4.Arthroscopy after Meniscectomy of the Knee: The Posterior Horn in 40 Cases

Diagnosis of the disorders following menlscectomy is occasionally difficult, as ther has frequently been involved many causes other than the torn menisci. The retained fragments of the menisci have been blamed to be responsible for postoperative knee disorders. Arthroscopic examination was carried out in 40 knees after meniscectomy and the frequency of retained posterior horn in relation to the postoperative trouble was observed. 1. In 11 out of total 40 knees, the retained posterior horn could be diagnosed, consisting of 27.5% of all. 2. Only flve of these 11 knees showed some kinds of postoperative disorders,while the reat had no trouble. 3. Those knees in which the excision of the posterior horn was incomplete, were often associated with Irregular healing pattern of the local tissue, which might be closely related to the occurrence of the secondary Intraarticular change.

5.Mating Study of Microsporum canis Isolated in Korea.

A total of 135 strains of M canis, composed of 1R4 from the patients and 1 from an infected pet cat, were crossed with the 2 tester strains of eVannizzia otae VUT 77054 "/" and VUT 77055 "-" supplied by Dr. A. Hasegawa. These 134 human strains were originated from 77 casea of tinea capitis, 20 of tinea faciei and 37 of tinea corporis from Taegu city, Kyungpook and neighboring provinces during the period from December 1975 to August 1982. Out of 134 hurnan isolates, 112 (83. 6%) were of "-" mating type of N. otae and 22 (16.4%) were nonreactive. None were compatible with the + mating type. One strain from a pet cat proved also to be of type. The majority of the tested strains were revealed to be sexually degenerated.

6.Clinical and Mycological Observations on Tinea Corporis.

The number of patients with tinea corporis diagnosed on the clinical findings and KOH examination was 1,709 during the five yea.rs between January 1976 and December 1980, representing l.5% of the total dermatologic out-patients of 105,267 examined at Chilgok Catholic Dermatological Clinic, Daegu, Korea. The annual number of patients with tinea corporis was 84 (0.47% of the total outpatients) in 1976,115 (0.6%) in 1977, and 263 (1.30%) in 1980, but the figure increased markedly in 1976 reaching 616 (2.51%) and 631 (2.89%) in 1980. Of the 1,709 patients, males numbered 793 and females 916. Although the age of peak incidence was in the third decade in both sexes, males outnumbered females slightly before age 19 and, as age advanced, female predominance became marked. The seasonal distribution of patients was interesting: in summer between June and September when the climate is very humid and hot, the incidence was markedly high. From the 1, 709 patients, 1,293 organisms composms of 6 species were isolated and identified. The species isolated were, in order of decreasing frequency, Trichophyton rubrum (622 organisms, 48. 1% of total organisms isolated), T. mentagrophytes (486 organisms, 36. 2%), Microsporum canis (147 organisms, 11. 4%), Epidermophyton floccosurn (29 organisms, 2.2%), M. gypseum (23 organisrns, l.8%) and T. ferrugineum (4 organisms, 0. 3%).

7.A Study on Stroke Patients' ADL, Depression, Self-Efficacy and Quality of Life.

