower than the screening value and regulatory value of soil pollution risk for agricultural land, while the content of heavy metal Cd in 13 soil samples (21.67%) exceeded the standard. The content difference in Pb, Cd, Cr, and As was statistically significant (P<0.05), while the difference in Hg was not statistically significant (P>0.05). The single pollution index Pi of Pb, Cr, As and Hg was less than 1, indicating a non-pollution state. However, the single pollution index Pi of Cd from 2017 to 2018 was higher than 1, with the highest value reaching 2.33. In 2017 and 2018, the comprehensive pollution index of rural soil samples was 1.67 and 1.06, and the evaluation result was light pollution, while in 2019, the comprehensive pollution index was 0.72, and the evaluation result was still clean. Conclusion The overall quality of rural environmental soil in some rural areas in Bozhou, Xinjiang was generally acceptable, but the heavy metal Cd pollution in some aeras deserves attention.
2.Monitoring results of veterinary drug residues in chicken and eggs in Xinjiang in 2019 - 2020

Objective To know the residues of 13 veterinary drug residues in chicken and eggs foods in some areas in Xinjiang. Methods A total of 170 chicken and egg samples were randomly selected from supermarkets and farmers' markets in seven cities in Xinjiang. Eleven quinolone antibiotics, two tetracycline antibiotics, ribavirin and metronidazole were examined for veterinary drug residues using liquid chromatography tandem mass spectrometry (LC-MS/MS) . Results The overall detection rate of veterinary drug residues in eggs and chicken were 20%(18/90)and11.25%(9/80). The overall over-standard rate were 18.89%(17/90)and 0(0/80). Veterinary drug residues in chicken are heavier than eggs. Veterinary drugs were detected and over-standarded in all seven cities in Xinjiang monitored. Conclusion The veterinary drug contamination in chicken and eggs in Xinjiang is relatively serious. It is recommended to strengthen the standardization of production and supervision to ensure food safety.
3.Simultaneous pancreas-kidney transplantation for type 2 diabetes mellitus complicated with endstaged renal disease in 1 case

We performed a simultaneous pancreas-kidney transplantation (SPK) for type 2 diabetes complicated with end-staged renal disease (ESRD) in March, 2007. The recipient was a 65-year old male, who suffered type 2 diabetes for 15 years and renal dysfunction for 5 years and other diabetic complications such as retinopathy and peripheral neuropathy. SPK was performed successfully for him, in which the kidney was placed in the left iliac fossa, while the pancreas in the right iliac fossa, with an entedc drainage for pancreas exocrine and a systemic drainage for endocrine. Serum C-peptide, creatinine and blood urea nitrogen reached normal levels on day 1,4 and 11 post-transplant, respectively. Blood glucose became stabilized gradually to normal level and therefore the injected insulin was stopped on day 16 post-transplant. Oral glucose tolerance test (OGTT) showed the function of grafted pancreas was normal after 3 weeks of transplant, and no transplant-related complications occurred. With the recipient followed up for 20 months, both his blood glucose level and renal function maintained normal without using insulin.
4.Diagnostic value of head-up tilt test in patients with cough syncope

Objective To investigate the diagnostic value of head-up tilt test (HUT) in cough syncope (CS).Methods Forty-seven outpatients or inpatients with CS and 79 patients with suspected vasovagal syncope(SVVS) due to syncope history in our hospital from January 2011 to May 2015 served as the observation group and control group respectively.HUT was performed in the two groups.The cough response during HUT,changes of systolic pressure and diastolic pressure caused by cough,HUT positive results and hemodynamic type were observed,Results In the observation group,4 cases(8.51%) were cough caused syncope,26 cases (55.32 %) were presyncope and 17 cases (36.17 %) had no symptoms,while 79 cases in the control group had no symptoms,the differences were statistically significant (P< 0.05).The total positive rate of cough response in the observation group was 63.83%,and the specificity was 100.00%.The systolic blood pressure had statistical difference between the two groups(.P<0.05).The diastolic pressure had statistical difference between the cases of non-symptoms with the cases of syncope and cases of presyncope in the observation group (P<0.05),but had no statistical difference compared with the cases of non-symptoms in the control group(P>0.05).The heart rate(HR) had no statistical difference among various groups(P>0.05).The HUT positive rate in the observation group was lower than that in the control group(P<0.05).The hemodynamic type had no statistical difference between the two groups(P>0.05).Conclusion Severe coughing during HUT may cause the blood pressure decrease,induces syncope or presyncope,can increase the diagnostic sensitivity of CS patients,which is very useful in the suspected diagnosis of CS patients,especially for CS patients with a unclear history.
5.Effects of micropulse laser photocoagulation under threshold on serum malondialdehyde and chemokine levels in patients with proliferative diabetic retinopathy

Objective:To investigate the effects of micropulse laser photocoagulation under threshold on serum malondialdehyde(MDA) and chemokine levels in patients with proliferative diabetic retinopathy(PDR).Methods:From October 2014 to February 2016, 102 patients with PDR who were treated in Lishui People's Hospital were selected and divided into two groups by random number table method, with 51 cases in each group.The patients in the control group were treated with traditional grating laser, and the patients in the observation group were treated with micropulse laser photocoagulation under threshold.The levels of MDA and chemokine were compared before and after treatment, and the changes of vision and clinical efficacy were observed.Results:The total effective rate of the observation group was 90.20%(46/51), which was higher than that of the control group [76.47%(39/51)](χ 2=19.585, P<0.01). Before treatment, there were no statistically significant differences in serum MDA, chemokine and visual acuity between the two groups(all P>0.05). After treatment, the serum MDA and chemokines in the observation group were (8.57±2.48)μmol/L and (1.58±0.51)ng/L, respectively, which were lower than those in the control group [(10.92±3.16)μmol/L and (2.83±0.92)ng/L]( t=4.178, 8.486, all P<0.05), which in both two groups after treatment were lower than those before treatment(all P<0.05). The visual acuity of the observation group was (0.66±0.19), which was higher than that of the control group(0.43±0.13)( t=7.135, P<0.05). Conclusion:Micropulse laser photocoagulation under threshold can effectively alleviate oxidative stress and reduce serum MDA and chemokine levels in patients with PDR.
