OBJECTIVETo study the molecular mechanism of extracts from Euodia rutaecarpa on hepatotoxicity in mice.
METHODTotally 30 KM mice were divided into 3 groups and orally administrated extracts from E. rutaecarpa for consecutively 15 days. The expressions of Erkl/2, CDK8, CK1e, Stat3 and Src were detected by Western blotting method.
RESULTThe extracts from E. rutaecarpa could up-regulated Erkl/2, CDK8 and CK1e expressions (P <0.01) and down-regulate Stat3 and Src (P <0.01).
CONCLUSIONThe molecular mechanism of E. rutaecarpa on hepatotoxicity may be correlated with Erkl/2, CDK8, CKle, Stat3 and Src signal molecules.

3.Application of functional neck dissection for lower collar-shaped incision at hair margin behind ear in the treat-ment of differentiated thyroid cancer

Objective To explore role of functional neck dissection for lower collar-shaped incision at hair margin behind ear in the treatment of differentiated thyroid cancer. Methods The clinical data of 11 patients with thyroid papillary carcinoma who underwent func-tional neck dissection for lower collar-shaped incision at hair margin behind ear were retrospectively analyzed. Results The incisions of all patients were unseen in daily life and patients were satisfied with the postoperative appearances. No case of shoulder syndrome,postoperative local recurrence and distant metastasis was observed. Conclusion Functional neck dissection for lower collar-shaped incision at hair margin behind ear is effective in the treatment of differentiated thyroid cancer. Meanwhile it can obviously improve patients’ postoperative appearance and life quality,therefore it is worth clinical application.

Objective To explore the therapeutic effect of combining continuous venovenous hemofiltration (CVVH) with hemoperfusion (HP) on patients with septic shock.Methods A prospective study was conducted. Thirty-two patients with septic shock admitted to Changshu No.1 People's Hospital were enrolled, and they were divided into two groups according to random digits table. The observation group (17 cases) was given HP before CVVH, in the control group (15 cases), only CVVH treatment was carried out, and lasting for 5 days in both groups. The changes of pulse oxygen saturation (SpO2), central venous pressure (CVP), systemic vascular resistance index (SVRI), cardiac index (CI), extra-vascular lung water index (EVLWI) were observed before treatment and 5 days after treatment.Results Compared with those before treatment, the levels of SpO2, CVP, SVRI, CI were obviously elevated, and EVLWI was markedly decreased after treatment in two groups. Compared with those of control group, the degrees of improvement of above indicators in observation group were more prominent [SpO2: 0.966±0.035 vs. 0.939±0.036, CVP (mmHg, 1 mmHg = 0.133 kPa): 8.5±3.2 vs. 8.1±4.2, SVRI (kPa·s·L-1·m-2): 1 497.35±157.08 vs. 1 343.60±144.48, CI (mL·s-1·m-2): 120.36±15.34 vs. 106.69±12.33, EVLWI (mL/kg): 6.84±0.73 vs. 7.65±0.70, allP < 0.05].Conclusion Combing HP with CVVH for treatment of patients with septic shock can improve their prognosis more obviously than using CVVH alone.

9.Surgery of ruptured human hepatic cystic echinococcosis

Objective To investigate the evolution of surgical treatment and it's influence on prognosis in reptured liver cystic echinococcosis.Method Clinical data of 110 surgically treated cases of reptured liver cystic echinococcosis from January 1990 to December 2008 were retrospectively analysed,and these cases were divided into three groups by different surgical approach;Group A(22 cases):removal of internal capsule,with formaldehyde or hydrogen peroxide regional anthelminthic,with residual cavity side edge of lock stitching or greater omentum filling or drainage after residual cavity closure;Group B (65 cases):Removal of intemal capsule,with hypertonic saline regional anthelminthic,with external capsule extrahepatic partial resection,with residual cavity bile leakage sutured,and with residual cavity open drainage,Group C(23 cases):the total or subtotal external capsule stripping operation,with hypertonic saline regional anthelminthic and drainage.Result The rate of overall postoperative complications of residual cavity was 40.9%、16.9%、0.0%(P<0.05)respectively and the recurrence rate was 18.2%、4.6%、0.0%(P<0.05)respectively;drainage time needed was the longest in group A,and shortest in group C,whereas the operation time was the longest in group C and it costed higher blood loss.Intraabdominal implantation of the hydatid significantly decreased with hypertonic saline treatment.Conclusion Total or near total removal of the external capsule of the hydatic cyst and hypertonic saline treatment with postoperative external drainage is the therapy of choice for reptured liver cystic echinococcosis.
10.Chemical constituents of Rauvolfia verticillata.

The study on the Rauvolfia verticillata (Lour.) Baill., which belongs to Apocynaceae, was carried out to look for its chemical constituents and pharmacological activity. The isolation and purification were performed by chromatography on silica gel, Sephadex LH-20 and ODS (octadecyl silane) open column. The structures of obtained compounds were elucidated on the basis of physicochemical properties and spectral analysis. Three indole alkaloids and one acridone alkaloid were isolated from chloroform layer extract and identified as ajmalicine B (1), sandwicine (2), raunescine (3) and 7-hydroxynoracronycine (4) separately. Ajmalicine B (1) is a new compound belonging to indole alkaloid. Compound 4 as an acridone alkaloid was a new type compound isolated from Rauvolfia genus for the first time. We also did some biological activity research on the new type compound (4) to explore other pharmacological activities in addition to antihypertensive activity.