This study was a descriptive research in search of a nursing intervention scheme to improve stroke patients' quality of life by understanding the relationship of stroke patients' ADL. depression, self-efficacy and quality of life with various variables and identifying factors that affect their quality of life. Each subject was interviewed one to one based on a structured questionnaire. With regard to research tools, Modified Barthel Index (MBI: Fortinsky et al., 1981), which was translated by Choi, Hye-sook (996), was used in measuring ADL, Randloff's (1977) tool. which was translated by Choi, Soon-hee, was used in measuring depression, and the tool developed by Sherer et al. (1982), which is to measures self-efficacy under general conditions not limited to specific conditions, and modified by O, Bok-ja (1994) was used in measuring self-efficacy. The quality of life was measured using the scale of satisfaction of life developed by Diener et al.(1985). The results of this study were as follows: 1. The means of ADL of the subjects was 79.5+/-1.9, depression 26.8+/-0.4. self-efficacy 47.1+/-5.7, and the quality of life 12.3+/-.9. 2. The subjects' quality of life showed a statistically significant difference according to gender (t=7.9, p= .006), satisfaction with income (F= 5.8, p = .004), the burden of medical fee (F=3.7, p= .028) and the period of disease (F=2.8. p= .042). 3. With regard to relationship among ADL, depression, self-efficacy and the quality of life, ADL was in a relatively low positive correlation (r= .293, p= .003) with and the quality of life, depression in a high negative correlation (r=- .634. p= .000) with the quality of life. and self-efficacy in a positive correlation with the quality of life (r= .388. p= .000). 4. Factors that made a significant influence on the quality of life were depression (8=- .309. p= .001) and satisfaction with income (8=-2.611. p= .001). Based on these results, this study made following suggestions: 1. It is necessary to run rehabilitation programs to improve stroke patients' ADL, depression and self-efficacy. 2. It is necessary to perform research of monitoring stroke patients' quality of life in various areas using measuring tools.

BACKGROUND: No-reflow is a specific type of vascular damage occuring when removal of coronary occlusion dose not lead to restoration of coronary flow. There are three major explanations for the no-reflow phenomenon such as endothelial cell edema, microvascular plugging by platelets or thrombi and coronary occlusion by ischemic contracture of the myocardium. But detailed mechanisms of no-reflow phenomenon are not known. The objects of this study are to elucidate the possibility whether elevation of cytosolic Ca2+ concentration during ischemic cardioplegic period is mechanism of no-reflow phenomenon or not. METHODS: Changes in cytosolic Ca2+ concentration were measured under varying experimental condition. Free [Ca2+] in the cytosole [Ca2+]i of single rabbit coronary artery cells was measured with fluorescent Ca2+ indicator, Fura-2. RESULTS: Resting [Ca2+]i was 134.2+/-34 nM (n=43). When single cells were perfused with cardioplegic or ischemic cardioplegic solution, [Ca2+]i was significantly increased and degree of [Ca2+]i elevation was further augmented by ischemic cardioplegic solution. Pretreatment of sarcoplasmic reticulum emptying agent (20mM caffeine) had no effect on cardioplegia-induced [Ca2+]i change, but application of Ca2+ channel blocker (5x10-7M nifedipine) or an antagonist of Na+/Ca2+ exchange (5mM Ni2+ ) partially (nifedipine) or completely (nickel) inhibited the [Ca2+]i elevation. Pretreament of caffeine had no effect on ischemic cardioplegia-induced [Ca2+]i change, but application of nifedipine or nickel partially inhibited the [Ca2+]i elevation. Magnitude of ischemic cardioplegia-induced [Ca2+]i elevation was dependent on the Ca2+ concentration of perfusate from 0 to 2.5mM. When Ni2+ was added to reperfusion solution, recovery of ischemic cardioplegia-induced [Ca2+]i elevation was very rapid compared with control. CONCLUSIONS: From the above results, it may be speculated that ischemic cardioplegia-induced [Ca2+]i elevation may act as one of the mechanism of no-reflow phenomenon in rabbit coronary artery.

10.A Study on Assessment System for Nursing Bachelor Degree Program Outcomes: Focused on Communication Ability Improvement.

PURPOSE: The core of accreditation for nursing education is to improve the ability and quality of graduates through a CQI system based on outcomes. For program outcomes of nursing education accreditation criterion, it is necessary to understand the relationship among education assessment, objective and curriculum. METHODS: The focus of this study was on program outcomes and the assessment. First measures for improving the general problems of program outcomes associated with the assessment system were stated. Detailed measure for practicing program outcomes, the assessment system and the assessment tool using Rubrics were discussed. RESULTS: Detailed measure were established for practicing the program outcomes, assessment system and assessment tool, which makes use of rubrics, program outcome No 3 of the Korean Accreditation Board of Nursing Education. CONCLUSION: Therefore it is expected that the assessment system established in this study will allow universities to conduct program outcomes and assessment.
